    Водно-секторальные нарушения у родильниц с преэклампсией

    Objective: to study the profile of water sectors in puerperas with preeclampsia and to determine whether it is expedient to include colloidal solutions into an infusion therapy program for this category of patients.Subjects and methods. Forty-two puerperas with moderate and severe preeclampsia, whose delivery was made by cesarean section, were examined. All the study puerperas were divided into 2 groups: 1) 22 puerperas who received heta-hydroethylized starch (HES) solutions as part of infusion therapy; 2) 20 puerperas who did not.Results. At the beginning of the study, both groups had increased systemic water and higher extracellular and interstitial fluid volumes as compared with the normal values. With infusion therapy, Group 1 exhibited a decrease in systemic water from 123.8% on day 1 of the study to 106.7% by day 5 and reductions in interstitial hyperhydration from 141.5 to 110.1% and in extracellular fluid from 139.7 to 108.6% as compared with Group 2. By the end of the study, significant impairments in the balance of water sectors preserved in Group 2.Conclusion. Inclusion of heta-HES (Stabisole) into infusion therapy in patients with preeclampsia on days 1—2 after delivery leads to a rapider normalization of impaired water balance.  Цель исследования . Изучить профиль водных секторов у родильниц, страдающих преэклампсией и определить целесообразность включения растворов коллоидов в программу инфузионной терапии у данной категории больных.Материалы и методы. Обследовано 42 родильницы с преэклампсией средней и тяжелой степени, родораз-решенных путем операции кесарева сечения. Все исследуемые родильницы разделены на 2 группы: группа 1 (n=22) — родильницы, получавшие в составе инфузионной терапии растворы хета-гидроксиэтилированного крахмала (ГЭК); группа 2 (n=20) — родильницы, которым в состав инфузионной терапии не включены растворы ГЭК.Результаты. Выявлено, что в обеих группах в начале исследования отмечалось увеличение общей воды организма, объемов внеклеточной и интерстициальной жидкости по сравнению с показателями нормы. На фоне проводимой инфузионной терапии в 1-й группе по сравнению со 2-й группой отмечено: уменьшение общей воды организма со 123,8% в 1-е сутки исследования до 106,7% к 5-м суткам; уменьшение интерстициальной гипергидратации со 141,5 до 110,1%; уменьшение внеклеточной жидкости со 139,7 до 108,6%. Во 2-й группе к концу исследования сохраняются значительные нарушения баланса водных секторов.Заключение. Включение хета-ГЭК (Стабизол) в состав инфузионной терапии больным с преэклампсией на 1—2-е сутки после родоразрешения приводит к более быстрой нормализации нарушений водного баланса.

    The article is devoted to the development of mechanisms for monetary policy aimed at accelerated economic growth in Russia. It proves the need for a differentiated approach to determining the conditions of access to financial resources for different categories of participants in economic relations. To finance investment projects, it was proposed to use the target project emission mechanism using escrow accounts. Particular attention is paid to the exclusion of risks of inflation. Thus, the target project issue mechanism allows creating an additional non-inflationary project and investment money channel in the economy, developing an approach underlying the modern multi-channel transmission mechanism of the monetary policy of the Bank of Russia

    Effect of the Composition of Infusion Media on the Blood Lipoprotein Profile in the Treatment of Gestosis

    Objective: to study whether reamberin and hydroxyethyl starch may be used to correct dyslipoproteinemia in gestosis. Subjects and methods: Twenty-two patients with early postoperative gestosis were examined. Group 1 patients (n=8) received the standard treatment. In addition to the standard treatment, Group 2 patients (n=7) were given 6% hydroxyethyl starch solution infusion at concentrations of 5-6 ml/kg body weight at a rate of 3 ml/ml. Just after hydroxyethyl starch infusion, Group 3 patients (n=7) were additionally injected 1.5% reamberin by the above scheme. The results of examinations were compared with the data obtained on examination of 8 puerperas after pregnancy and normal delivery (Group 4). The plasma concentration of triglycerides, total cholesterol (TC), and the cholesterols of high-density lipoprotein (HDL), low-density lipoprotein (LDL), and very low-density lipoprotein (VLDL) were measured. Results and discussion. All the puerperas with gestosis were found to have hypertriglyceridemia and elevated levels of VLDL cholesterol with decreased concentrations of HDL cholesterol. The moderately higher levels of triglycerides and TC were also observed in patients without gestosis. Hydroxyethyl starch lowered the concentration of triglycerides by postpartum days 3—4. When hydroxyethyl starch was used in combination with reamberin, there was a significant reduction in the concentrations of triglycerides and VLDL and LDL cholesterols and a substantial rise in the level of HDL cholesterol. By postpartum days 3 and 4, Group 1 showed a considerable increase in the atherogenicity coefficient, which was significant as compared with the baseline level. Hydroxyethyl starch alone prevented an increase in the atherogenici-ty coefficient while its use in combination with reamberin significantly lowered this index and normalized it by postpartum days 3—4. Conclusion. Hydroxyethyl starch alone and in combination with reamberin shows an antiatherogenic effect in the treatment of gestosis in the early postpartum period, the action of hydroxyethyl starch in combination with reamberin was much more pronounced.
