Генетический анализ гена дистрофина у больных миодистрофией Дюшенна/Беккера мальчиков и у женщин носительниц в Республике Молдова за период с 1992-2012 гг.
National Centre of Reproductive Health and Medical GeneticsIntroducere. În cadrul Centrului Naţional de Sănătate a Reproducerii şi Genetică Medicală s-a efectuat analiza
mutaţiilor în gena distrofinei, folosind multiplex PCR, a probelor colectate în perioda 1992-2012. Acest studiu a inclus
206 familii cu risc pentru distrofia musculară Duchenne/Becker (DMD/BMD), necesitînd consultaţie medico-genetică.
Analiza ADN a fost efectuată la familiile cu cel puţin un copil bolnav de DMD sau la rudele apropiate ale acestuia.
Material şi metode. Studiul molecular a fost efectuat la 190 bolnavi DMD/BMD prin multiplex PCR, cu seturi de
primeri specifici care favorizează amplificarea a 20 exoni diferiţi, şi prin metoda analizei site-urilor polimorfe RFLP:
pERT87-8/Tag1, pERT87-15/BamH1, 16intron/Tag1.
Rezultate. Efectuarea amplificării „Multiplex” a genei distrofinei a permis identificarea a 81,5% din pacienţi cu
deleţii în genă. Cea mai frecventă regiune supusă ruperii a fost intronul 44. Cu o frecvenţă înaltă a fost identificată
deleţia exonului 48 (14%), exonului 45 (17%), exonului 47 (10%) şi a exonului 50 (10%). Din cele 206 familii cu risc
înalt de DMD/BMD 198 au fost găsite informative cel puţin după un site polimorfic pentru analiza ADN. Statutul de
purtător a fost confirmat la 81 % femei înrudite şi infirmat la 17% din ele (nivelul de încredere de 90-95%). La 2% din
femei a fost imposibil de stabilit statutul de purtător, familiile fiind neinformative. Şase pacienţi cu DMD/BMD au fost
diagnosticaţi prenatal prin analiza directă a deleţiilor sau prin testul RFLP.
Concluzii. Aplicarea ambelor metode de testare molecular genetică au asigurat detectarea a cca 94% de cazuri
informative. Diferenţele în frecvenţa deleţiilor exonilor implicaţi împreuna cu instabilitatea regiunilor genei distrofinei
observate, în comparaţie cu cele observate anterior în alte studii, probabil că sunt specifice populaţiei Republicii Moldova.
Variaţiile interpopulaţionale respective în pattern-urile deleţiilor genei distrofinei sunt discutate.Введение. В Национальном Центре Репродуктивного Здоровья и Медицинской Генетики был проведен
анализ мутаций гена дистрофина, используя мультиплексную ПЦР. В исследование вошли 206 семей высокого
риска Миодистрфии Дюшенна/Беккера, прошедшие медико-генетическое консультирование в Национальном
Центре Репродуктивного здоровья и медицинской генетики (Республика Молдова) с 1992 по 2012 годы. ДНК
анализ был проведен в семьях с минимцм одним больным мальчиком или у его ближайших родственников
Материал и методы. Прямой поиск протяженных делеций гена дистрофина проводился методом
мультиплексной ПЦР (МПЦР) у 190 больных мальчиков МДД/Б со специфичными олигопраймерами на 20
экзонов. Косвенная ДНК диагостика проводилась с использованием внутригенных полиморфных сайтов
pERT87-8/Tag1, pERT87-15/BamH1, 16intron/Tag1.
Результаты исследования. Методом МПЦР в 81,5% случаев в гене DMD у больных были обнаружены
протяженные делеции, захватывающие от одного до нескольких соседних экзонов. .Так, чаще всего точка
разрыва делеций наблюдается в интроне 44. Высокая частота делеций наблюдается у экзона 48 (14%), экзона 45 (17%), экзона 47 (10%) и экзона 50 (10%). Из 206 семей высокого рискоа МДД/Б 198 семей были информативны
хотябы по одному полиморфному варианту. для проведения косвенной диагностики. Гетерозиготное
носительство было установлено у 81 % женщин родственниц и отклонено у 17% (с вероятностью 90-95%). В
2% случае было невозможно установить носительство из-за неинформативности семьи. В шести случаях был
установлен диагноз МДД пренатально посредством прямого поиска делеций или методом ПДРФ.
Выводы. Применение этих двух методов молекулярной генетики позволяет повысить информативность
семьи до 94% Установленные различия в частотах делеционных экзонов, анализ нестабильных районов гена
дистрофина и сравление полученных результатов с данными исследований проведенных в других странах
позволяет говорить о популяционной особенности паттерна делеций в Республике Молдова. Обсуждены

Molecular genetic diagnosis of primary immunodeficiencies in the Republic of Moldova
Summary
Primary immunodeficiency diseases (PID) are a heterogeneous group of inborn
errors of immunity, the majority of which present in infancy and result in significant
morbidity and mortality. In Institute of Mother and Child, in Laboratory of Molecular
Human genetics research on primary immunodeficiency has been started in 2015.
Medium TREC (6565.2354 for 100000 cells) and KREC (8173.212 for 100000 cells)
concentrations for presumably healthy newborns from Moldova were counted. We
developed our proprietary TREC/KREC assay for newborn screening. A detailed
analysis of the clinical profiles, biochemical and molecular-genetic investigations and
outcome of the 3 children diagnosed with WAS in our laboratory during the period
2016-2019 was performed. During selective screening we found 9 cases of DiGeorge
type I Syndrome and one case of familial Mediterranean fever (NGS and Sanger
confirmed).
Molecular diagnosis of PID helps in genetic counseling and to make therapeutic
decisions including the need for a stem cell transplantation.Rezumatul
Imunodeficiențele primare (IDP) sunt un grup eterogen de erori înnăscute ale
imunității, dintre care majoritatea se manifestă în copilărie și duc la o morbiditate și
mortalitate semnificativă. În cadrul IMSP Institutul Mamei și Copilului, în Laboratorul
de genetică moleculară umană, cercetările privind imunodeficiențele primare au început
în anul 2015. Au fost cuantificate concentrațiile medii de TREC (6565.2354 pentru 100.000 de celule) și KREC (8173.212 pentru 100.000 de celule) pentru nou-născuții
prezumtiv sănătoși din Republica Moldova. A fost dezvoltat un test propriu
TREC/KREC pentru screening-ul nou-născuților. A fost efectuată o analiză detaliată a
profilurilor clinice, investigațiilor biochimice și molecular-genetice, precum și
prognosticul celor 3 copii diagnosticați în cadrul laboratorului cu sindromul WiskottAldrich în perioada 2016-2019. În timpul screening-ului selectiv au fost identificați 9
pacienți cu sindromul DiGeorge tip I și un caz de febră mediteraneană familială
(confirmate prin NGS și secvențierea Sanger). Diagnosticul molecular al IDP ajută la
consilierea genetică și la luarea deciziilor terapeutice, inclusiv pentru necesitatea unui
transplant de celule stem
Appreciation of short repeats number in neurogenetic disorders diagnostic
There are known many hereditary ataxias caused by short repeats expansions, most prominent of them being Huntington disease, Friedreich ataxia and a vast number of spinocerebellar ataxias (SCA’s).
These ataxias are very similar by clinical signs, being sometimes almost phenocopies, so a molecular-genetic diagnostic method is the only option for differential diagnostics.
There is also a well-known effect called genetic anticipation, when some alleles with a quite large, but still normal
number of repeats may further expand in next generations. It puts potential children of people harboring these
alleles at risk of disease development. And it is a cause why a screening of affected child’s parents and their siblings
is necessary.
A large number of publications shows that number of repeats most frequently directly correlates with age of disease
onset and speed of its progression. So, counting short repeats number may give not only diagnostic, but prognostic
information for affected persons and their relatives.
In this publication we propose a diagnostic approach based on a combination of agarose gel AFLP and capillary
electrophoresis based fragment analysis to detect short repeats expansion and calculate total repeats number for
multiple forms of SCA and Friedreich ataxia. As a test group a selection of patient’s DNA with proposed diagnosis
of SCA or Friedreich ataxia stored in LGMU biobank was used.Exista multe ataxii ereditare cauzate de expansiile a repetarilor scurte, dintre ele cele mai cunoscute sunt boala
Huntington, ataxie Friedreich si ataxii spinocerebelare (SCA).
Aceste ataxii sunt foarte similare in simptoame clinice fiind aproape fenocopii una de alta, asadar investigatiile
molecular-genetice sunt o singura optiune pentru un diagnostic diferential.
Este cunoscut si asa numit efectul anticiparii genetice, cand un alel cu un numar larg, dar normal de copii a repetarii tinde spre largirea numarului de copii a repetarii scurte in urmatoarele generatii. Acest fapt supune copii potentiali a acestor oameni riscului de dezvoltarea bolii corespunzatoare. Și aceasta este cauza necesetatii screeningului
de părinții a copiilor afectati și rudelor lor.
Un număr mare de publicații arată că numărul de repetări cel mai frecvent se corelează direct cu vârsta de debut a
bolii și viteza progresiei acesteia. Așadar, calcularea numarului de repatari scurte are nu numai un rol diagnostic
dar și prognostic pentru pacienți și rudele lor.
În aceasta publicație noi propunem o metoda diagnostica bazata pe AFLP în gel de agaroza și analiza fragmentilor bazata pe electroforeza capilara pentru a detecta expansie repetarilor scurte și de a calcula un numar total a
repetarilor pentru multe forme a SCA și ataxie Friedreich. Ca un grup de test a fost utilizata o selecție de ADN a
pacientilor cu propunerea dioagnozei de SCA sau ataxie Friedreich care au fost stocate în banca biologica LGMU.Существует множество наследственных атаксий, вызываемых экспансией коротких повторов, самыми
известными из которых являются хорея Хантингтона, атаксия Фридрейха и спиноцеребеллярные атаксии.
Данные атаксии имеют крайне схожие клинические проявления, являясь почти что фенокопиями друг друга.
Таким образом, молекулярно-генетические исследования играют ключевую роль в их дифференциальной
диагностике.
Существует также явление, известное как эффект предожидания, проявляющийся в дальнейшей экспансии
однажды удлинившихся аллелей повторов у следующих поколений. Этот факт полвергает потенциальному
риску заболевания детей носителей данных аллелей. Также это обуславливает необходимость скрининга
родителей больных детей и их родственников на предмет нахождения предрасположенных к экспансии
аллелей.
В большом количестве публикаций подмечается также частая зависимость возраста дебюта и скорости
прогрессирования заболеваний от количества коротких повторов в связанном с заболеванием гене. Таким
образом, для пациентов и их родственников подсчёт количества повторов имеет не только диагностическое,
но и прогностическое значение.
В данной публикации, мы предлагаем метод диагностики, основанный на комбинации AFLP анализа в
агарозном геле и фрагментного анализа посредством капиллярного электрофореза для определения
экспансии коротких повторов и подсчёта количества повторов как таковых у больных с подозрением
на различные формы СЦА и атаксию Фридрейха. В качестве тестовой группы использовались образцы
ДНК пациентов с предполагаемым диагнозом СЦА или атаксии Фридрейха, хранившиеся в биобанке
Лаборатории Молекулярной Генетики Человека Института Матери и Ребёнка, Кишинёв, Молдова
Sindromul Wiskott-Aldrich la copii - particularități de diagnostic
Summary.
Wiskott-Aldrich syndrome is a form of primary immune deficiency combined
with thrombocytopenia and for a long time it was thought it is a single one with such
symptoms, but now phenocopies are known. Another problem with this syndrome is a
common problem of all primary immune deficiencies — majority of medical workers
know very little about this group of diseases what leads to a very long times from the
debute of disease to the specific diagnosis. Because of that, we shortly describe criteria
for Wiskott-Aldrich and rekated disorders diagnostic and how we are performing
molecular genetic diagnostic of it.Rezumat.
Sindromul Wiskott-Aldrich este o formă de imunodeficiență primară combinată
cu trombocitopenie și multă vreme s-a crezut că este una singură cu astfel de simptome,
dar acum se cunosc fenocopii. O altă problemă cu acest sindrom, comună tuturor
deficiențelor imune primare - majoritatea lucrătorilor medicali știu foarte puțin despre
acest grup de boli, ceea ce duce la un timp foarte lung de la debutul bolii până la
diagnosticul specific. Din acest motiv, descriem pe scurt criteriile de diagnosticare a tulburărilor Wiskott-Aldrich și rekated și modul în care efectuăm diagnosticul genetic
molecular al acesteia
Exchangeable copper - a new diagnostic indicator for Wilson's disease
Introduction. Wilson”s disease (WD) biochemical markers
continue to evolve. Classical tests have their own limits
(tab.1), and they are often insufficient to diagnose
or exclude WD. The free copper was proposed as a
diagnostic test, but it was showed a large overlapping of
this parameter between non-WD subjects and WD
patients. New biomarkers are being investigated.
Purpose. The paper aims to analyze the bibliographic
data on the new tools for diagnostic in WD, like
exchangeable copper (CuEXC).
Material and methods. An advanced search was performed
in the PubMed, and ScienceDirect databases, using the
search English terms: ”Wilson's disease”, ”diagnostic test”
and ”relative exchangeable copper”.
Results. CuEXC is a new validated method for the direct
determination of labile copper that can be correlated with
the toxic fraction of copper and used to monitor treatment
in Wilson patients. The relative exchangeable copper (REC) -
the ratio of CuEXC/total serum copper is the best
biomarker for the diagnosis of WD showing 100% sensitivity
and 100% specificity. Studies confirm that a REC value
>18.5% appears to be a highly discriminatory tool to
differentiate WD between controls, presymptomatic
patients, heterozygotes, and patients with non-Wilsonian
liver disease, in cirrhosis and cholestasis, both in adults and
in children. Family screening in asymptomatic subjects
observed that REC determination significantly differentiated
subjects non-WD from WD patients with a cutoff of 15%.
Conclusions. CuEXC proved a helpful contribution in starting quickly the treatment without waiting for genetic testing
results. Being a tool with high sensitivity and specificity, the determination of REC can be useful, reliable, rapid, and
easy to set up to confirm or exclude WD in both adults and children, in carriers or asymptomatic patients
Wilson's disease: clinical evolution of moldovan patients

Wilson's disease (WD) is an autosomal
recessive genetic disorder associated with a high
mortality and disability rate. WD manifests as chronic
liver disease and/or neurological impairment due to
accumulation of copper in several tissues, principally
in the liver and brain (Fig.1). Early diagnosis and
therapy can result in a good prognosis of WD.
Purpose. To analyze the
clinical and laboratory
evolutions of WD and the
effects of the standard
treatments in Moldovan
patients with WD.
Material and methods.
15 patients (6 females and
9 males) with WD were
evaluated retrospectively
between 2018-2021. (Fig.2)
Results. The median age at diagnosis was 22 years
(2 – 36 years). The distribution by clinical fenotypes
is presented in Tab.1
Fibrosis analysis (by Fibroscan) revealed that: 6
patients - F2, 2 - F4, 2 - steatosis, 1 - F0. The treatment
consisted of D-penicillamine associated with
pyridoxine for all patients. At the end of the study, all
treated hepatic patients were asymptomatic. (Fig.3)
Conclusions. The study suggests that Wilson's disease must be ruled out in children older than two years
presenting with abnormal levels of hepatic enzymes because of the heterogeneity of symptoms and the
encouraging treatment results obtained so far
Cardiomyopathy secondary to Duchenne muscular dystrophy in children

Background: Cardiomyopathy (CM) associated with Duchenne muscular dystrophy (DMD) is a commonly recognized appearance of this neuromuscular
disease, significantly increased morbidity and mortality, as well as the necessity for cardiological management. CM in DMD is defined by left ventricular
(LV) systolic dysfunction and both atrial and ventricular dysrhythmias and is associated with higher mortality than other cases of pediatric dilated CMs.
Notwithstanding the high rate of cardiac involvement, patients are usually asymptomatic despite significant LV dysfunction, because of likely poor mobility
that masks the usual heart failure (HF) symptoms. Also, imagistic predictors are provided to be very helpful in defining early LV dysfunction, especially
electrocardiogram and cardiac imaging (transthoracic echocardiography, speckle-tracking, cardiac magnetic resonance) are used to detect the onset and
progression of dilated cardiomyopathy (DCM) in DMD.
Conclusions: As most DMD patients are asymptomatic for a long time of their life, so identifying predictors of HF is crucial to support these patients.
Ventricular dysfunction based on the ejection fraction (EF) measurement helps to choose therapy. In the case of early DCM (LVEF≥50%) the great
purpose is to prevent ventricular dysfunction incipience with first-line HF therapy with Angiotensin-converting-enzyme inhibitors (ACE-I) or angiotensin
receptor blockers (ARBs). Current guidelines recommend the use of conventional HF medication in case of disease progression and DCM with MidRange Reduction of LV EF (40-49%). The therapeutic approach for patients with DCM and severe ventricular dysfunction (<40%) has been studied less
profoundly and contemporary guidelines recommend all drugs used for HF treatment
First manifestation and evolution of early left ventricular dysfunction in children with Duchenne muscular dystrophy

Chisinau, the Republic of MoldovaBackground: Standard pediatric cardiology examinations and echocardiography fail to discover when the cardiomyopathy will occur in patient with
Duchenne muscular dystrophy (DMD). Noninvasive markers are needed to fill this gap.
Material and methods: This cohort study included a total number of 30 children (21 children (70%) with DMD and 9 (30%) healthy children. Blood
samples were used for biochemical (level of creatine kinase, creatine kinase-MB, lactate dehydrogenase) and miRNA (presence of miR133a 3p, miR133b
3p, miR206 3p, miR208a 3p, miR208b 3p) analysis. All patients underwent partial conventional echocardiography ECOCG and Speckle Tracking.
Results: The children in the working group presented compared to healthy children: FCC values increased by 15 (71%) vs 2 (22%), high levels of CK,
CK-MB, LDH, which is characteristic for the disease and reflects its stage. Also, there is a decrease in systolic function indicators in the working group:
mean FE 59 ± 3.8 %, and GLS: -16.2 ± 3.1%. MiRNA analyses confirmed the presence of miR133a 3p, miR133b 3p, miR206 3p, miR208a 3p, miR208b
3p in both working and control group.
Conclusions: For the first time in the Republic of Moldova, we developed and adapted protocols for RNA extraction from human blood, performing
screening of specific miRNA in the serum of patients with DMD and healthy children. Also, altered LV strain notwithstanding a normal or mildly modified
LVEF represents an essential viewpoint for prospective pediatric drug trials in DMD-related cardiomyopathy prevention

domeniul biologiei moleculare şi celulare încearca să descopere multe mistere a bolii Wilson. În timpul actual există un
interes crescut în cercetarile legate cu fi ziologia metalelor. În timpul de faţă, noi observăm discuţii fierbinţi în cercurile
ştiinţifi ce cu privire la utilizarea preparatelor ce conţin Zn la tratarea bolii Wilson. Lucrarea dată decernează studiile
activităţii funcţionale a liniilor de celule CHO, ce se expresează în diferite mutaţii ATP7B prin influenţa anumitor soluţii cu concentraţie determinată de Cu şi Zn şi determinarea citotoxicitaţii legăturilor date.
Material şi metode. Au fost utilizate culturile celulare din colecţia germană de culturi celulare (DSMZ). Folosind
metoda transducţiei retrovirale, sau obţinut 12 tulpini celulare CHO, ce se expreseaza în diferite mutaţii ATP7B. Mutaţiile se localizează în domeniul de legatură cu Cu (n=2), TM (n=4), TGE bucle (n=1) legături de bucle ATP (n=5)
gene ATP7B. Pentru aprecierea citotoxicităţii se foloseşte testul MTT.
Rezultate. În rezultatul cercetărilor noastre, noi am obţinut următoarele date: concentraţia soluţiei de Zn 500mikroM reprezintă o concentraţie toxică procent de moarte a celulelor se echivalează cu 0,22%. Concentraţia soluţiei de
50microM stimulează diviziunea celulară. Procentul de celule CHO ce au supravieţuit, şi ce expresează gena ATP7B,
în soluţie de 1mM de Cu creşte pîna la 20% la adăugarea în soluţia dată 50microM de soluţie cu Zn. Celulele CHO, ce
nu expresează gena ATP7B îşi măresc cu 57% capacitatea de supravieţuire în concentraţia de 0,5mM CuCl2 şi 50 Mm
ZnCl2 în mediul de cultură.
Concluzii. Ionii de Zn preîntîmpină toxicitatea ionilor de Cu fără dependenţă de faptul dacă se expresează sau nu
gena ATP7B şi independent faţă de ce fel de mutaţie-gravă sau medie după forma clinică.Введение. Болезнь Вильсона является отличным примером современного поступательного развития
медицинской науки. Современные исследования в области клеточной и молекулярной биологии пытаются
раскрыть многие загадки болезни Вильсона. В настоящее время существует повышенный интерес к
исследованиям, связанных с физиологией металлов. В настоящее время, мы наблюдаем горячие дебаты в
научных кругах вокруг применения препаратов цинка при лечении болезни Вильсона. Данная работа посвящена
исследованию функциональной активности СНО клеточных линий, экстрессирующих различные мутации ATP7B при воздействии определенных концентраций растворов Cu и Zn и определение цитотоксичности данных
соединений.
Материал и методы. Культуры клетки были использованы из Немецкой Коллекции Культур Тканей (DSMZ).
Используя метод ретровирусной трансдукции, были получены 12 СНО клеточных линий, экспрессирующие
различные мутации ATP7B. Мутации находятся в медь-связывающем домене (n = 2), TM (n = 4), TGE петли
(n = 1), АТФ-связывающей петли (п = 5) гена ATP7B. Для оценки цитотоксичности использовался МТТ-тест
Analiza caracteristicilor clinice și molecular-genetice ale sindromului Wiskott-Aldrich și trombocitopenia X-linkată

Wiskott-Aldrich syndrome is a rare X-linked disorder characterized by microthrombocytopenia, eczema, and recurrent infections. It is caused by mutations of the WAS
gene which encodes the WAS protein (WASp) – a key regulator of actin polymerization in
hematopoietic cells. Mutations within the WASp gene result in a wide heterogeneity of clinical disease, ranging from ‘classical WAS’ to mild asymptomatic thrombocytopenia (X-linked
thrombocytopenia [XLT]), or congenital neutropenia (X-lined neutropenia [XLN]).
Case presentation. This present paper reports a phenotypical and laboratory description of
two children diagnosed with WAS and one child diagnosed with XLT. The first case was a six
months old male with septicemia, thrombocytopenia, eczema and petechial rash. The second
case was a 2 years old boy presenting with complaints of recurrent infections, eczema and
thrombocytopenia with small platelet size. The third case was a 16 years old boy who presented with thrombocytopenia and recurrent sinopulmonary infections.
Conclusions. Due to a wide spectrum of clinical findings, the diagnosis of WAS/XLT should
be considered in any male patient presenting with petechiae, bruises, and congenital or
early-onset thrombocytopenia associated with small platelet size.Introducere. Sindromul Wiskott-Aldrich este o afecțiune rară X-linkată, caracterizată prin

clinice, variind de la „WAS clasic” la trombocitopenie asimptomatică ușoară (trombocitopenie X-linkată [XLT]) sau la neutropenie congenitală (neutropenie X-linkată [XLN]).
Prezentarea cazului. Este raportată descrierea, fenotipică și de laborator, a doi copii diagnosticați cu WAS și a unui copil diagnosticat cu XLT. În primul caz, un băiat în vârstă de șase
luni, cu septicemie, trombocitopenie, eczemă și erupții de tip peteșii. În al doilea caz, un băiat
de 2 ani, care a prezentat acuze de infecții recurente, eczemă și trombocitopenie, cu dimensiune mică a trombocitelor. Iar în al treilea caz, un băiat de 16 ani, care s-a adresat cu acuze
de infecții sinopulmonare recurente și trombocitopenie.
Concluzii. Datorită spectrului larg de manifestări clinice, diagnosticul WAS/XLT trebuie luat
în considerare la orice pacient de sex masculin, care prezintă erupții de tip peteșii, echimoze
și trombocitopenie congenitală sau cu debut precoce, asociată cu o dimensiune mică a trombocitelor
