    Multi-Modal Neuroimaging Analysis and Visualization Tool (MMVT)

    Sophisticated visualization tools are essential for the presentation and exploration of human neuroimaging data. While two-dimensional orthogonal views of neuroimaging data are conventionally used to display activity and statistical analysis, three-dimensional (3D) representation is useful for showing the spatial distribution of a functional network, as well as its temporal evolution. For these purposes, there is currently no open-source, 3D neuroimaging tool that can simultaneously visualize desired combinations of MRI, CT, EEG, MEG, fMRI, PET, and intracranial EEG (i.e., ECoG, depth electrodes, and DBS). Here we present the Multi-Modal Visualization Tool (MMVT), which is designed for researchers to interact with their neuroimaging functional and anatomical data through simultaneous visualization of these existing imaging modalities. MMVT contains two separate modules: The first is an add-on to the open-source, 3D-rendering program Blender. It is an interactive graphical interface that enables users to simultaneously visualize multi-modality functional and statistical data on cortical and subcortical surfaces as well as MEEG sensors and intracranial electrodes. This tool also enables highly accurate 3D visualization of neuroanatomy, including the location of invasive electrodes relative to brain structures. The second module includes complete stand-alone pre-processing pipelines, from raw data to statistical maps. Each of the modules and module features can be integrated, separate from the tool, into existing data pipelines. This gives the tool a distinct advantage in both clinical and research domains as each has highly specialized visual and processing needs.     Currently, the methods of delivering high local doses of chemotherapy to locally-advanced tumors of the extremities, while avoiding systemic toxicity, are actively investigated. Isolated regional limb chemoperfusion is one of these methods. From 2004 to 2014, a total of 70 patients underwent isolated limb perfusion (ILP) for locally-advanced soft tissue sarcoma and transit metastases of extremity melanoma. Complications of ILP, causes of their occurrence and preventive measures were analyzed.В настоящее время активно изучаются методы локального воздействия на местно-диссеминированные опухоли конечностей, позволяющие избежать калечащей операции и сохранить высокое качество жизни больного. Одной из таких методик является изолированная регионарная химиоперфузия конечностей. Мы изучили данные по 70 больным, которым в отделении общей онко логии НИИ онкологии им. Н.Н. Петрова в период с 2004 по 2014  выполнена изолированная химиоперфузия по поводу местно распространенных сарком мягких тканей, а также транзитных метастазов меланомы конечностей. Представлен анализ возможных осложнений ИРП, причины их возникновения и возможные пути профилактики

    A rational approach to opioid dose reduction in the treatment of bone pain. Clinical discussion

    Narcotic drugs have become more available for use, but it is obvious that monotherapy of pain syndrome with narcotic drugs is not always effective. Patients sometimes change prescribed opiates to NSAIDs on their own, because it is more effective despite the high risk of complications. In this case patient has a grade 2 pain syndrome associated with bone metastases despite taking the maximum daily dose of tramadol complicated by nausea. Treatment was successfully changed with medium doses of tramadol and dexketoprophen. We consider the combination of narcotic drugs and NSAIDs as opiate-sparing and suggest that dexketoprophen is effective for treatment of pain associated with bone metastases because of the effect on neuropathiс and central components of pain syndrome. We have analyzed the main mechanisms and options for systemic pharmacotherapy of pain syndrome in bone metastases. Some NSAIDs are known to have central analgesic effects. For example, the analgesic effect of ketorolac after an injury of sciatic nerve is explained by its ability to inhibit the synthesis of algogenic peptides in the posterior horns of the spinal cord and the decrease in astrocyte activation. However, it is the dexketoprofen/tramadol combination that is recognized as the most effective in the world
