    Milk and milk products consumption patterns of elderly consumers in Burdur province

    Bu çalışma, Ege Üniversitesi Sağlık Bilimleri Enstitüsünde Zeliha Fulden Saraç'ın danışmanlığında Deniz Say Şahin tarafından hazırlanan "Burdur İlindeki 65 Yaş ve Üzeri Tüketicilerin Süt ve Süt Ürünleri Tüketim Alışkanlıkları" adlı yüksek lisans/doktora tezine dayanılarak hazırlanmıştırBu çalışma Burdur ili ve ilçelerinde ikamet eden 65 yaş ve üzerindeki tüketicilerin süt ve süt ürünleri tüketim alışkanlıklarını belirlemek amacı ile yapılmıştır. Gelir düzeyi düştükçe yaşlıların pastörize ve UHT süt tüketimleri ve süt ve süt ürünlerine yaptıkları harcamanın düştüğü ve gruplar arasındaki farkın istatistiksel olarak da anlamlı olduğu saptandı (p<0.001). Gelir gruplarına göre süt satın alınan yerlerin sıralaması yapıldığında da tercih edilen yerlerin farklılık gösterdiği ve bu farkın istatistiksel olarak da anlamlı olduğu (p<0.005) saptandı. Araştırmamıza katılan bireylerin tercihen en çok (%35.2) sütü sabah saatlerinde ve sonbahar ayında (%26.75) tükettikleri ve sütü en çok (%50.5) içme sütü olarak kullandıkları saptanmıştır. Tüketicilerin açık süt alırken en çok fiyatına (%27.02), sterilize süt alırken ise en çok son kullanma tarihine (%41.45) dikkat ettikleri görülmüştür. Tüm sonuçlar değerlendirildiğinde 65 yaş ve üzeri tüketicilerde gelir düzeyi düştükçe, fiyatı daha ucuz olan açık ürünlerin daha çok tercih edildiğini ve ürünün tadının da satın alırken fiyatı kadar etkili olduğunu saptadık. Bu yaş grubu tüketiciler için aroması ve besin içeriği daha özelleştirilmiş olan ürünlerin ve hatta 65 yaş ve üzerindeki bireylerin süt tüketimini artırmak için yapılacak özel indirimlerin tüketim alışkanlıklarını ve miktarlarını değiştirebileceği değerlendirilmiştir.The aim of this study was to determine the 65 aged and over consumers’ milk and milk products consumption who living in Burdur province and district. Pasteurized and UHT milk consumption and their spending on milk and milk products were decreased of the elderly people who have the lower income and the difference between groups were found to be statistically significant (p <0.001). The places where the milk was purchased, were different among income groups and the difference between groups were found to be statistically significant (p <0.005). The people who joined our research, they were mostly prefer using milk (35.2%) during the hours morning and fall months (26.75%) and they consume milk mostly (50.5%) drinking milk. The most consumers were found to be careful the price of fresh milk (27.02% ), while buying sterilized milk, they were pay attention the expiration date (41.45%). All results are evaluated over the 65 years and older consumers, the lower income level of people preferred cheaper and outdoor products and the taste of product as effective as the price while purchased it. According to us the production of aroma and nutrient content more customized products will increase the consuption and we believe that the consumption habits and quantity will increase when the special discount for the milk and milk products for this age grou

    A Case of Denosumab-associated Hyperparathyroidism: Differential Diagnosis Challenge

    Denosumab is a relatively new medicine that has become the second option in the treatment of bisphosphonate-resistant or intolerant osteoporosis. An 85-year-old female patient was referred to the outpatient clinic of our department with a complaint of waist pain. She stated that her waist pain started 2 weeks before her visit and she had taken the fourth denosumab injection. Her laboratory results revealed normal serum calcium, phosphorus, and vitamin D levels (8.8 mg/dL, 3.1 mg/dL, and 32 ng/L, respectively) before denosumab injection. She had a sensitive point with pain on her lumbar vertebrae. Lower extremity movements were painful and muscle strength was bilaterally reduced. Vital signs and other systemic examinations were normal. Her laboratory tests for serum parathormone (PTH), calcium, phosphorus, and vitamin D levels showed 487 ng/L, 8.6 mg/dL, 0.6 mg/dL, and 30 ng/mL, respectively. She did not have any chronic renal and other bone metabolism diseases, except for osteoporosis. Her X-ray revealed a loss of height in L3 lumbar vertebrae. The spinal magnetic resonance indicated an acute fracture. However, we could not explain hyperparathyroidism in the absence of hypocalcemia and low vitamin D levels. Therefore, we started to investigate primary hyperparathyroidism in the patient. Neck ultrasonography did not show any associated abnormal findings with the parathyroid glands. Parathyroid scintigraphy resulted in normal parathyroid gland activity. During her follow-up, the PTH level decreased with time. Spontaneous regression of PTH led us to suspect an association between denosumab injection and hyperparathyroidism. Mazokopakis (1) reported a similar case in the literature in 2018, reporting a 62-year-old female patient with normal electrolytes and a high PTH level after three months of denosumab injection. The hypocalcemic effect of denosumab combined with inadequate oral calcium and vitamin D intake may have triggered exaggerated PTH secretion

    Sarcopenia prevalence and the quality of life in older adults: A study from Turkey’s east

    Aim: Sarcopenia as a geriatric syndrome decreases quality of life and increases functional dependency, hospitalization and mortality. The aim of this study is to investigate the prevalence of sarcopenia among community-dwelling people aged 65 years and older in Muş province and to examine its effects on quality of life. Materials and Methods: In this study we have interviewed 371 community-dwelling older adults. A total of 186 older adults from the rural area and 185 older adults from Muş city center were enrolled in the study. Data was collected through face-to-face interviews conducted in the participants’ homes by trained researchers. The presence of sarcopenia, socio-demographic characteristics (place of residence, gender, age), presence of chronic diseases, polypharmacy, body mass index, and quality of life with Short Form-36 were determined. Muscle mass, handgrip strength and gait speed were determined for defining sarcopenia. Results: The mean age of participants was 72.3 ± 6.8 years (65-100) and 46.1% of them were females. The prevalence of sarcopenia was 11.8% among older people residing in the rural area whereas the prevalence was 21.6% among older individuals living in the city center (p = 0.012). Sarcopenic patients were older, more underweight, hypertensive, and had lower calf circumference, gait speed, and grip strength in males than non-sarcopenic patients (p <0.0001, p <0.0001, p = 0.005, p <0.0001, p = 0.001, p <0.0001, respectively). There were important distinction between sarcopenic individuals and non-sarcopenic individuals in terms of physical function, physical role weakness and emotional role weakness (p = 0.021, p = 0.006 and p = 0.009, respectively). Conclusion: Sarcopenia is very common among older adults, especially in the city center, and the presence of sarcopenia has detrimental effects on the quality of life

    Calcium, vitamin D supplements with or without alendronate and supragingival calculus formation in osteoporotic women: a preliminary study

    Objectives: Long-term calcium intake is related to the formation of urinary stones. Structure and composition of kidney and gallstones are similar to dental calculus. Saliva is the source of calcium for supragingival dental calculus formation. The aim of this preliminary study was to evaluate the possible effects of long-term calcium and vitamin D supplementation with or without alendronate administration on salivary electrolyte concentrations and supragingival calculus formation in osteoporotic women. Methods: Thirty-one female patients with osteoporosis for at least 3 years participated in this study. Eighteen women were taking calcium plus vitamin D plus alendronate, while 13 women were taking only calcium plus vitamin D supplements. Eleven systemically healthy women volunteered for the control group. Whole saliva samples were collected from all women before initiation of any periodontal intervention. Plaque index, probing depth, clinical attachment level, bleeding on probing, and calculus index were recorded at six sites/tooth. Salivary concentrations of ionic calcium, potassium, magnesium and sodium were determined by atomic absorption spectrophotometer. Results were evaluated statistically by non-parametric tests. Results: No significant differences were found in clinical parameters or results of saliva analysis between the study groups (p > 0.05). Conclusion: Within the limits of this preliminary study, it is suggested that long-term calcium and vitamin D supplementation with or without alendronate does not appear to have a significant effect on supragingival calculus formation or saliva total calcium, potassium, magnesium and sodium concentrations. Larger-scale studies investigating the possible effects of various treatment modalities of osteoporosis on supragingival calculus formation are required to better clarify this issue

    Psikoz, düşünce ve algının ağır oranda bozulduğu zihin durumunu tanımlamakta kullanılan genel bir psikiyatri terimidir. Psikotik epizot geçiren hastalar halüsinasyonlar görebilirler, delüzyonel inançlar taşıyabilirler, kişilik değişiklikleri ve düşünce bozukluğu gösterebilirler. Psikozun ileri yaşta başlaması daha nadir görüldüğünden; özellikle halüsinasyon, delüzyonel inançlar, kişilik değişiklikleri ve düşünce bozukluğu ile başvuran ileri yaştaki kişilerde ayırıcı tanıda psikoz tanısının yanı sıra depresyon ve demans tanıları da düşünülmelidir. Bu vakada, taktil halüsinasyonla başvuran, depresyonun eşlik ettiği 85 yaşında demans hastası sunulmuştur. Vücutta karıncalanma ve sürekli pirelerin ısırdığını hissetme şikâyeti ile başvuran erkek hastaya, fizik ve psikiyatrik muayene ile kapsamlı geriatrik değerlendirme sonrası psikotik bulgulu depresyon ve demans tanısı konmuştur. Hekim tarafından ileri yaştaki her hasta demansiyel süreç açısından değerlendirilmeli ve demansiyel duruma başka psikiyatrik hastalıkların eşlik edebileceği göz önünde bulundurulmalıdır.Psychosis is a general psychiatric definition used in different psychiatric disorders with severe disruption in thought and perceptions. Patients with psychotic episodes may have hallucinations, delusional beliefs and disorientation. They may show changes in personality. Since onset of psychosis occurs rarely in old age, depression and dementia should be primarily considered especially in elderly patients presenting with hallucinations, delusional beliefs, personality changes and thought disorder. We present a case report of patient with dementia and late onset depression presenting with tactile hallucination. The aim of this case presentation is to focus on the hazards of symptom-based diagnosis and treatment that may cause misdiagnosis in elderly patients. An 85-year-old male patient was admitted to outpatient clinic with a complaint of tingling in his body and the feeling of insects crawling underneath your skin. After physical examination and comprehensive geriatric assessment, psychiatric consultant evaluated patient in Geriatric Medicine department. He was diagnosed with psychotic depression and dementia. Every patient should be examined for the cognitive impairment by the physician. It should also be considered that other psychiatric disorders may accompany by the dementia
