    A study of serum sodium and potassium levels in subjects of acute myocardial infarction in a tertiary care hospital in Eastern India

    Background: Cardiovascular disease is one of the most common causes of morbidity and mortality around the world. In countries like India, there is difference in the lifestyle and behavioural risk factors among different areas. There is existence of rural-urban, Eastern-Western-Northern-Southern zonal differences. Studies done in other parts of India may not completely be applicable to Eastern Indian population. Serum electrolytes play an important role in maintaining electrophysiological homeostasis of the myocardial membrane, and alterations of these electrolyte levels can affect the pathogenesis, complications of myocardial infarction. Major serum electrolytes affecting the myocardial electrophysiological properties are sodium, potassium. Hence, this study was undertaken keeping in mind the miniscule lacunae in current knowledge especially pertaining to diseases in Eastern Indian population.Methods: 50 consecutive patients in the age group of 30-70 years admitted in the medicine department of Burdwan medical college with recent onset acute chest pain, diagnosed to be acute myocardial infarction were taken, their serum sodium, potassium levels were determined and these levels were compared among these patients with and without risk factors.Results: Serum sodium was decreased among patients with history of smoking, hypertension, BMI ≥25, complicated by atrial fibrillations. Serum potassium was decreased in patients with hypertension, BMI ≥25, ventricular premature complexes and increased in smokers, alcoholics.Conclusions: Electrolyte imbalances are common in patients of Acute Myocardial Infarction. Hypokalemia and hyponatremia were present and they were associated with cardiac complications. They adversely affect prognosis, so should be corrected

    As the number of catheterization laboratory procedures increase, there is more incidence of retention of foreign body in the form of torn catheters or devices or stents. Improvised snares are necessary in the absence of readymade snares. Retrieval of peripherally inserted central catheter from the pulmonary artery can be very challenging sometimes.     Interdepartmental dermatology: Characteristics and impact of dermatology inpatient referrals at a teaching hospital in Eastern India

    Background: Dermatology is primarily considered to be an outpatient-centered specialty. However, several inpatient admissions to other specialties require dermatologic consultation for optimum management. Aims: To analyze the causes of inpatient dermatology referrals, departments sending referrals, and impact of dermatology consultation on patient management. Materials and Methods: We conducted a cross-sectional study by analyzing the records of 486 patient referrals over a 4-year period. The demographic details, specialties requesting consultation, cause of referral, and dermatological advice have been recorded and analyzed. Results: Dermatology consultation changed the dermatologic diagnosis and treatment of almost two-thirds of patients. General medicine requested the maximum number of referrals, “skin rash” being the most common cause for referral. Accurate diagnosis on referrals was provided by only 30.2% of nondermatologists. Common dermatological disorders were often misdiagnosed by these physicians, and dermatology referrals had significant impact on the diagnosis and subsequent management of these patients. Conclusion: While dermatologic referral leads to improved patient care, there is a need for better training of nondermatologists enabling them to recognize and treat common dermatoses

    Angiokeratomas are a group of vascular ectasias that involve the papillary dermis and may produce papillomatosis, acanthosis and hyperkeratosis of the epidermis. Angiokeratoma circumscriptum is the least common variant among many types. Angiokeratoma circumscriptum neviforme is a still rarer variety of angiokeratoma, which is classically seen at birth. We report here a case of congenital, linear, unilateral, verrucous plaques on the leg of a young girl, diagnosed as angiokeratoma circumscriptum neviforme (ACN)
