    Genetic modifiers of stroke in patients with sickle cell disease: a scoping review

    This work was partially supported by H&TRC, FCT/MCTES grant number (UIDB/05608/2020 and UIDP/05608/2020) and the Dana-Farber/Boston Children’s Cancer and Blood Disorders Center Talent Development Award.Sickle cell disease (SCD) clinically manifests itself with a myriad of complications. Stroke, both ischemic and hemorrhagic, as well as silent white matter changes, occurs at a relatively high prevalence. Understanding why and in whom stroke is most likely to occur is critical to the effective prevention and treatment of individuals with SCD. Genetic studies, including genome- and exome-wide association studies (GWAS and EWAS), have found several key modifiers associated with increased stroke/stroke risk in SCD via mechanisms including Hemoglobin F (HbF) modulation, inflammation, cellular adhesion, endothelial disruption, and hemolysis. We present a review of the modifiers that have most clearly demonstrated an association to date. More studies are needed to validate other potential polymorphisms and identify new ones. Incorporating gene-focused screenings in clinical care could provide avenues for more targeted, more effective, and less toxic prevention of stroke in this population.     Difficult management of an extremely rare case of giant pigmented epithelioid melanocytoma

    Pigmented epithelioid melanocytoma (PEM), a heavily pigmented and highly uncommon melanocytic lesion, is extremely rare. It comes under several forms and names. We report the case of a three-year-old boy born with an isolated right lumbopelvic and femoral giant PEM. The boy was unable to walk due to the size and the shape of the lesion. He underwent two stages of intra lesional resections, one in Benin and the second in Switzerland. The first surgery was followed by hypothetic hypovolemic and anaphylactic shocks, and the second surgery by a continuous lymphatic leakage from the wounds for months. The strategic management approach, the surgery and follow-up of this case of giant PEM presented a real challenge. (C) 2017 The Authors. Published by Elsevier Inc.     Let HH be a group and EE a set such that HEH \subseteq E. We shall describe and classify up to an isomorphism of groups that stabilizes HH the set of all group structures that can be defined on EE such that HH is a subgroup of EE. A general product, which we call the unified product, is constructed such that both the crossed product and the bicrossed product of two groups are special cases of it. It is associated to HH and to a system ((S,1S,),,,f)\bigl((S, 1_S,\ast), \triangleleft, \, \triangleright, \, f \bigl) called a group extending structure and we denote it by HSH \ltimes S. There exists a group structure on EE containing HH as a subgroup if and only if there exists an isomorphism of groups (E,)HS(E, \cdot) \cong H \ltimes S, for some group extending structure ((S,1S,),,,f)\bigl((S, 1_S,\ast), \triangleleft, \, \triangleright, \, f \bigl). All such group structures on EE are classified up to an isomorphism of groups that stabilizes HH by a cohomological type set K2(H,(S,1S)){\mathcal K}^{2}_{\ltimes} (H, (S, 1_S)).     We investigate one question regarding bicrossed products of finite groups which we believe has the potential of being approachable for other classes of algebraic objects (algebras, Hopf algebras). The problem is to classify the groups that can be written as bicrossed products between groups of fixed isomorphism types.     Pulmonary valve stenosis due to undifferentiated pleomorphic sarcoma

    Pulmonary artery sarcomas are uncommon intracardiac tumours with poor prognosis. We report the case of a 69-year-old woman in good health presenting with rapidly progressive dyspnoea and an unfamiliar systolic murmur. Echocardiography revealed pulmonary valve stenosis due to an obstructing mobile mass. Imaging studies confirmed the presence of a contrast-enhancing lesion adherent to the valve, extending into the pulmonary trunk and right ventricular outflow tract, and suggestive of malignancy. Endovascular biopsy was attempted with no success. Surgical resection with autologous graft valve replacement and pulmonary artery reconstruction was performed. Postoperative histological examination confirmed the diagnosis of an undifferentiated pleomorphic sarcoma. Pulmonary artery sarcoma should be considered as a rare differential diagnosis in patients presenting with dyspnoea and a crescendo-decrescendo systolic murmur increasing with inspiration. Echocardiography is a useful first diagnostic approach but multi-imaging assessment is almost always necessary for definite diagnosis. Our case provides insights into the challenges met by cardiologists, radiologists and cardiac surgeons in the management of such cases
