    Evaluation of the learning environment and the perceived weakness of the curriculum: student perspective

    Background: Educational environment has been shown to have influence on learning outcome and affects student achievement, satisfaction, and success. The study was conducted to assess medical students’ perception of their learning environment and to explore areas of weakness within the educational environment.Methods: A mixed method of study, was performed using non-probability sampling at the Faculty of Medicine, Gezira University (FMUG), Sudan. An updated Arabic Dundee Ready Education Environment Measure (DREEM) was developed and administered to 854 students during the academic year 2016-2017.Results: Seventy five percent of students completed the inventory (638/854) and Sixty four percent (546/854) responded to the open-ended question providing comments and suggestions. The overall reliability coefficient alpha in this study was 0.914. The global score for this study was found positive (122/200). Students were most satisfied with the learning aspects, academic environment, and academic self-perception. However, they were unsatisfied with their teaching and social atmosphere (inadequate social support for stressed students, substandard teaching, overemphasised factual learning, unpleasant accommodation). The qualitative content analysis was performed and emerged with four themes: the physical environment, a number of students, pedagogical approaches and faculty-student communication.Conclusions: The study suggested the overall students’ perceptions of the educational environment in the FMUG were on the positive side. However, certain specific elements of the learning environment and educational programme need to be critically investigated and remedied. The updated Arabic DREAM can be used reliably in the context of medical education in Arabic speaking countries

    Urinary iodine level assessment during third trimester in a sample of Egyptian pregnant women and its relation to thyroid function

    Background: Inadequate intakes of iodine during pregnancy may cause thyroid dysfunctions that adversely affect pregnancy outcomes. Aim of the work was to evaluate the urinary iodine level as a marker of iodine status in a sample of Egyptian pregnant women during 3rd trimester and assess its relation to thyroid functions.Methods: This cross-sectional study was conducted on 100 pregnant females at their 3rd trimester aged (18-35) years. They were divided according to their urinary iodine concentration into 3 groups; Group (I): Pregnant females with deficient iodine (<150 μg/l), Group (II): Pregnant females with adequate iodine (150-249) μg/l, Group (III): pregnant females including who have above requirements (250-499 μg/l), and excessive (≥500 μg/l). TSH, free t4, free t3, Anti-Thyroglobulin (TgAb) and anti-thyroid perioxidase (TPOAb), medium urinary Iodine concentration (UIC) by ELISA and neck U/S were performed to all participants.Results: 18% of the pregnant women in our study had iodine deficiency during 3rd trimester (UIC<150 ug/l) whereas 55% of pregnant women had excess iodine level, and adequate iodine level was observed in 27%.  Serum TSH was significantly higher in group I with deficient iodine level (p value<0.01). All pregnant women included at group I were suffering from subclinical hypothyroidism. Serum TSH and thyroid volume were inversely correlated with urinary iodine among pregnant females at 3rd trimester (p value<0.01).Conclusions: Serum TSH and thyroid volume were inversely correlated with urinary iodine level among pregnant females at 3rd trimester

    Vocational Interests of Middle and High School Students in the UAE

    This paper investigated the vocational interests of students in the UAE and determined its relationship to gender and grade level. The Emirates Scale for Vocational Interests- Revised (ESVI-R) was used to attain the goal of the investigation. The ESVE-R was administered to 1920 students in the different emirates who were in the 9th, 10th, 11th, and 12th grades. Accordingly, the sample was comprised of 866 (45.1%) male and 1054 (54.9%) female students. The data were analyzed using the appropriate descriptive and inferential statistical protocols. The results showed that there are significant gender differences in most of the subscales of ESVI-R whereas there are significant differences in some of the subscales according to students' grade level. The study concluded that the vocational interests of the students demonstrated stability along grade levels and that there was a notable shift in the vocational interests of female students in the trajectory of seeking an equal opportunity with their male counterparts.     Study of lipid profiles high and normal body mass index in polycystic ovary syndrome women in Aljouf, Saudi Arabia

    Background: For women of reproductive age, polycystic ovary syndrome (PCOS) is the most prevalent endocrinological condition. Hyperandrogenism, persistent ovulatory dysfunction, obesity, and insulin resistance have all been associated with PCOS. However, recently PCOS is detected in high-body weight and normal-body-weight women. No information was available to evaluate specific lipid profiles. The goal of this study is to analyze the lipid profiles of women with polycystic ovarian syndrome who have high or normal body weight. Methods: This polycystic ovarian syndrome (PCOS) retrospective study was carried out between January 2021 and January 2022 at Sakaka’s Maternity and Children Hospital (MCH), Aljouf, Saudi Arabia. A total of 68 PCOS women were included in the study; we divided them into high (n=34) and normal body weight (n=34) according to the calculation of BMI. We obtain the test results of lipid profiles and demographic data from hospital record files. Results: We noticed changes but no significance in our research of lipid profiles in high and normal PCOS participants. The CHOL, TG, HDL, and LDL, 159.30±4.193, 97.89±7.140, 60.91±9.564, and 99.47±9.22 shown in the high BMI PCOS women respectively. While, 129.28±3.702, 83.69±4.49, 46.84±1.68 and86.53±4.36 were detected in normal BMI PCOS women respectively. There were none that were statistically significant, with the exception of cholesterol p=0.001. Conclusions: Our results show that POCS women with normal body weight and PCOS women with high BMI have different changes in their lipid profiles, but no significance has been found other than higher cholesterol levels. Therefore, losing weight can stop lipid profiles from altering, which may reduce difficulties in the future

    Hypertension and associated risk factors in some selected rural areas of Bangladesh

    Background:Although, Bangladesh has the ninth highest rate of age-standardized rates of deaths due to chronic diseases, mostly due to cardiovascular diseases and diabetes. There is scanty literature on the estimated burden, and the determinants of hypertension in the rural areas of Bangladesh.Methods:This facility based cross-sectional survey was conducted at sub-urban field research clinic from four unions of Araihazar upazilla of Narayangong district, Bangladesh. The analyses were based on 212 male and female respondents of more than 30 years of age. The prevalence of hypertension was the main outcome of interest.  Results:The study found that the prevalence of systolic hypertension among the respondents is 15.6% and that of diastolic pressure is 12.3%. Systolic pressure was found in an increasing manner with the increase of age and BMI. Tendency of increase systolic pressure was observed among those who took extra salt. Significant association was also found between increasing systolic pressure with the heart disease. No significant association was found between increasing systolic pressure with smoking habit, physical activity and vegetable intake and with the development of systemic diseases namely cardiovascular disease (CVD), diabetes mellitus and coronary obstetrics pulmonary disease.Conclusion:Suggestion on regular periodical health check-up especially including blood pressure monitoring and restriction of taking extra salt to all the people of age more than 30 years is recommended.

    Development of Novel 1,3-Disubstituted-2-Thiohydantoin Analogues with Potent Anti-Inflammatory Activity; In Vitro and In Silico Assessments

    Inflammation is the main cause of several autoimmune diseases, including type I diabetes, rheumatoid arthritis, bullous pemphigoid, paraneoplastic pemphigoid, and multiple sclerosis. Currently, there is an urgent demand for the discovery of novel anti-inflammatory drugs with potent activity but also safe for long-term application. Toward this aim, the present study reported the design, synthesis, and characterization of a set of novel 1,3-disubstituted-2-thiohydantoins derivatives. The anti-inflammatory activity of synthesized compounds was assessed against murine leukemia cell line (RAW264.7) by evaluating the cytotoxicity activity and their potency to prevent nitric oxide (NO) production. The results revealed that the synthesized compounds possess a considerable cytotoxic activity together with the ability to reduce the NO production in murine leukemia cell line (RAW264.7). Among synthesized compounds, compound 7 exhibited the most potent cytotoxic activity with IC50 of 197.68 μg/mL, compared to celecoxib drug (IC50 value 251.2 μg/mL), and demonstrated a significant ability to diminish the NO production (six-fold reduction). Exploring the mode of action responsible for the anti-inflammatory activity revealed that compound 7 displays a significant and dose-dependent inhibitory effect on the expression of pro-inflammatory cytokines IL-1β. Furthermore, compound 7 demonstrated the ability to significantly reduce the expression of the inflammatory cytokines IL-6 and TNF-α at 50 μg/mL, as compared to Celecoxib. Finally, detailed molecular modelling studies indicated that compound 7 exhibits a substantial binding affinity toward the binding pocket of the cyclooxygenase 2 enzyme.
