    Nucleotide degradation and production of hypoxanthine in some Indian marine and freshwater fishes

    Changes in nucleotides and production of hypoxanthine in rohu (Labeo rohita), mrigal (Cihhrina mrigala) and common carp (Cyprinus carpio) during storage at 2-4°C were examined. Differences were observed between common carp and others. Production of hypoxanthine in pomfret (Stromateus argenteus), cat fish (Arius macronotacanthus), shark (Scoliodon spp.), seer fish (Scomberomorus guttatus), ray fish (Dasyatis imbricata) and prawns (Parapenaeopsis stylifera) was examined during storage at 2-4°C and -28°C. At 2-4°C hypoxanthine increased regularly but at -28°C there was no increase during storage over 28 weeks

    A comparative study of safety and efficacy of tiotropium bromide with salbutamol in moderate persistent bronchial asthma in a tertiary care hospital

    Background: The objective of this study was to compare the safety and efficacy of tiotropium bromide with salbutamol in moderate persistent cases of bronchial asthma at tertiary care hospital. Methods: This was an open label, randomized parallel group study done in Government General Hospital, Srikakulam for a period of 12 weeks. Group-1 was given tiotropium bromide metered dose inhalational therapy 18mcg once daily in 50 patients. Group-2 was given salbutamol metered dose inhalational therapy 100mcg thrice daily in 30 patients. Results: Symptomatic improvement was observed in l two groups. At end point, mean FEV1 in tiotropium bromide treatment group improved by 149.96% compared with 135.16% salbutamol treatment group. At end point, mean FVC value in tiotropium bromide treatment group improved by 145.71% compared with 122.34% in salbutamol group. Mean FEV1 / FVC also improved by 155.41% in tiotropium bromide group compared with 105.41 % in salbutamol group.Conclusion: The present study proved tiotropium provide superior safety and efficacy relative to control drug in bronchial asthma patients in both clinical assessment score and spirometrically.

    Constraints on stellar grain formation from presolar graphite in the murchison meteorite

    We report the results of isotopic, chemical, structural, and crystallographic microanalyses of graphitic spherules (0.3-9 μm) extracted from the Murchison meteorite. The spherules have 12C/13C ratios ranging over 3 orders of magnitude (from 0.02 to 80 times solar), clearly establishing their presolar origin as stellar condensates. These and other isotopic constraints point to a variety of stellar types as sources of the carbon, including low-mass asymptotic giant branch (AGB) stars and supernovae. Transmission electron microscopy (TEM) of ultrathin sections of the spherules revealed that many have a composite structure consisting of a core of nanocrystalline carbon surrounded by a mantle of well-graphitized carbon. The nanocrystalline cores are compact masses consisting of randomly oriented graphene sheets, from PAH-sized units up to sheets 3-4 nm in diameter, with little graphitic layering order. These sheets probably condensed as isolated particles that subsequently coalesced to form the cores, after which the surrounding graphitic mantles were added by vapor deposition. We also detected internal crystals of metal carbides in one-third of the spherules. These crystals (5-200 nm) have compositions ranging from nearly pure TiC to nearly pure Zr-Mo carbide. Some of these carbides occur at the centers of the spherules and are surrounded by well-graphitized carbon, having evidently served as heterogeneous nucleation centers for condensation of carbon. Others were entrained by carbon as the spherules grew. The chemical and textural evidence indicates that these carbides formed prior to carbon condensation, which indicates that the C/O ratios in the stellar sources were very close to unity. Only one of the 67 spherules studied in the TEM contained SiC, from which we infer that carbon condensation nearly always preceded SiC formation. This observation places stringent limits on the possible delay of graphite formation and is consistent with the predictions of equilibrium thermodynamics in the inferred range of pressure and C/O ratios. We model the formation of the observed refractory carbides under equilibrium conditions, both with and without s-process enrichment of Zr and Mo, and show that the chemical variation among internal crystals is consistent with the predicted equilibrium condensation sequence. The compositions of most of the Zr-Mo-Ti carbides require an s-process enrichment of both Zr and Mo to at least 30 times their solar abundances relative to Ti. However, to account for crystals in which Mo is also enriched relative to Zr, it is necessary to suppose that Zr is removed by separation of the earliest formed ZrC crystals from their parent gas. We also explore the formation constraints imposed by kinetics, equilibrium thermodynamics, and the observation of clusters of carbide crystals in some spherules, and conclude that relatively high formation pressures (≳ 0.1 dynes cm-2), and/or condensable carbon number densities (≳108 cm-3) are required. The graphite spherules with 12C/13C ratios less than the solar value may have originated in AGB stellar winds. However, in the spherically symmetric AGB atmospheres customarily assumed in models of stellar grain formation, pressures are much too low (by factors of ≳102) to produce carbide crystals or graphite spherules of the sizes observed within plausible timescales. If some of the graphite spherules formed in the winds from such stars, it thus appears necessary to assume that the regions of grain formation are density concentrations with length scales less than a stellar radius. Some of the spherules with both12C/13C ratios greater than the solar value and 28Si excesses probably grew in the ejecta of super-novae. The isotopic compositions and growth constraints imply that they must have formed at high densities (e.g., with p≳10-12 g cm-3) from mixtures of inner-shell material with material from the C-rich outer zones

    Receipt of Cardiac Medications Upon Discharge Among Men and Women With Acute Coronary Syndrome and Nonobstructive Coronary Artery Disease

    Background Management of acute coronary syndrome (ACS) patients with nonobstructive epicardial coronary artery disease (CAD) remains poorly understood. Hypothesis Acute coronary syndrome patients with nonobstructive CAD are less likely to receive effective cardiac medications upon discharge from the hospital. Methods We identified patients hospitalized with ACS that underwent coronary angiography and had a 6-month follow-up. Patients were grouped by CAD severity: nonobstructive CAD (<50% blockage in all vessels) or obstructive CAD (≥50% blockage in ≥ 1 vessels). Data were collected on demographics, medications at discharge, and adverse outcomes at 6 months, for all patients. Results Of the 2264 ACS patients included in the study: 123 patients had nonobstructive CAD and 2141 had obstructive CAD. Cardiac risk factors including hypertension and diabetes were common among patients with nonobstructive CAD. Men and women with nonobstructive CAD were less likely to receive cardiac medications compared to patients with obstructive CAD including aspirin (87.8% vs 95.0%, P = 0.001), Β-blockers (74.0% vs 89.2%, P < 0.001), or statins (69.1% vs 81.2%, P = 0.001). No gender-related differences in discharge medications were observed for patients with nonobstructive CAD. However, women with nonobstructive CAD had similar rates of cardiac-related rehospitalization as men with obstructive CAD (23.3% and 25.9%, respectively). Conclusions Patients with nonobstructive CAD are less likely to receive evidence-based medications compared to patients with obstructive CAD, despite the presence of CAD risk factors and occurrence of an ACS event. Further research is warranted to determine if receipt of effective cardiac medications among patients with nonobstructive CAD would reduce cardiac-related events.     A Prior Myocardial Infarction: How Does it Affect Management and Outcomes in Recurrent Acute Coronary Syndromes?

    Background Despite improved secondary prevention efforts, acute coronary syndrome (ACS) recurrence among patients with prior history of coronary events remains high. The differences in presentation, management, and subsequent clinical outcomes in patients with and without a prior myocardial infarction (MI) and presenting with another episode of ACS remain unexplored. Methods A total of 3,624 consecutive patients admitted to the University of Michigan with ACS from January 1999 to June 2006 were studied retrospectively. In-hospital management, outcomes, and postdischarge outcomes such as death, stroke, and reinfarction in patients with and without a prior MI were compared. Results Patients with a prior MI were more likely to be older and have a higher incidence of diabetes mellitus, hypertension, hyperlipidemia, and peripheral vascular disease. In-hospital outcomes were not significantly different in the 2 groups, except for a higher incidence of cardiac arrest (4.3% versus 2.5%, p < 0.01) and cardiogenic shock (5.7% versus 3.9%, p = 0.01) among patients without a prior MI. However, at 6 mo postdischarge, the incidences of death (8.0% versus 4.5%, p < 0.0001) and recurrent MI (10.0% versus 5.1%, p < 0.0001) were significantly higher in patients with a prior history of MI compared with those without. Conclusion Patients with prior MI with recurrent ACS remain at a higher risk of major adverse events on follow-up. This may be partly explained by the patients not being on optimal medications at presentation, as well as disease progression. Increased efforts must be directed at prevention of recurrent ACS, as well as further risk stratification of these patients to improve their overall outcomes.     A Study on Impact of Clinical Pharmacist Interventions on Medication Adherence and Quality of Life in Rural Hypertensive Patients

    Hypertension is the very common chronic disease in rural, urban and semi-urban areas of today's world, which needs continuous monitoring and treatment through out the life. Lack of education, lifestyle modification, and low level of understanding on disease management in rural people will influence directly on their quality of life (QOL). The objective of this study was to know the impact of clinical pharmacist interventions on medication adherence and QOL. It was a prospective, randomized and interventional study. Fifty-six patients were enrolled; only 52 patients completed the study. Interventional group patients received patient counselling, patient information leaflets (PILS), and frequent telephonic reminding. In the baseline, first and second follow-ups, medication adherence and QOL were assessed by using Morisky Medication Adherence Scale (MMAS) and Medication Adherence Report Scale (MARS) Questionnaires and SF-12v2 Quality of life Questionnaire in both the groups. The results showed that systolic blood pressure P value in the second follow-up was 0.086+ when compared to baseline follow-up P value 0.094. The diastolic blood pressure reading of the intervention group at the second follow-up was 77.73 ± 3.63 in mmHg when compared to the baseline, i.e. 86.62 ± 11.35 in mmHg. The MMAS and MARS scores P values were 0.007**, 1.000, <0.001**; 0.007, 0.014 and 0.000 at the baseline, first and second follow-ups, respectively. The QOL score P values of physical component scale and mental component scale showed highly significant. This study concluded/showed that the impact of clinical pharmacist provided patient counselling had a positive impact on medication adherence and QOL

    Navigating institutional pressure in state-socialist and democratic regimes: The case of movement brontosaurus

    Using the case of Movement Brontosaurus, a Czech organization founded in state socialist times, this article investigates how civic associations and nongovernmental organizations seeking to promote alternatives to the status quo respond to institutional pressures in different political and social contexts. The case shows that under state socialism, Brontosaurus appeared to conform to state mandates and societal expectations. However, its formal structure was decoupled from many activities to obscure its oppositional intent.After the transition to democracy, the organization was only able to maintain its place in society after it aligned its structure and practices with each other and openly expressed its alternative agenda. The findings demonstrate how social change and alternative lifestyle organizations vary their responses to institutional pressure in ways that enable them to realize their values and pursue their missions while accounting for the political and social contexts in which they are embedded

    Failure or success? Defensive strategies and piecemeal change among racial inequalities in the Brazilian banking sector

    We analyze how Brazilian Black Movement organizations and banks deployed different mechanisms like cooperation, cooptation, and confrontation that generated affirmative action initiatives in the banking sector at the beginning of this century. Black movement organizations triggered an institutional change by connecting fields and exploring a constellation of strategies. However, Brazilian banks adopted defensive strategies aiming to accommodate their interests. We find that only piecemeal change occurred, as the field’s structures – resource distribution and power – remained unscratched. We conclude by noting how the success of social movement strategies can depend upon the framing and sense-giving work that social movements conduct in their continuous jockeying activity toward incumbents
