    Fogarty catheter for OLV of a neonate

    Here, we report two cases involving a neonate and child in which a slip joint section was used to thread a Fogarty catheter into the endotracheal tube for one-lung ventilation (OLV). Both the neonate and infant required OLV, and were placed under general anesthesia. A Fogarty catheter was used for OLV. The Fogarty catheter was passed into the intraluminal side of the endotracheal tube through a slip joint section. OLV was maintained successfully without severe air leakage or Fogarty catheter displacement. The neonate had been intubated pre-operatively with a 3.5-mm inner diameter endotracheal tube, and we used that tube. These cases indicate that the technique can be applied to pre-operatively intubated patients and does not require surgeons to exchange endotracheal tubes.     Variable Shape Attitude Control Demonstration with Microsat Hibari

    This paper presents the ongoing feasibility study and bus system for microsatellite “Hibari”. The main technical missions for Hibari is called “Variable Shape Attitude Control (VSAC)”. This VSAC is based on an idea to utilize a reaction torque when a part of the satellite structure, for example, solar array paddles is appropriately rotated by actuators. The previous research concluded that VSAC successfully achieved the rapid maneuvering while maintain the high attitude stability against disturbances [1], and thus, it can be applied to a variety of advanced attitude control missions. Hibari project also aims at its application to astronomical mission requiring high pointing stability and agile maneuvering. This paper is mainly comprised of 3 parts: detail mission statement, ongoing feasibility studies and bus system configuration. First, we mention the mission requirement and detail mission sequence for both technical and science missions. Second, we show the ongoing feasibility studies to confirm that all mission requirement is satisfied by VSAC. Third, this paper describes each subsystem configuration to meet the system requirement stated in the mission’s section. And then, we wrap up in the conclusion section and stated the future study for advanced VSAC use in the end

    Development of Attitude Sensor using Deep Learning

    A new method for attitude determination utilizing color earth images taken with COTS visible light camera is presented. The traditional earth camera has been used for coarse attitude determination by detecting the edge of the earth, and therefore it can only provide coarse and 2-axis information. In contrast, our method recognizes the ground pattern with an accuracy of sub-degrees and can provide 3-axis attitude information by comparing the detected ground pattern and the global map. Moreover, this method has advantages in the size, mass and cost of the detector system which consists of a cheap optical color camera and a single board computer. To demonstrate the method in space, we have developed a sensor system named “Deep Learning Attitude Sensor (DLAS)”. DLAS uses COTS camera modules and single board computers to reduce the cost. The obtained images are promptly analyzed with a newly developed real-time image recognition algorithms

    Combined experimental and theoretical studies on glasslike transitions in the frustrated molecular conductors θ\theta-(BEDT-TTF)2MM_2MM'(SCN)4_4

    We present results of the coefficient of thermal expansion for the frustrated quasi-two-dimensional molecular conductor θ\theta-(BEDT-TTF)2_2RbZn(SCN)4_4 for temperatures 1.5 K T\leq T \leq 290 K. A pronounced first-order phase transition anomaly is observed at the combined charge-order/structural transition at 215 K. Furthermore, clear evidence is found for two separate glasslike transitions at TgT_{\mathrm{g}} = 90-100 K and TgT_{\mathrm{g}}^\dagger = 120-130 K, similar to previous findings for θ\theta-(BEDT-TTF)2_2CsZn(SCN)4_4 and θ\theta-(BEDT-TTF)2_2CsCo(SCN)4_4, reported in T. Thomas et al., Phys. Rev. B 105, L041114 (2022), both of which lack the charge-order/structural transition. Our findings indicate that these glasslike transitions are common features for the θ\theta-(BEDT-TTF)2MM_2MM^\prime(SCN)4_4 family with MM = (Rb, Cs) and MM^\prime = (Co, Zn), irrespective of the presence or absence of charge order. These results are consistent with our model calculations on the glasslike dynamics associated with the flexible ethylene endgroups of the BEDT-TTF molecules for various θ\theta-(BEDT-TTF)2MM_2MM^\prime(SCN)4_4 salts, predicting two different conformational glass transitions. Moreover, calculations of the hopping integrals show a substantial degree of dependence on the endgroups' conformation, suggesting a significant coupling to the electronic degrees of freedom. Our findings support the possibility that the glassy freezing of the ethylene endgroups could drive or enhance glassy charge dynamics.Comment: 11 pages, 10 figures, 1 tabl

    Group cognitive behavioural therapy (GCBT) versus treatment as usual (TAU) in the treatment of irritable bowel syndrome (IBS): A study protocol for a randomized controlled trial

    Background: Irritable bowel syndrome (IBS) is a common disease that affects the quality of life (QOL) and social functioning of sufferers. Visceral anxiety is currently considered a key factor in the onset and exacerbation of IBS, and cognitive-behavioural therapy (CBT) targeting visceral anxiety is thought to be effective. However, access to CBT is limited due to the lack of trained therapists, the substantial time required for therapy and the associated costs. Group CBT (GCBT) may solve some of these problems. We have therefore planned this trial to examine the efficacy of GCBT for IBS. Methods: The trial is a two-armed, parallel group, open label, stratified block randomized superiority trial. The study group will consist of 112 participants (aged 18–75 years) with IBS (Rome-III or IV criteria). Participants will be randomly allocated 1:1 to (i) the intervention group: ten-week GCBT plus treatment as usual (TAU) or (ii) the control group: waiting list (WL) plus TAU. The co-primary outcomes are the change in IBS severity or disease-specific quality of life from baseline to week 13 which is 1 month after the end of treatment. The efficacy of GCBT for IBS will be examined through mixed-effects repeated-measures analysis. Discussion: GCBT, if found effective, can address the issues of the shortage of therapists as well as the time required and the costs associated with individual CBT. Clinically, the findings will help make effective CBT programmes accessible to a large number of distressed IBS patients at lower costs. Theoretically, the results will clarify the relationship between IBS and psychological stress and will help elucidate the underlying mechanisms of IBS. Trial registration: UMIN, CTR-UMIN000031710.     A mid term comparison of open wedge high tibial osteotomy vs unicompartmental knee arthroplasty for medial compartment osteoarthritis of the knee

    <p>Abstract</p> <p>Background</p> <p>The choice of surgical treatments for unicompartmental osteoarthritis (OA) of the knee is still somewhat controversial. Midterm results from cases treated using unicompartmental knee arthroplasty (UKA) or open wedge high tibial osteotomy (OWHTO) were evaluated retrospectively.</p> <p>Methods</p> <p>Twenty-seven knees of 24 patients with varus deformities underwent OWHTO and 30 knees of 18 patients underwent UKA surgeries for the treatment of medial compartmental osteoarthritis (OA). The KSS score, FTA, range of motion and complications were evaluated before and after surgery.</p> <p>Results</p> <p>The preoperative mean KSS scores were 49 points in the OWHTO group and 62 in the UKA group which improved postoperatively to 89 (excellent; 19 knees, good; 8 knees), and 88 (excellent; 25, good; 4, fair; 1), respectively. There was no significant difference between the OWHTO and UKA scores. Seventeen patients in the OWHTO group could sit comfortably in the formal Japanese style after surgery. The preoperative mean FTA values for the OWHTO and UKA groups were 182 degrees and 184, and at follow-up measured 169 and 170, respectively. In the UKA group, the femoral component and the polyethylene insertion in one patient was exchanged at 5 years post-surgery and revision TKAs were performed in 2 cases. In the OWHTO group, one tibial plateau fracture and one subcutaneous tissue infection were noted.</p> <p>Conclusions</p> <p>Treatment options should be carefully considered for each OA patient in accordance with their activity levels, grade of advanced OA, age, and range of motion of the knee. OWHTO shows an improved indication for active patients with a good range of motion of the knee.</p
