Le mécanisme varie d'un cas à l'autre, les épaules en abduction et en rétro-pulsion, coudes en extention et en supination a été décrite une seule fois dans la littérature.     Pseudarthrosis of the Carpal Scaphoid Treated with Non-Vascularized Corticocancellous Bone Graft

    Objectives: To carry out an epidemiological study and to assess the evolutionary profile of patients with scaphoid non-union treated by anterior non-vascularized corticocancellous bone graft. Introduction: The non-union is a serious complication of carpal scaphoid fractures. Untreated, it leads to instability and osteoarthritis of the carp. The most used technique is that of Matti-Russe employing a corticocancellous graft. Materials and methods: We report in this retrospective study the experience of the department of Orthopaedic Surgery & Traumatology II, Mohamed V Military Hospital, Rabat, in the treatment of non-unions of the carpal scaphoid by anterior non-vascularized corticocancellous graft in nine cases collected between January 2012 and March 2016. We identified six cases of stage IIA and three cases of stage IIB. All our patients were operated by the Matti-Russian technique followed by an immobilization by plaster cast cuff on three months. Results: All our patients achieved a union within an average of three months. The Mayo Wrist score was excellent in 66.6% of cases. Our results were very satisfying in 90% of cases, satisfying in 10%. Conclusion: The treatment of non-unions of the carpal scaphoid without necrosis or osteoarthritis, using anterior non-vascularized bone graft remains the most widely used technique. The presence of initial instability is a factor of poor prognosis, its correction reduces the risk of arthritis

    Interest of Surgical Treatment of Per-Trochanteric Fractures by Gamma Nail: About 100 Cases

    Gamma nail closed osteosynthesis has surpassed several methods previously used in the treatment of fractures in the trochanteric region.          The objective of the research was to carry out an epidemiological study and to assess the evolutionary profile of patients with per-trochanteric fracture treated with gamma nail.          Materials and methods. In this retrospective study, we report the experience of the Traumatology-Orthopedics Service II of the Military Teaching Hospital Mohamed V-Rabat, in the treatment of per-trochanteric fractures by gamma nail in 100 cases, collected between January 2011 and March 2016.          Results. The average age of our patients was 68 years, with a male predominance of 67%. The most reported etiology (92% of cases) was simple drop. Stable fractures were the most common (81%), with a predominance of simple per-trochanteric fractures. The average response time was 24 hours, the average duration of the surgical procedure was 45 minutes. The rate of infectious complications was 1%; the rate of mechanical complications was 1%. The functional results were excellent and good in 95% of cases. The average consolidation time was 8 weeks.          Conclusions. Trochanteric fracture is a delayed emergency which is seen primarily in the elderly. The gamma nail represents one of the latest improvements in implants for the treatment of these fractures. We report a series of 100 cases of trochanteric fractures treated surgically by gamma nail. The functional results were very encouraging compared to those reported in the literature. &nbsp

    Fat embolism syndrome (FES) remains a diagnostic dilemma on a world scale. It has a variable degree of presentation, which makes the diagnostic confirmation hard to obtain. FES is a life-threatening condition which is usually associated with orthopedic trauma, particularly long bones fractures whose early fixation helps in preventing it. It requires supportive care, and no specific treatment is needed. Here, we report the case of a FES in a 20 year-old male patient with right femoral shaft fracture following a motorbike accident, which is diagnosed by Gurd’s criteria and confirmed after exclusion of other diagnosis with similar clinical presentation

    Surgical treatment of traumatic isolated rupture of the long head of the biceps - A case report

    Background: Traumatic isolated rupture of the long head of the biceps (LHB) tendon is an uncommon injury that can result in significant pain and functional impairment. Surgical treatment is often required to restore normal shoulder function. Case presentation: A 40-year-old male presented with a chronic pain and weakness in his right shoulder. An MRI revealed a complete tear of the LHB tendon with retraction. Keyhole technique was performed, in which the tendon was reattached to the bicipital tuberosity. Results: The patient was discharged on the next day of the surgery and was able to return to normal activities within 12 weeks. At the final follow-up, the patient reported significant improvement in pain and function. Conclusion: This case illustrates the successful use of Keyhole technique in the treatment of traumatic isolated rupture of the LHB tendon. Keyhole technique is a safe and effective surgical technique for restoring normal shoulder function in cases of LHB tendon rupture
