    Nonchirurgicale treatment of cervical and lumber disc herneatons

Tehnici transcutanate a nucleoplastice pot fi aplicate ca o măsură intermediară între tratamentul conservator și chirurgie, cu scopul de a evita evenimentele adverse asociate cu discectomia chirurgicală. În unele centre specializate, rezultate bune s-au obținut cu etanol pur, amestecat cu etil celuloza pentru a crește vâscozitatea și îmbunătățit cu material opac de radio. Scopul. Determinarea eficacității tratamentul herniilor lombare și cervicale cu etanol radio-opac gelificat. Material și metode. Un lot de 49 de pacienți au fost incluși în acest studiu și s-au tratat de hernie de disc intervertebral în regiunea lombară și cervicală cu etanol radio-opac gelificat. Am evaluat nivelurile de durere pentru fiecare pacient în timpul procedurii în sine și apoi, după 3-4 și 8 săptămâni, 4 - 24 de luni după proceduri. Rezultate. Intensitatea de durere imediat după intervenție a fost semnificativ mai mică decât înainte de procedură. Nu au existat complicații. La două luni după administrarea procedurii, nivelul durerii inițiale a scăzut cu o medie de 84%.Rezultate foarte bune sau bune s-au obținut în 42 (89,14%) din cei 49 pacienți, satisfăcătoare - în 16 cazuri și rele, în 2 cazuri (8,6%). Doar 2 cazuri cu un rezultat rău la nivel lombar au dus la o necesitatea de a interveni chirurgical. Concluzie. Studiul prezentat arată eficacitatea și inocuitatea acestei metode. Mai ales, ea a demonstrat absența complicațiilor și recidive pe termenul monitorizării pentru mai mult de 3 ani pentru primele cazuri.Introduction. Percutaneous intradiscal techniques of nucleoplasty can be applied as an intermediate measure between conservative treatment and surgery, with a view to avoiding the adverse events associated with surgical discectomy. In some specialist centres, good results were obtained with pure ethanol, mixed with ethylcellulose to increase its viscosity and enhanced with radio opaque material. Work’s goal. Determinate the safety and efficacy of gelified ethanol in the percutaneous treatment of lumbar disk hernias. Material and methods. A total of 49 patients were included in this study and treated with radio opaque gelified ethanol and intraarticular steroids of a lumbar and cervical intervertebral disk hernia. We evaluated each patient’s pain levels during the procedure itself and then after 3-4 and 8 weeks, and 4- 24 months after procedure.Results. Pain levels immediately after the procedure were markedly lower than before the procedure. There were no complications. Two months after procedure administration, the initial pain level had fallen by an average of 84 %. Very good or good results were obtained in 42 (89,1%) of the 49 patients, fair — in 16 cases and bad in 3 cases (8,6%). Only 2 cases with a bad outcome at lumbar level went to surgery. Conclusion. This preliminary study shows the efficacy and inocuity of this substance. More especially, it demonstrated the absence of complications and recidivates in the immediate and long-term follow-up for more than 3 years for the first cases

În ultimii ani în ortopedie, în tratamentul afecțiunilor osoase se utilizează preparatele compozite pe baza de hidroxiapatită și colagen. Hidroxiapatita și materialele la baza ei, au proprietatea de a se reabsoarbe și utiliza de către organism, de a stimula procese reparative în țesutul osos. Scopul. Creșterea eficacității tratamentului în focarele patologice a aparatului locomotor ale copilului. Materiale și metode. În Clinica de Vertebrologie, Ortopedie și Traumatologie a Centrului Național Științifico- Practic de Chirurgie Pediatrică "N. Gheorghiu" la 19 copii cu diverse focare patologice a aparatului locomotor, s-a utilizat materialul "LitAr". Pacienții au fost pre- și post operator examinați și urmăriți pe o perioadă de 1-8 ani. Copii cu vârsta cuprinsă între 3 și 17 ani. Prin puncție în focarul de afecțiune, în combinare cu antibiotice la 70% de cazuri s-a introdus materialul "LitAr". Volumul defectului osos varia între 4cm³ - 200cm³. Rezultate. La toți pacienții, peste un an s-a observat reconstrucția totală a focarului patologic. Concluzii. Introducerea prin injectare a compozitului "LitAr" permite de a asigura consolidarea fragmentelor osoase în cazul chisturilor, consolidării întârziate a fracturii sau a pseudoartrozei, fără a interveni chirurgical. Materialele compozite biodegradante au în șir de avantaje semnificative în comparație cu alo-osul (având proprietatea de a se resorbi și de a fi utilizat de către organism, de a stimula procese reparative).Introduction. In recent years in orthopedic surgery for treatment of osteal pathology began to apply preparations on the hydroxidepatid and collagen basis. Hydroxidepatid and materials on its basis are capable to be utilized by organism and stimulate reparative processes in an osteal tissue. Work’s goal. Rising of efficiency of children's treatment with innocent osteal lesions of a locomotorium and the acquired nearthrosis. Material and methods. In Clinic of Vertebrology, Orthopedics and Traumotology of the "N.Gheorghiu" Scientifically-Practical Center of Children's Surgery 19 children with different pathologies of the musculoskeletal system that was used composite «LitAr», have been pre- and postoperatively examined with a follow-up of 1 to 8 years. Children were aged between 3 and 17 years. In all cases performed filling the cavity with material "LitAr" by puncture the outbreak of disease, in combination with antibiotics to 70% of cases. Completed effect volume ranging from 4cm³ to 200cm3. Results. At all patients after 1 year there came full reorganization of the pathological center. Conclusion. Injection introduction of a composite allows providing an adnation of osteal fragments without operation in case of the slowed-down consolidation of fracture or nearthrosis. Biodegraded composite materials have essential advantages in comparison with an allobone: ability to stimulate reparative processes and to be utilized by the organism

    The aim of this study is to improve the sweet cherry tree assortment for the Northeastarea of Romania by promoting the new cultivars created at RSFG Iasi, renewing the current sweet cherry tree assortment with new quality cultivars and especially extending the sweet cherry season, filling the existing gaps during the consumption period. At this time, the market is unbalanced in favor of cultivars in the first part of the fruit season. The tendency is towards rebalancing, by reducing the proportion of cultivars with medium season maturity and increasing the early and the late ones. In termsof productivity, the three-year average production (2016-2018) cultivars as followes Golia (19.7 kg / tree), Margonia (23.3 kg / tree) and Bucium (24.7 kg / tree) was remarkable. Regarding the average weight of the fruit, it recorded values between 4.8 g (Cetatuia) and 7.6 g (Bucium)

    Evaluarea unor soiuri de cireş amar obţinute la S.C.D.P. Iaşi

    The aim of the paper is to present the valuable features of bitter cherry cultivars obtained at RSFG Iasi, that improve the cultivars’ assortment with different maturation ages of the fruits sequenced all over the cherries’ maturation season. In terms of fruit’s weight (g) and equatorial diameter (mm), the cultivars Amaris (5.1 g and 21.1 mm) and Amar Galata (4.1 g and 18.0 mm) got remarked statistically during the five years. They recorded very significant differences and distinct positive significant differences in comparison with the witness cultivar Silva (3.0 g and 15.8 mm). For the stone’s size, the cultivars recorded a weight between 0.25-0.33 g, recording very negatively significant differences (Amar Maxut with 0.25 g) and negatively distinct significant differences (Amaris with 0.26 g) in comparison with the cultivar Silva as control (0.33 g). Regarding the fruits’ resistance to cracking, Amar Maxut (0.3%), Amaris (0.3%) and Amar Galata (3.3%) present a resistance superior to the control cultivar Silva (4.1%

    Investigations on the fruit resistance to deformation in some sweet cherry tree varieties

    The knowledge of sweet cherry resistance to deformation was important for a good choice of means and methods of packing, storage and transportation of fresh fruits for consumption, both on internal market and for exportation. In this paper, the authors have shown the results of some determinations on fruit deformation, carried out on seven sweet cherry varieties (Cetăţuia, Cătălina, Maria, Golia, Ştefan, Bucium and Tereza), harvested at three maturation stages: before ripening (10 days before ripening), almost ripe (five days before maturity) and at maturity. We have recorded the fruit resistance to deformation at a weight below 500 g, using an original device made by the Fruit Growing Research and Development Station of Iaşi. This mechanical device can be used for measuring the resistance to deformation of the fruit belonging to some fruit species or of grapes. The fruit deformation was determined for a certain weight (in our determinations, below the weight of 500 g). By comparing the varieties between them, we found that Maria and Bucium varieties were the most resistant to deformation, at all the three harvest stages, significant differences being found, compared to the other varieties. We have used these determinations for the statistical calculation by the analysis of variance for bifactorial trials, in order to establish the most resistant varieties to fruit deformation at different harvest stages

    Механизмы внутристентового рестеноза анализируемые путём коронарной ангиографии и внутрикоронарной ультрасонографии

    Although the coronary angiography remains a proven method of in-stent restenosis assessment, intracoronary ultrasound (IVUS) is underlined as a method of better coronary lesion indices evaluation. This study was aimed as an attempt at relevant in-stent restenosis mechanisms estimation via concomitant using of IVUS and coronary angiography. These exams were applied to 47 patients having 54 bare metallic and eluting stents. Obtained results indicated different mechanisms of restenosis development in bare metallic and eluting stents. The neointima hyperplasia and negative artery remodeling in metallic stent restenosis are more expressed. Likewise, the IVUS method appears as a feasible tool of the coronary plaque area determination and guiding of stent implantation during angioplasty. The last aspect is especially important in the case of coronary artery diameter less than 3 mm.Хотя коронарная ангиография продолжает оставаться апробированным методом выявления внутристентового рестеноза, внутрикоронарная ультрасонография (IVUS) выделяется как метод лучшего определения показателей коронарных поражений. С целью анализа механизмов развития рестеноза эти два метода были использованы на 47 больных, имеющих 54 имплантированных металлических и фармакологических стентов. Полученные данные показывают наличие различных механизмов развития рестеноза металлических и фармакологических стентов. Гиперплазия неоинтимы и негативное ремоделирование артерии более выражены в металлических стентах. Метод IVUS представляется достоверным способом определения размеров коронарной бляшки и контроля имплантации стента, особенно, когда диаметр коронарной артерии меньше 3 мм
