Роль психоэмоциональных факторов при ведении пациентов с хроническим вирусным гепатитом С
USMF Nicolae Testemiţanu, Clinica Medicală nr. 4 GastroenterologieSummary. Chronic viral hepatitis C is characterized by high comorbidity with depressive symptoms that affect patient’s quality of life. In this study we have
examined associations between depression, clinical presentation, liver
process activity, quality of life in HCVC. The study included 42 patients
with chronic viral hepatitis C. We analyzed clinical and biochemical
parameters, psychometric testing appreciated the presence and degree of
depression (Hamilton test), the quality of life was revealed using SF-36.
All patients’ data were compared with results from the control group - 29
healthy individuals. In this study we determined the high comorbidity of
chronic hepatitis C with depression (85%), which was responsible for the
clinical presentation. HCVC people with depression tended to negate the
effects of previous treatments, showing a lower satisfaction with health
care in the past. We determined the associations between depressive
status and immunologic imbalance manifested by CIC concentration.
The low quality of life in chronic viral hepatitis C was determined, and
depends, in particular, on depressive state of patients but was not infl uenced by parameters of cytolytic syndrome.Резюме. Хронический вирусный гепатит С характеризуется высокой коморбидностью с депрессивными симптомами, что обуславливает
снижение качества жизни больных. В данной работе были изучены
взаимные связи между депрессией, особенностями клинической
картины, активностью печеночного процесса, качеством жизни
больных с хроническим вирусным гепатитом С (ХВГС). Было обследовано 42 пациента с ХВГС с проведением клинического осмотра,
биохимических исследований и психометрического тестирования
для выявления депрессии (тест Гамильтона) и определения качества жизни (опросник SF-36). Данные, полученные в группе больных,
сравнивали с результатами контрольной группы – 29 практически
здоровых лиц. Была выявлена высокая коморбидность ХВГС с
депрессией (85%), которая приобрела соматическое выражение
и обусловила выраженность клинической картины. Депрессивные пациенты были более склонны отрицать эффективность
предыдущих курсов лечения и были субъективно менее удовлетворены медицинской помощью в прошлом. Выявлена связь между
депрессией и иммунным дисбалансом, выраженным повышением
концентрации ЦИК. Снижение качества жизни пациентов с ХВГС
было обусловлено, в основном, наличием депрессии и не зависело от
выраженности цитолиза

Summary:
Chronic liver diseases, including viral hepatitis, have a
negative impact on the quality of life (QL), but few studies
have evaluated the quality of life in chronic viral hepatitis B
(CVH B). The purpose of this study was to evaluate the
QL in patients with CVH B and the impact of various factors on QL. We investigated 35 patients with CVH B, using
clinical methods and laboratory tests. Psychometric testing
appreciated the presence and degree of depression (Hamilton test), the quality of life was revealed using SF-36. All
patients’ data were compared with results from the control
group - 29 healthy individuals. Results. CVH B patients
had significantly lower total SF-36 scores than healthy
individuals (51,85±2,10 vs 74,90±1,09, p<0,001), with
better results in men and in people younger than 40 years.
The majority of patients (74%) have had positive depressive
scores, in 48% of patients depression was sub clinic (mild
depression). Total SF-36 scores (also all scales of the test) were
lower in depressive CVH B patients in comparison with
non depressive ones (47,02±1,88 vs 68,83±2,94, p<0,001).
The higher depressive scores were associated with the lowest
quality of life (41,19±2,23). The activity of transaminazes and the type of viral infection (presence of HBeAg) have
not influenced QL in CVH B patients. Conclusion. Chronic
viral hepatitis B has a negative impact on the quality of life due
to alteration of emotional state and depressive reactions (in
74% of patients), with better results in men and in people
younger than 40 years. The activity of transaminazes and
the presence of HBeAg has not influenced the quality of life in
CVH B patients.Резюме:
Хронические заболевания печени, в том числе и хронические вирусные гепатиты, сопровождаются снижением качества жизни (КЖ) больных. Однако, качество
жизни при хроническом вирусном гепатите В не достаточно исследовано. Целью данного исследования
было оценить качество жизни у больных хроническим
гепатитом В ( ХВГ В) и факторы влияющие на их КЖ.
Материал и методы. Обследовано 35 больных с ХВГ В
с проведением клинического осмотра, биохимических
исследований и психометрического тестирования для
выявления депрессии (тест Гамильтона) и определения
качества жизни (опросник SF-36). Данные, полученные в
группе больных, сравнили с результатами контрольной
группы – 29 практически здоровых лиц. Результаты
показали, что в группе пациентов с ХВГ В качество
жизни было значительно ниже по сравнению с контрольной группой (47,02±1,88 vs 68,83±2,94, p<0,001).
У мужчин и у лиц моложе 40 лет данный показатель
был несколько лучше. У большинства пациентов (74%)
с ХВГ В было выявлено депрессивное состояние, у 48%
больных определили легкую степень депрессии. Общие
показатели КЖ (а также все шкалы теста) были ниже
в группе депрессивных больных в сравнении с больными
ХВГ В без депрессии (47,02±1,88 vs 68,83±2,94, p<0,001).
Самые низкие показатели КЖ выявлены у больных
со средне-высоким уровнем депрессии (41,19±2,23).
Активность трансаминаз и фаза вирусной инфекции
(наличие HВeAg) не повлияли на КЖ у обследованных
лиц с ХВГ В. Выводы. Хронический вирусный гепатит В
сопровождается снижением качества жизни больных,
обусловленным психоэмоциональным статусом, в частности депрессивным состоянием (у 74%). Показатели
КЖ несколько лучше у мужчин, у лиц моложе 40 лет.

The FGID symptoms result from the interaction of pathophysiological mechanisms with psychological factors. Emotional problems in persons with alexithymia may be expressed at somatic level (the process of somatization) and led to poor quality of life. Alexithymia is the psychological construct, specific for psychosomatic patients, and have been defined as the difficulty in recognition and description of emotions. FGIDs treatment should be carried out taking into account the psychological characteristics of the patient, including the correction of alexithymia.Пациенты с функциональными желудочно-кишечными расстройствами (ФЖКР) представляют сложный контингент с диагностической и терапевтической точек зрения. Симптомы ФЖКР являются результатом взаимодействия патофизиологических механизмов с психологическими факторами, которые у алекситимичных лиц могут находить соматичкское выражение (процесс соматизации) и обусловливают снижение качества жизни. Алекситимия, психологический конструкт, характерный для психосоматических больных, была определена как сложность в распознавании и описании собственных и чужих эмоций. Диспептические симптомы и симптомы рефлюкса – соматическое выражение нозогенной депрессии у лиц с диагнозом хронического вирусного гепатита?

Laboratorul GastroenterologieThe disclosure of viral hepatitis is a stressful event that involves the threat of death through the cirrhosis and its complications. Psychological factors may play a causal role in the pathogenesis of some dyspeptic and reflux symptoms. Aim of our study was to determine the possible impact of depresive state on the prevalence of dyspeptic and reflux symptoms after the confirmation of diagnosis of chronic viral hepatitis. Rezults. In 147 patients with chronic viral hepatitis (CVH) we found a large prevalence of depression (85%). After the disclosure of CVH, especially in depressive participants, the hearburn, eructation, epigastric pain and postprandial dyspeptic symptoms were found more often in comparison with anamnestic data before the diagnosis confirmation. CVH patients had lower scores of quality of life. Concluzion. CVH disclosure is a stressful event, which may lead to depression with reflection on somatic level, manifested by dyspeptic and reflux symptoms and worsening of quality of life.Обнаружение хронического вирусного гепатита может вызывать психогенную реакцию, сопровождающуюся депрессией, озабоченностью и страхом перед возможным прогрессированием болезни, развитием цирроза и его последствиями. Эмоциональные факторы участвуют в патогенезе диспепсических симптомов. Цель: выявить возможные влияния эмоционального депрессивного статуса людей с диагностированным хроническим вирусным гепатитом (ХВГ) на частоту диспепсических симптомов рефлюкса. Результаты. Среди 147 обследованных больных с ХВГ была выявлена депрессия в 85% случаев. После обнаружения ХВГ, особенно у депрессивных пациентов, частота изжоги, отрыжки, эпигастральной боли и постпрандиальных диспепсических симптомов была выше чем до постановки диагноза ХВГ. Показатели качества жизни при ХВГ были значительно ниже по сравнению с таковыми в контрольной группе (29 практически здоровых лиц). Выводы. Обнаружение ХВГ может играть психотравмирующую роль, способствуя развитию депрессивных состояний с возможным соматическим выражением в виде диспепсических симптомов рефлюкс

Knouwledge about the psychosocial aspects of the functional gastrointestinal disorders (FGIDs) is important in understanding their pathogenesis and treatment. Chronic viral hepatitis C and depression

depression in patients with hepatitis C is important because psychiatric complications of
interferon therapy may represent a serious obstacle for antiviral therapy. Management and
correction of mental condition in these patients contributes to better adherence to therapy,
improve patient quality of life and treatment outcomes.
Hepatita cronică virală C deseori este acompaniată de simptome depresive. Screeningul
depresiei la pacienţii cu hepatita C este important din cauza că complicaţiile psihiatrice ale

Particularities of the chemical composition of dental enamel in children with neuromotor disabilities and gastro-esophageal reflux disease
Abstract. Objective: The purpose of the hereby paper consisted in the study of particularities of the chemical composition of dental enamel in
children suffering from severe neuromotor disabilities associated with gastro-esophageal reflux disease (GERD). Material and methods. Using
the Fourier transform spectroscopy method (FTIR) we have studied the structure and the chemical composition of various areas of the dental
enamel, namely 64 pieces of enamel prepared from 32 teeth extracted from children aged between 13 and 17, following orthodontic indications.
Results: We established that in the enamel of the children suffering from severe neuromotor disabilities associated with GERD the weight of
the organic component of enamel is increased, the content of hydroxyapatite content is reduced, it contains carbon substituted hydroxyapatite
with low intensity of peak phosphorus and significant increase of organic components. The detected particularities of the chemical composition
of dental enamel indicate the considerable reduction of the enamel resistance to acid attack, thus leading to a high risk of dental erosion or new
carious lesions. Conclusions: The FTIR spectroscopy method of dental enamel offers new opportunities in the study of pathogenic mechanisms
involved in the initiation and evolution of carious lesions and dental erosions, to make a prediction of these disorders, to elaborate measures for
the prevention of tooth decay and evaluate their efficienc
