Functional and Radiological Outcome of Periprosthetic Fractures following Primary Hip Arthroplasty

    Get PDF
INTRODUCTION: Periprosthetic fractures of the femur after hip arthroplasty represent a difficult treatment challenge. The results of management of periprosthetic fractures have varied greatly due to factors such as bone quality, fracture pattern and method of treatment including non operative measures, reduction fixation, or revision surgery. These treatments have historically been fraught with high incidence of complications, treatment failures and compromised outcomes. Despite these problems, surgical treatment has become the standard in treating the majority of periprosthetic femur fracture. AIM OF THE STUDY: To assess the functional outcome of patient who underwent treatment for periprosthetic fracture following hip arthroplasty. MATERIALS AND METHODS: It is a prospective and retrospective study of 15 patients, 12 male and 3 female, conducted in Government Kilpauk Medical College and Hospital, with average follow up was 12 months. Outcome was analyzed functionally by HARRIS HIP SCORE and radiologically. Inclusion Criteria: Patient who underwent treatment for periprosthetic fractures following hip arthroplasty. Age >18 years. Exclusion Criteria: Medically unfit patients, Age<18years, periprosthetic fractures following Revision THR. Instruments: Revision long stem. Locking plate, SS wire. RESULTS: Out of 23 patients, 2 showed excellent, 3 patients showed good outcome, 7 showed fair outcomes, 3 showed poor outcome.4 patient died during follow up, 4 patient were lost in follow up. CONCLUSION: Our study demonstrates good functional outcome in periprosthetic fracture with few complications

The Importance of Multimodal Emotion Conditioning and Affect Consistency for Embodied Conversational Agents

Previous studies regarding the perception of emotions for embodied virtual agents have shown the effectiveness of using virtual characters in conveying emotions through interactions with humans. However, creating an autonomous embodied conversational agent with expressive behaviors presents two major challenges. The first challenge is the difficulty of synthesizing the conversational behaviors for each modality that are as expressive as real human behaviors. The second challenge is that the affects are modeled independently, which makes it difficult to generate multimodal responses with consistent emotions across all modalities. In this work, we propose a conceptual framework, ACTOR (Affect-Consistent mulTimodal behaviOR generation), that aims to increase the perception of affects by generating multimodal behaviors conditioned on a consistent driving affect. We have conducted a user study with 199 participants to assess how the average person judges the affects perceived from multimodal behaviors that are consistent and inconsistent with respect to a driving affect. The result shows that among all model conditions, our affect-consistent framework receives the highest Likert scores for the perception of driving affects. Our statistical analysis suggests that making a modality affect-inconsistent significantly decreases the perception of driving affects. We also observe that multimodal behaviors conditioned on consistent affects are more expressive compared to behaviors with inconsistent affects. Therefore, we conclude that multimodal emotion conditioning and affect consistency are vital to enhancing the perception of affects for embodied conversational agents

The prevalence of anti-thyroid peroxidase antibodies in subclinica and clinical hypothyroid patients

    Get PDF
Background: Subclinical hypothyroidism is a state of mild thyroid failure and is essentially a laboratory diagnosis with elevated serum thyroid stimulating hormone (TSH) and a normal free thyroxine (FT4) concentration. The main objective of study is to evaluate the prevalence of anti-thyroid peroxidise (anti-TPO) antibodies among patients with clinical and subclinical hypothyroidism. Methods: A prospective study was conducted involving 50 patients with biochemical evidence of hypothyroidism. Subclinical hypothyroidism was defined as thyroid stimulating hormone (TSH) >5.0 µIU/ml with normal FT4 and clinical hypothyroidism as free thyroxine (FT4) and high TSH. A detailed history, clinical examination, and investigations comprising of complete haemogram, fasting plasma glucose, fasting FT4, TSH, anti-TPO antibodies and lipid profile were done for all the patients. Results: Out of 50 cases, 28 subjects had clinical hypothyroidism (25 females and 3 males) and 22 had subclinical hypothyroidism (14 females and 8males). Among the 50 subjects with clinical and subclinical hypothyroidism, 33 were anti-TPO positive. The corresponding percentage of anti-TPO positivity noted in the clinical hypothyroidism and subclinical hypothyroidism groups were 80 % and 50% respectively. Conclusions: Serum TSH and anti-TPO analyses are essential in determining the etiology of hypothyroidism and risk of progression to overt hypothyroidism in patients with subclinical.
