    Consideration for care for your patient with cerebral palsy

    Cerebral palsy (CP) is a common developmental neurological disorder affecting about 2-3 children out of 1,000. CP is the result of infant brain damage or abnormal development resulting in impaired muscle control, coordination, tone, reflex, posture, and balance. These patients are unable to control motor movements of their muscles of mastication and facial expression, causing excessive drooling, clenching, bruxism, and other oral health-related issues. This lack of motor control affects their ability to swallow and often limits these patients to a liquid diet. This can lead to vitamin deficiencies and result in further developmental problems. As an example, a deficiency in vitamin D may lead to osteoporosis, which manifests in the oral cavity as periodontal disease. Even into adulthood, these individuals are often reliant on the care of others. It becomes the caregiver’s responsibility to ensure the individual with cerebral palsy is receiving consistent and effective oral hygiene, and to monitor the oral tissues for signs of disease or injury. The researchers reviewed primary and secondary literature published after 2014 on the subjects of cerebral palsy, general health considerations, and oral care.     Food Aid as Surplus Disposal? The WTO, Export Competition Disciplines and the Disposition of Food Aid

    The empirical investigation suggests that there exists an endogenous relationship between subsidy/credit shipments and food aid for wheat in the US. The empirical VAR demonstrates a contemporaneous increase in food aid shipments as alternative vents constrict. This result suggests that a trade agreement that disciplines export subsidies and credits may put upward pressure on food aid shipments as agricultural exporters vent the pressure of their domestic surpluses. The empirical results suggest that in the US wheat market the effects are not large. The same phenomenon has been noted in the case of skim milk powder by Margulis; skim milk powder would provide another interesting empirical case, were the data available.Food Security and Poverty,

    A Missed Opportunity: The Texas Economic Development Act, Texas Public School Funding, and Wind Energy

    Some critics called the uncapped supplemental payments received by predominately rural West Texas school districts prior to 2009 “windfalls.” The result of this one-sided understanding, and subsequently the complete elimination of such agreements between school districts and companies owning qualifying projects, is a handicap to what could be utilized as a valuable tool in filling the current deficit for public school funding. In 2011, the 82nd Texas Legislature appropriated at least 4billionbelowwhathadbeenthecurrentformulafundinglevelforpubliceducationforthe20122013biennium;someestimateshadthisfigureover4 billion below what had been the current formula funding level for public education for the 2012-2013 biennium; some estimates had this figure over 5 billion. More recently, in 2013, the 83rd Legislature only returned $3.4 billion to public schools. In a time when funding options are shrinking, why would the Comptroller’s office suggest, and the Legislature pass, caps on incentives that provide additional funding for school districts? This Article will explore the development of public school funding in Texas and the possibility of utilizing the Act as a means of providing additional funding for the public school system while encouraging large-scale capital investment, specifically by renewable energy projects qualifying under Chapter 313 of the Texas Tax Code

    Sequential Electrocoagulation-Electrooxidation For Virus Mitigation in Drinking Water

    Electrochemical water treatment is a promising alternative for small-scale and remote water systems that lack operational capacity or convenient access to reagents for chemical coagulation and disinfection. In this study, the mitigation of viruses was investigated using electrocoagulation as a pretreatment prior to electrooxidation treatment using boron-doped diamond electrodes. This research is the first to investigate a sequential electrocoagulation-electrooxidation treatment system for virus removal. Bench-scale, batch reactors were used to evaluate mitigation of viruses in variable water quality via: a) electrooxidation, and b) a sequential electrocoagulation-electrooxidation treatment train. Electrooxidation of two bacteriophages, MS2 and ΦX174, was inhibited by natural organic matter and turbidity, indicating the probable need for pretreatment. However, the electrocoagulation-electrooxidation treatment train was beneficial only in the model surface waters employed. In model groundwaters, electrocoagulation alone was as good or better than the combined electrocoagulation-electrooxidation treatment train. Reduction of human echovirus was significantly lower than one or both bacteriophages in all model waters, though bacteriophage ΦX174 was a more representative surrogate than MS2 in the presence of natural organic matter and turbidity. Compared to conventional treatment by ferric salt coagulant and free chlorine disinfection, the electrocoagulation-electrooxidation system was less effective in model surface waters but more effective in model groundwaters. Sequential electrocoagulation-electrooxidation was beneficial for some applications, though practical considerations may currently outweigh the benefits

    A survey of speech pathologists’ opinions about the prospective acceptability of an online implementation platform for aphasia services

    Background Online knowledge translation (KT) approaches are becoming increasingly prevalent within healthcare due to their accessibility and facilitation of international support networks. Online platforms enable timely and far-reaching dissemination of current evidence and best-practice recommendations. Although there is potential to improve the uptake of rehabilitation guidelines, it is essential to consider the acceptability of online approaches to healthcare professionals to ensure their successful integration within everyday clinical settings. Aims To establish the prospective acceptability of a theoretically informed online intervention for speech pathologists, Aphasia Nexus: Connecting Evidence to Practice, that aims to facilitate the implementation of aphasia best practice. Methods & Procedures A mixed-methods multinational electronic survey based on the Theoretical Framework of Acceptability (TFA) completed by aphasia researchers and clinicians. Outcomes & Results A total of 43 participants completed the survey with 91% (n = 39) indicating that they would use Aphasia Nexus. Understanding the intervention and how it works (intervention coherence as per the TFA) was the key factor influencing the likelihood of integration within everyday clinical practice. Participants identified potential areas where the intervention could influence service change and also recommended further design and content changes to improve the intervention. Conclusions & Implications Aphasia Nexus is an acceptable platform for further feasibility testing in the form of a pilot trial within an Australian-based health service. The study progresses the theory of TFA as it was a valuable framework facilitating the identification of prominent factors influencing acceptability. The study also informs further intervention refinements in preparation for the next stage of research. WHAT THIS PAPER ADDS What is already known on the subject Online strategies have the potential to enhance KT and promote the uptake of rehabilitation guidelines. An online intervention, however, can only be effective if implemented well. For this reason, it is essential to establish the acceptability of online interventions to the intended recipients and therefore increase the likelihood of successful implementation. What this paper adds to existing knowledge This study used a theoretically based framework to establish the acceptability of an online implementation intervention, Aphasia Nexus, to multinational aphasia clinicians and researchers. It demonstrated the value in identifying the prominent factors influencing acceptability to inform further intervention refinements and warrant continuing research. What are the potential or actual clinical implications of this work? Speech pathologists should use online platforms to drive the implementation of best practice on an international scale. It is important for clinicians to have an in-depth understanding of online interventions and how they work to enhance their successful uptake into routine clinical practice.     Sub-lethal Effects of 2,4-D Exposure on Golf Course Amphibians

    Amphibians are among the most common vertebrates inhabiting golf courses. They are frequent inhabitants of golf course wetlands, where they are likely to be episodically exposed to small quantities of herbicides in proper golf course maintenance. The goal of this study was to investigate whether amphibian larvae subjected to sublethal concentrations of a common herbicide used in golf course maintenance would likely result in significant life history, locomotor, or behavioral effects (e.g., changes in growth, swimming speed, or feeding ability). For these experiments we selected as models tadpoles of the southern leopard frog (Rana sphenocephala) and the herbicide 2,4- dichlorophenoxyacetic acid (2,4-D). The study’s findings include: Some life history traits (e.g., survival to metamorphosis) may be affected by chronic exposure at high doses, but other traits (e.g., growth, timing of metamorphosis, and size at metamorphosis) are not affected. Acute exposure is unlikely to have significant impact on life history traits. Acute exposure does not alter locomotor ability. Acute exposure to 2,4-D reduces the activity of tadpoles, and it also reduces feeding activity when predators are present. We conclude that 2,4-D does not represent a particularly strong threat to amphibian larvae inhabiting golf course where the herbicide is applied responsibly

On windy days, water from the main jet blows mist outside the boundaries of the fountain and onto the civilian walkway. This affects the visitors of the zoo. The present countermeasure to this complication requires a manual valve adjustment or temporary pump shutdown. This causes the zoo staff to have to divert from their usual tasks, wasting time. This countermeasure also wastes energy, causing the zoo to overspend on electricity
