    Timing matters: impact of anticonvulsant drug treatment and spikes on seizure risk in benign epilepsy with centrotemporal spikes

    OBJECTIVE: Benign epilepsy with centrotemporal spikes (BECTS) is a common, self-limited epilepsy syndrome affecting school-age children. Classic interictal epileptiform discharges (IEDs) confirm diagnosis, and BECTS is presumed to be pharmacoresponsive. As seizure risk decreases in time with this disease, we hypothesize that the impact of IEDs and anticonvulsive drug (ACD) treatment on the risk of subsequent seizure will differ based on disease duration. METHODS: We calculate subsequent seizure risk following diagnosis in a large retrospective cohort of children with BECTS (n = 130), evaluating the impact of IEDs and ACD treatment in the first, second, third, and fourth years of disease. We use a Kaplan-Meier survival analysis and logistic regression models. Patients were censored if they were lost to follow-up or if they changed group status. RESULTS: Two-thirds of children had a subsequent seizure within 2 years of diagnosis. The majority of children had a subsequent seizure within 3 years despite treatment. The presence of IEDs on electroencephalography (EEG) did not impact subsequent seizure risk early in the disease. By the fourth year of disease, all children without IEDs remained seizure free, whereas one-third of children with IEDs at this stage had a subsequent seizure. Conversely, ACD treatment corresponded with lower risk of seizure early in the disease but did not impact seizure risk in later years. SIGNIFICANCE: In this cohort, the majority of children with BECTS had a subsequent seizure despite treatment. In addition, ACD treatment and IEDs predicted seizure risk at specific points of disease duration. Future prospective studies are needed to validate these exploratory findings.Published versio

    Positive and Negative Symptoms of Schizotypy and the Five-Factor Model: A Domain and Facet Level Analysis

    In this study, we investigated the Five-factor model in the concurrent prediction of positive symptomschizotypy as measured by the Magical Ideation (Eckblad & Chapman, 1983) and Perceptual Aberration (Chapman, Chapman, & Raulin, 1978) scales and negative symptom schizotypy as measured by the Physical Anhedonia (Chapman, Chapman, & Raulin, 1976) and Revised Social Anhedonia (Eckblad, Chapman, Chapman, & Mishlove, 1982; Mishlove & Chapman, 1985) scales. Previous studies suggest that these measures reflect the core symptoms found in schizotypal and schizoid personality disorder (Bailey, West, Widiger, & Freiman, 1993). Negative symptoms were significantly predicted by Neuroticism (+), Extraversion (-), Openness (-), and Agreeableness (-) domains of the Revised NEO Personality Inventory (NEO-PI-R; Costa & McCrae, 1992). Additionally, positive symptoms were significantly predicted by Neuroticism (+), Openness (+), and Agreeableness (-). In addition, we examined the validity of lower order traits in describing these symptoms of character pathology. These findings lend further support for the use of domain and facet scales of the NEO-PI-R in the identification of personality pathology

    Psychopathy and the Five Factor Model in a Noninstitutionalized Sample: A Domain and Facet Level Analysis

    The current study examined the relationship of the Five Factor Model (FFM) of personality to primary and secondary psychopathic dispositions in a noninstitutionalized sample. Previous investigations suggest that Agreeableness, Conscientiousness, and Neuroticism are basic personality traits that characterize psychopathy. However, few studies have examined the relationship of the FFM to primary and secondary psychopathic attributes, respectively. In the current study, the relationship of the FFM using the NEO-PI-R to primary and secondary psychopathic dispositions was investigated in a sample of young adults. Previous findings were extended by (1) addressing the relationship of higher and lower order FFM traits (i.e., facet scales) to primary and secondary psychopathy in a subclinical sample and (2) examining sex differences in FFM traits in relation to these two psychopathic dispositions. Although a number of differences were found between men and women for the FFM in terms of primary psychopathy, few sex differences were noted with respect to secondary psychopathy. In addition, examination of facet scale relationships to primary and secondary psychopathy further clarify the role of the FFM in psychopathy. These findings lend further support to the use of domain and facet scales of the NEO-PI-R in the identification of personality pathology in noninstitutionalized populations

    Evaluating artificial shelter arrays as a minimally invasive monitoring tool for the hellbender \u3ci\u3eCryptobranchus alleganiensis\u3c/i\u3e

    Hellbenders Cryptobranchus alleganiensis are critically imperiled amphibians throughout the eastern USA. Rock-lifting is widely used to monitor hellbenders but can severely disturb habitat. We asked whether artificial shelter occupancy (the proportion of occupied shelters in an array) would function as a proxy for hellbender abundance and thereby serve as a viable alternative to rock-lifting. We hypothesized that shelter occupancy would vary spatially in response to hellbender density, natural shelter density, or both, and would vary temporally with hellbender seasonal activity patterns and time since shelter deployment. We established shelter arrays (n = 30 shelters each) in 6 stream reaches and monitored them monthly for up to 2 yr. We used Bayesian mixed logistic regression and model ranking criteria to assess support for hypotheses concerning drivers of shelter occupancy. In all reaches, shelter occupancy was highest from June-August each year and was higher in Year 2 relative to Year 1. Our best-supported model indicated that the extent of boulder and bedrock (hereafter, natural shelter) in a reach mediated the relationship between hellbender abundance and shelter occupancy. More explicitly, shelter occupancy was positively correlated with abundance when natural shelter covered \u3c20% of a reach, but uncorrelated with abundance when natural shelter was more abundant. While shelter occupancy should not be used to infer variation in hellbender relative abundance when substrate composition varies among reaches, we showed that artificial shelters can function as valuable monitoring tools when reaches meet certain criteria, though regular shelter maintenance is critical

These foods contain probiotic bacteria which are described as live microbial supplements that improve the intestinal microbial balance and are intended for maintenance of health and/or the prevention of disease. Probiotic bacteria for human use must be proven to be safe and beneficial, and should preferably be of human origin as evidence suggests that these bacteria are species specific and perform best in the species from which they were isolated. They must also retain both viability and efficacy in a particular food product throughout its shelf-life, and following consumption. Above all however, probiotic food products must be proved effective in controlled validated clinical trials. Dairy foods, including in particular, fermented milks and yogurt are among the best accepted food carriers for probiotic cultures.     Unbending Gender: Why Family and Work Conflict and What To Do About It (Panel Two: Who\u27s Minding the Baby?)

    A central characteristic of our current gender arrangements is that they pit ideal worker women against marginalized caregiver women in a series of patterned conflicts I call gender wars. One version of these are the mommy wars that we see often covered in the press between employed mothers and mothers at home. Employed mothers at times participate in the belittlement commonly felt by homemakers. Also mothers at home, I think, at times participate in the guilt-tripping that\u27s often felt by mothers who are employed. These gender wars are a central but little understood characteristic of the gender system that grew up after 1780, which historians call domesticity. One of the basic arguments in the book is that gender has proved unbending in the sense that we\u27ve progressed from the original form of domesticity, the breadwinner/housewife version, to the contemporary form of domesticity, the ideal worker-marginalized caregiver system. This modern form is what I sometimes call an attempt again to invent a language that is widely accessible, the dominant domestic ecology. I found that if you call it the sex-gender system, people feel somewhat differently than if you call it the dominant family ecology. The important point is that these gender wars, which are an inherent characteristic of domesticity, are seriously undertheorized by feminist theorists. I think they\u27re really important, because they go to the core of building an effective coalition for gender change with respect to this work-family axis, these economic meanings of gender that Adrienne and I are focusing on. The classic strategy of American feminists has been that women should achieve equality by performing as ideal workers along with the men, with child care delegated to the market. I call this the full commodification model, until Adrienne came up with a far better name. She calls it the delegation model. So I\u27ll call it the delegation model. Delegation to the market in this country, which was originally conceived of to involve some degree of social subsidy, has become delegation to a largely unsubsidized market in which child care workers are among the lowest-paid workers in the society. They also have extremely high rates of turnover, which is not good because children need continuity of care. The result is a delegation model that is not likely to appeal to nonprivileged people, because in this social context, delegation means that working-class people, who a generation ago had access to the same kind of mothercare that middle-class people had, today have only market child care that reflects their disadvantaged class position

    Influence of GABA and GABA-producing Lactobacillus brevis DPC 6108 on the development of diabetes in a streptozotocin rat model

    peer-reviewedThe aim of this study was to investigate if dietary administration of γ-aminobutyric acid (GABA)-producing Lactobacillus brevis DPC 6108 and pure GABA exert protective effects against the development of diabetes in streptozotocin (STZ)-induced diabetic Sprague Dawley rats. In a first experiment, healthy rats were divided in 3 groups (n=10/group) receiving placebo, 2.6 mg/kg body weight (bw) pure GABA or L. brevis DPC 6108 (~109microorganisms). In a second experiment, rats (n=15/group) were randomised to five groups and four of these received an injection of STZ to induce type 1 diabetes. Diabetic and non-diabetic controls received placebo [4% (w/v) yeast extract in dH2O], while the other three diabetic groups received one of the following dietary supplements: 2.6 mg/kg bw GABA (low GABA), 200 mg/kg bw GABA (high GABA) or ~109 L. brevis DPC 6108. L. brevis DPC 6108 supplementation was associated with increased serum insulin levels (P0.05), compared with non-diabetic controls while all other diabetic groups displayed reduced diversity (P<0.05). L. brevis DPC 6108 attenuated hyperglycaemia induced by diabetes but additional studies are needed to understand the mechanisms involved in this reduction.The authors and their work were supported by the APC Microbiome Institute.     Observation of Amounts of Movement Practice Provided during Stroke Rehabilitation

    Objective To investigate how much movement practice occurred during stroke rehabilitation, and what factors might influence doses of practice provided. Design Observational survey of stroke therapy sessions. Setting Seven inpatient and outpatient rehabilitation sites. Participants We observed a convenience sample of 312 physical and occupational therapy sessions for people with stroke. Interventions Not applicable. Main Outcome Measures We recorded numbers of repetitions in specific movement categories and data on potential modifying factors (patient age, side affected, time since stroke, FIM item scores, years of therapist experience). Descriptive statistics were used to characterize amounts of practice. Correlation and regression analyses were used to determine whether potential factors were related to the amount of practice in the 2 important categories of upper extremity functional movements and gait steps. Results Practice of task-specific, functional upper extremity movements occurred in 51% of the sessions that addressed upper limb rehabilitation, and the average number of repetitions/session was 32 (95% confidence interval [CI]=20–44). Practice of gait occurred in 84% of sessions that addressed lower limb rehabilitation and the average number of gait steps/session was 357 (95% CI=296–418). None of the potential factors listed accounted for significant variance in the amount of practice in either of these 2 categories. Conclusions The amount of practice provided during poststroke rehabilitation is small compared with animal models. It is possible that current doses of task-specific practice during rehabilitation are not adequate to drive the neural reorganization needed to promote function poststroke optimally

    Breast Milk, a Source of Beneficial Microbes and Associated Benefits for Infant Health

    peer-reviewedHuman breast milk is considered the optimum feeding regime for newborn infants due to its ability to provide complete nutrition and many bioactive health factors. Breast feeding is associated with improved infant health and immune development, less incidences of gastrointestinal disease and lower mortality rates than formula fed infants. As well as providing fundamental nutrients to the growing infant, breast milk is a source of commensal bacteria which further enhance infant health by preventing pathogen adhesion and promoting gut colonisation of beneficial microbes. While breast milk was initially considered a sterile fluid and microbes isolated were considered contaminants, it is now widely accepted that breast milk is home to its own unique microbiome. The origins of bacteria in breast milk have been subject to much debate, however, the possibility of an entero-mammary pathway allowing for transfer of microbes from maternal gut to the mammary gland is one potential pathway. Human milk derived strains can be regarded as potential probiotics; therefore, many studies have focused on isolating strains from milk for subsequent use in infant health and nutrition markets. This review aims to discuss mammary gland development in preparation for lactation as well as explore the microbial composition and origins of the human milk microbiota with a focus on probiotic development

    Lactic Acid Bacteria and Bifidobacteria with Potential to Design Natural Biofunctional Health-Promoting Dairy Foods

    peer-reviewedConsumer interest in healthy lifestyle and health-promoting natural products is a major driving force for the increasing global demand of biofunctional dairy foods. A number of commercial sources sell synthetic formulations of bioactive substances for use as dietary supplements. However, the bioactive-enrichment of health-oriented foods by naturally occurring microorganisms during dairy fermentation is in increased demand. While participating in milk fermentation, lactic acid bacteria can be exploited in situ as microbial sources for naturally enriching dairy products with a broad range of bioactive components that may cover different health aspects. Several of these bioactive metabolites are industrially and economically important, as they are claimed to exert diverse health-promoting activities on the consumer, such as anti-hypertensive, anti-inflammatory, and anti-diabetic, anti-oxidative, immune-modulatory, anti-cholesterolemic, or microbiome modulation.
