    XYZ Biotech: A Fictional Business Development Case Utilizing Quantitative Tools to Evaluate a New Antibody-Drug-Conjugate Opportunity for Metastatic Breast Cancer

    Multiple considerations must be evaluated with a new acquisition or license in biopharmaceutical business development. This case utilizes a fictional company, XYZ Biotech, to illustrate the quantitative considerations in evaluating an antibody-drug combination product in the metastatic breast cancer (mBC) space. The steps shown in this case also include the development of a market model to define the potential size of the market opportunity, a weighted benefit pricing model to evaluate the present price, and a net present value model to estimate projections from the product. The case also engages multiple qualitative considerations as part of the discussion section. It concludes by offering several nuanced questions at the end to stimulate discussion around the analysis, probe additional considerations needed for a business case, and analyze what other elements or issues would drive a decision. In the appendix is a short case summary of key facts for participants to develop solutions and share in class before reviewing the case narrative

    Occupational Therapy in Veterans Treatment Court

    Veterans may face difficulty transitioning back into a civilian lifestyle. Their difficulties may be attributed to PTSD, TBI, sexual trauma, anxiety, depression, unemployment, and more. Due to these obstacles, veterans may commit crimes that place them in the Veterans Treatment Court (VTC). It was noted in October of 2018 that there were possible gaps in service for our veterans. The purpose of this project was to create an occupational therapy-based curriculum that will reduce the recidivism rate in St. Johns County VTC. The OT-based manual is intended to replace negative habits, roles, and routines with positive ones in order to increase quality of life, health, and well-being of the veterans. This program is theory-driven and evidence-derived. The needs of the veterans were identified through the utilization of the AOTA Occupational Profile Template and therapeutic interviewing. Evaluation and intervention of the veterans regarding roles, habits, routines that support or detract from recovery, life skills, cognitive, sensory, and motor factors have been included as well.https://soar.usa.edu/otdcapstonespring2020/1000/thumbnail.jp

    The applications of the tetracycline class of antibiotics extends beyond their antimicrobial activity to anti-inflammatory, immunosuppressive and neuroprotective applications making it a commonly used class of medication. Minocycline, a second generation tetracycline, has inherent characteristics that improve absorption and distribution. These benefits promote even more wide spread use. This familiarity of usage breeds prescriptive complacency toward the dermatologic complications including hyperpigmentation.     Radiative Decay Rate and Branching Fractions of MgF

    We report measured and calculated values of radiative decay rates and vibrational branching fractions for the A2Π^2\Pi state of MgF. The decay rate measurements use time-correlated single photon counting with roughly 1% total uncertainty. Branching-fraction measurements are performed using two calibrated imaging systems to achieve few percent total uncertainty. We use the highly accurate multireference relativistic ab initio methods to calculate the Franck-Condon factors and transition dipole moments required to determine the decay rates and the branching fractions. The measurements provide a precision benchmark for testing the accuracy of the molecular structure calculations. The determination of the decay rate and vibrational branching fractions can be used to inform future optical cycling and laser cooling schemes for the MgF molecule

    Inhibitors of Foot and Mouth Disease Virus Targeting a Novel Pocket of the RNA-Dependent RNA Polymerase

    Foot-and-Mouth Disease Virus (FMDV) is a picornavirus that infects cloven-hoofed animals and leads to severe losses in livestock production. In the case of an FMD outbreak, emergency vaccination requires at least 7 days to trigger an effective immune response. There are currently no approved inhibitors for the treatment or prevention of FMDV infections.Using a luciferase-based assay we screened a library of compounds and identified seven novel inhibitors of 3Dpol, the RNA-dependent RNA polymerase of FMDV. The compounds inhibited specifically 3Dpol (IC(50)s from 2-17 µM) and not other viral or bacterial polymerases. Enzyme kinetic studies on the inhibition mechanism by compounds 5D9 and 7F8 showed that they are non-competitive inhibitors with respect to NTP and nucleic acid substrates. Molecular modeling and docking studies into the 3Dpol structure revealed an inhibitor binding pocket proximal to, but distinct from the 3Dpol catalytic site. Residues surrounding this pocket are conserved among all 60 FMDV subtypes. Site directed mutagenesis of two residues located at either side of the pocket caused distinct resistance to the compounds, demonstrating that they indeed bind at this site. Several compounds inhibited viral replication with 5D9 suppressing virus production in FMDV-infected cells with EC(50) = 12 µM and EC(90) = 20 µM).We identified several non-competitive inhibitors of FMDV 3Dpol that target a novel binding pocket, which can be used for future structure-based drug design studies. Such studies can lead to the discovery of even more potent antivirals that could provide alternative or supplementary options to contain future outbreaks of FMD

    Coronary computed tomography angiography investigation of the association between left main coronary artery bifurcation angle and risk factors of coronary artery disease

    To explore the association between the left main coronary artery bifurcation angle and common atherosclerotic risk factors with regard to the development of coronary artery disease (CAD) using coronary computed tomography angiography (CCTA). A retrospective review of 196 CCTA cases (129 males, 67 females, mean age 58 ± 10.5 years) was conducted. The bifurcation angle between the left anterior descending (LAD) and left circumflex (LCx) was measured on two-dimensional (2D) and three-dimensional (3D) reconstructed images and the type of plaque and degree of lumen stenosis was assessed to determine the disease severity. An association between bifurcation angle and patient risk factors [gender, body mass index (BMI), hypertension, cholesterol, diabetes, smoking and family history] of CAD was also assessed to demonstrate the relationship between these variables. The mean bifurcation angle between the LAD and LCx was 79.40° ± 22.97°, ranging from 35.5° to 178°. Gender and BMI were found to have significant associations with bifurcation angle. Males were at 2.07-fold greater risk of having a >80° bifurcation angle and developing CAD than females (P = 0.003), and patients with high BMI (>25 kg/m2) were 2.54-fold more likely to have a >80° bifurcation angle than patients with a normal BMI (P = 0.001) and thus were at greater risk of developing CAD. There is a direct relationship between the left main coronary artery bifurcation angle and patient gender and BMI. Measurement of the bifurcation angle should be incorporated into clinical practice to identify patients at high risk of developing CAD

    Vimentin regulates Notch signaling strength and arterial remodeling in response to hemodynamic stress

    The intermediate filament (IF) cytoskeleton has been proposed to regulate morphogenic processes by integrating the cell fate signaling machinery with mechanical cues. Signaling between endothelial cells (ECs) and vascular smooth muscle cells (VSMCs) through the Notch pathway regulates arterial remodeling in response to changes in blood flow. Here we show that the IF-protein vimentin regulates Notch signaling strength and arterial remodeling in response to hemodynamic forces. Vimentin is important for Notch transactivation by ECs and vimentin knockout mice (VimKO) display disrupted VSMC differentiation and adverse remodeling in aortic explants and in vivo. Shear stress increases Jagged1 levels and Notch activation in a vimentin-dependent manner. Shear stress induces phosphorylation of vimentin at serine 38 and phosphorylated vimentin interacts with Jagged1 and increases Notch activation potential. Reduced Jagged1-Notch transactivation strength disrupts lateral signal induction through the arterial wall leading to adverse remodeling. Taken together we demonstrate that vimentin forms a central part of a mechanochemical transduction pathway that regulates multilayer communication and structural homeostasis of the arterial wall
