    Evaluation of Metallo-β-Lactamase-Production and Carriage of bla-<sub>VIM</sub> Genes in Pseudomonas aeruginosa Isolated from Burn Wound Infections in Isfahan

    Background: Metallo-β-lactamase-production among Gram-negative bacteria, including Pseudomonas aeruginosa, has become a challenge for treatment of infections due to these resistant bacteria. Objectives: The aim of the current study was to evaluate the metallo-β-lactamase-production and carriage of bla-VIM genes among carbapenem-resistant P. aeruginosa isolated from burn wound infections. Patients and Methods: A cross-sectional study was conducted from September 2014 to July 2015. One hundred and fifty P. aeruginosa isolates were recovered from 600 patients with burn wound infections treated at Imam-Musa-Kazem Hospital in Isfahan city, Iran. Carbapenem-resistant P. aeruginosa isolates were screened by disk diffusion using CLSI guidelines. Metallo-β-lactamase-producing P. aeruginosa isolates were identified using an imipenem-EDTA double disk synergy test (EDTA-IMP DDST). For detection of MBL genes including bla-VIM-1 and bla-VIM-2, polymerase chain reaction (PCR) methods and sequencing were used. Results: Among the 150 P. aeruginosa isolates, 144 (96%) were resistant to imipenem by the disk diffusion method, all of which were identified as metallo-β-lactamase-producing P. aeruginosa isolates by EDTA-IMP DDST. Twenty-seven (18%) and 8 (5.5%) MBL-producing P. aeruginosa isolates harbored bla-VIM-1 and bla-VIM-2 genes, respectively. Conclusions: Our findings showed a high occurrence of metallo-β-lactamase production among P. aeruginosa isolates in burn patient infections in our region. Also, there are P. aeruginosa isolates carrying the bla-VIM-1 and bla-VIM-2 genes in Isfahan province

    Cool Roof Impact on Building Energy Need: The Role of Thermal Insulation with Varying Climate Conditions

    Cool roof effectiveness in improving building thermal-energy performance is affected by different variables. In particular, roof insulation level and climate conditions are key parameters influencing cool roofs benefits and whole building energy performance. This work aims at assessing the role of cool roof in the optimum roof configuration, i.e., combination of solar reflectance capability and thermal insulation level, in terms of building energy performance in different climate conditions worldwide. To this aim, coupled dynamic thermal-energy simulation and optimization analysis is carried out. In detail, multi-dimensional optimization of combined building roof thermal insulation and solar reflectance is developed to minimize building annual energy consumption for heating-cooling. Results highlight how a high reflectance roof minimizes annual energy need for a small standard office building in the majority of considered climates. Moreover, building energy performance is more sensitive to roof solar reflectance than thermal insulation level, except for the coldest conditions. Therefore, for the selected building, the optimum roof typology presents high solar reflectance capability (0.8) and no/low insulation level (0.00-0.03 m), except for extremely hot or cold climate zones. Accordingly, this research shows how the classic approach of super-insulated buildings should be reframed for the office case toward truly environmentally friendly buildings.The work was partially funded by the Spanish government (RTI2018-093849-B-C31).     Determinants of lifestyle behavior in Iranian adults with prediabetes: Applying the theory of planned behavior

OBJECTIVE: Prediabetic condition can lead to development of type 2 diabetes, especially in individuals who do not adhere to a healthy lifestyle. The aim of the present study was to investigate the socio-cognitive factors using the Theory of Planned Behavior (TPB) that may be associated with the choice of lifestyle in prediabetic patients. METHODS: A prospective study with one-month follow up was designed to collect data from 350 individuals with prediabetic conditions. A questionnaire was used to collect the information, including demographic variables, exercise behavior, food consumption, as well as the constructs of the TPB (attitude, subjective norms, perceived behavioral control, and behavioral intention) regarding physical activity and dietary choice. The correlations between TPB variables and the dependent variables (dietary choice, physical activity) were assessed using Spearman correlation and multiple regression models. RESULT: In total, 303 people participated. The mean age of the participants was 53.0 (SD 11.5) years and 42% were males. Significant correlations were found between all TPB constructs and both dependent variables (healthy eating and exercise behaviors) both at baseline and after one month (P < 0.01). The predictive validity of the TPB over time was proved for both dependent variables where past and future behaviors were significantly correlated with the constructs. Nearly 87% of the variance in exercise behavior and 72% of the variance in healthy eating behavior were explainable by TPB constructs. CONCLUSION: The TPB may be a useful model to predict behaviors of physical activity and dietary choice among prediabetic people. Therefore, it may be used to monitor lifestyle modification to prevent development of diabetes among people with prediabetic conditions

    Ability of biofilm production and molecular analysis of spa and ica genes among clinical isolates of methicillin-resistant Staphylococcus aureus

    Objective: This study aimed to evaluate the phenotypic and genotypic characterization of biofilm formation and spa and ica genes among clinical isolates of methicillin-resistant Staphylococcus aureus. Result: This cross-sectional study was performed on 146 Staphylococcus aureus isolates from hospitalized patients in Isfahan Province Hospitals. MRSA isolates were confirmed using disk diffusion test with oxacillin disk and amplification of mecA gene by PCR assays. Ability of biofilm production was evaluated targeting the icaA and icaD genes. Of 146 Staphylococcus aureus isolates, 24 (16.4) carried mecA genes and identified as MRSA strains. Strong ability of biofilm production was seen among 76.02 (111/146) S. aureus isolates and 87.5 (21/24) MRSA strains, respectively. Also, 75.0 (18/24) MRSA isolates carried icaA and icaD was not detected in these strains. Analysis of spa gene showed 70.83 (17/24) MRSA strains were spa positive. From which 14 and 3 strains identified with one band (150, 270, 300, 360, 400 bp) and two bands (150-300 bp), respectively. According to data obtained, the prevalence of MRSA isolates from Isfahan Province Hospitals is relatively high and a remarkable percentage of them show strong power in biofilm production. Also analysis of spa gene showed a fairly large diversity among MRSA strains. © 2020 The Author(s)
