European Union’s approach to the Arctic policy development through the lens of the Russian regional interests

Based on an extensive database, the authors analyzed the main features of the EU Arctic strategy and their relationship with Russian interests in the region. The authors proceed from the fact that the probability of reaching any significant points of contact between the two actors is minimal. The incompatibility of their strategic lines is because the EU considers the Arctic as another platform for its potential of “normative strength”, while Russia, in cooperation with its foreign partners, is primarily aimed at attracting investments in the development of the Arctic infrastructure. In the context of the ongoing crisis in bilateral relations caused by the events in Ukraine, it is not possible to talk about the prospects for harmonizing the regional aspirations of the EU and the Russian Federation

Комбинированное лечебное питание после обширных резекций печени

Objective: to compare the time course of nutritional changes in relation to the components of nutritive support in the early postoperative period after extensive hepatectomies.Subjects and methods. Beginning with the 1st postoperative day, 12 patients had parenteral feeding comprising 20% glucose, fat emulsions, and specialized amino acid solutions (Group 1). Beginning with the same day, 14 patients were given the parenteral formula Oliclinomel (Group 2). Starting from the 2nd postoperative day, both groups additionally received a balanced feeding formula by sipping.Results. Postoperatively, first-to-second degree encephalopathy was recorded in both groups. Positive changes as consciousness recovery were found in both Group 1 in which the patients used branched-chain amino acid-enriched solutions and Group 2 (parenteral Oliclinomel feeding). The patients under examination were not given albumin solutions and fresh frozen plasma in the postoperative period. Moreover, by postoperative day 7, there were significant increases in the levels of albumin and total protein in Groups 1 and 2, in those of cholinesterase and the absolute count of lymphocytes, and a reduction in the concentration of C-reactive protein.Conclusion. Combined parenteral and enteral feedings are effective in patients after extensive hepatectomies. The parenteral feeding formula Oliclinomel may be used in this group of patients. Цель исследования — сравнить динамику показателей питательного статуса в зависимости от компонентов нутритивной поддержки в раннем п/о периоде после обширных резекций печени.Материалы и методы. В I группе (n=12) пациенты с 1-х п/о суток получали парентеральное питание, включавшее: глюкозу 20%, жировые эмульсии и специализированные аминокислотные растворы. Во II группе (n=12) с 1-х п/о суток вводили парентеральную смесь «Оликлиномель». Со 2-х послеоперационных суток в обеих группах дополнительно проводили питание методом «sipping» (глоточками) сбалансированной питательной смесью.Результаты. В I и II группах исследования регистрировалась энцефалопатия I—II степени в п/о периоде. Выявлена положительная динамика восстановления уровня сознания как в I группе, в которой пациентам назначали растворы, обогащенные аминокислотами с разветвленной цепью, так и во II группе (парентеральное питание «Оликлиномель»). Исследуемым пациентам не назначали растворы альбумина и свежезамороженную плазму в послеоперационном периоде. При этом к 7-м п/о суткам отмечено достоверное увеличение уровня альбумина и общего белка в I и во II группах, холинэстеразы и абсолютного числа лимфоцитов, снижение уровня С-реактивного белка.Заключение. Комбинированное парентеральное и энтеральное лечебное питание является эффективным у пациентов после обширных резекций печени. Использование препарата для парентерального питания «Оликлиномель» возможно у этой группы пациентов.

Displayed was the information on the preventive procedures for consumer financial and insurance protection that were being carried out by the Rospotrebnadzor Administration in cooperation with other agencies and services, allowing for relief of social tensions on the part of the population affected by flood

The paper proposes an automatic system of fuel oil metering in vertical cylindrical tanks. The system makes it possible to execute continuous control over weight and temperature of fuel oil in a tank and metering of dead residue and initiates a signal system when fuel oil reaches an appropriate level in the case of filling. Nowadays the automatic weight control system of fuel oil has passed through metrological certification and is in operation at the Polotsk thermal power station. Предложена автоматизированная система учета мазута в вертикальных цилиндрических резервуарах, позволяющая вести непрерывный контроль его массы и температуры в резервуаре, обеспечивающая учет мертвого остатка и сигнализацию при достижении определенных высот налива. В настоящее время автоматизированная система учета массы мазута прошла метрологическую аттестацию и эксплуатируется на Полоцкой ТЭЦ

Острый инфаркт миокарда как осложнение коронавирусной инфекции (клиническое наблюдение)

Coronavirus infection caused by the SARS-CoV-2 virus is a multifaceted disease due to generalized vascular endothelial damage. Endothelial damage also underlies COVID-associated coagulopathy.The paper presents a case of coagulopathy causing myocardial infarction in a 43-year-old patient with no history of coronary disease. We have reviewed the available literature for the pathophysiological rationale of the assumed possibility of coronary thrombosis resulting from coagulopathy with the intact intima of the coronary arteries.Conclusion. The present observation of coronary thrombosis with radiographically intact coronary artery intima confirms the important role of coronavirus infection in triggering endothelial dysfunction. Currently, the most effective strategy for this type of coronary lesions is the use of anticoagulants and antiplatelet agents along with ECG, echocardiography and troponin level monitoring.Коронавирусная инфекция, вызываемая вирусом SARS-CoV-2, является полиморфным заболеванием за счет генерализованного поражения эндотелия сосудов. Повреждение эндотелия лежит в основе ковид-ассоциированной коагулопатии.Привели наблюдение подобной коагулопатии, которая стала причиной острого инфаркта миокарда у 43-летнего мужчины без предшествующего коронарного анамнеза. Выполнили анализ доступных литературных источников на предмет патофизиологического обоснования гипотезы о возможности коронарного тромбоза как исхода ковид-ассоциированной коагулопатии при интактной интиме коронарных артерий.Заключение. Приведенное наблюдение подтверждает важную роль коронавирусной инфекциив запуске эндотелиальной дисфункции на примере коронарного тромбоза при рентгенологически интактной интиме венечных артерий. В настоящий момент наиболее эффективной тактикой при данном виде поражения коронарного русла остается антикоагулянтная и антиагрегантная терапия под контролем электрокардиографической, эхокардиографической картины и динамики тропонина

Опыт применения вено-венозной экстракорпоральной мембранной оксигенации у беременной с тяжелым острым респираторным дистресс-синдромом, вызванным вирусом SARS-CoV-2

 Pregnant women occupy a special place in the incidence structure of the new coronavirus infection COVID -19. Taking into account the likelihood of a more severe course of acute respiratory syndrome (ARDS) in this group, it is worth remembering the possibility of timely use of veno-venous extracorporeal membrane oxygenation (IV ECMO) in order to correct life-threatening hypoxia. At the Lapino Clinical Hospital, a cesarean section was successfully performed in a 37-year-old female patient at 20–21 weeks of gestation against the background of IV ECMO with further decannulation  and discharge from the hospital.  Особое место в структуре заболеваемости новой коронавирусной инфекцией COVID-19 занимают беременные. Принимая во внимание вероятность более тяжелого течения острого респираторного синдрома  (ОРДС) у данной группы, стоит помнить о возможности своевременного  применения вено-венозной экстракорпоральной мембранной оксигенации  (ВВ ЭКМО) с целью коррекции жизнеугрожающей гипоксии. В клиническом госпитале «Лапино» было успешно выполнено кесарево сечение у пациентки 37 лет на 20–21-й неделе гестации на фоне ВВ ЭКМО с дальнейшей деканюляцией и выпиской из стационара.

MULTIMODAL ANALGESIA AFTER TOTAL HIP ARTHROPLASTY

Purpose - to assess the effect of multimodal analgesia in the early rehabilitation of patients after hip replacement. Materials and methods. A prospective single-centre randomized research, which included 32 patients. Patients of the 1st group received paracetamol, ketorolac and tramadol, the 2nd group of patients - ketorolac intravenously and the 3rd group of patients - etoricoxib and gabapentin. Patients of the 2nd and the 3rd groups underwent epidural analgesia with ropivacaine. Multimodal analgesia was carried out for 48 hours after the surgery. Assessment of pain intensity was performed by the VAS (visual analogue scale), a neuropathic pain component - on the DN4 questionnaire . Time was recorded during the first and second verticalization of patients, using the distance walkers and by fixing the distance covered with in 2 minutes. Results. The intensity of pain for more than 50 mm on VAS at movement at least once every 48 hours after the surgery was occurred among 9% of the 1st group, 22% of patients from the 2nd group and 8% of patients of the 3rd group. Number of patients with neuropathic pain component decreased from 25% to 3% (p ≤ 0.05). The first verticalization was performed 10 ± 8 hours after the surgery, the second - 21 ± 8 hours later. Two-minute walk distance was 5 ± 3 and 8 ± 4 m, respectively. It is noted more frequent adverse events in patients of the 1st group was noted compared to patients of the 2nd and the 3rd groups during first (91%, 33% and 25%, p ≤ 0.05) and the second verticalization (70%, 25% and 17%, p ≤ 0.05). Multimodal analgesia allows to proceed with the successful activation of patients after hip replacement with in the first day after the surgery. The 3rd group patients are noted with a tendency for the optimal combination of efficient and safe of analgetic therapy
