Mental health beds in general hospitals: the case of Rio de Janeiro

As questões de estudo levantadas na pesquisa pretendem descrever o modelo de provisão de leitos de saúde mental em hospitais gerais na cidade do Rio de Janeiro, identificar o papel dos leitos de saúde mental em hospitais gerais na Rede de Atenção Psicossocial e verificar a adequação dos leitos em hospitais gerais às diretrizes do Ministério da Saúde. Para realização deste estudo, optou-se por um estudo de caso da situação atual dos leitos de saúde mental em hospital geral existentes na cidade. A pesquisa foi constituída de entrevistas individuais com roteiros semiestruturados. Foram pesquisados bancos de dados do Sistema Único de Saúde (SUS) e documentos oficiais sobre o tema. Os dados encontrados foram categorizados e confrontados com a bibliografia de referência. Os resultados revelaram a existência de poucos leitos de saúde mental na rede hospitalar municipal. Os leitos existentes funcionam integrados a Rede de Atenção Psicossocial (RAPS), com participação variável dos Centros de Atenção Psicossocial (CAPS) na condução de projetos terapêuticos individuais; apresentam média de permanência inferior a 20 dias e acontecem em espaços restritivos, com pouca estimulação. Além disso, possuem regras rígidas para os pacientes, submetidas fortemente ao modelo biomédico. A cidade do Rio de Janeiro vem promovendo a inversão do modelo hospitalar para o modelo assistencial comunitário. Estruturas antigas de cuidado e estruturas contemporâneas convivem em um sistema que se apresenta em transição.The objective of this study is to analyze the management and functional conditions of the Rio de Janeiro Reference Hospital Service relative to the care of mental illness. The study also seeks to describe the organizational arrangement of the provision of mental health beds in general hospitals in Rio de Janeiro, to identify the role of mental health beds in general hospitals in the Psychosocial Attention Network (RAPS) and to verify the adequacy of the beds in general hospitals to the guidelines of the Ministry of Health. To carry out this study, we chose a case study of the current situation of mental health beds in a general hospital in the city. The research consisted of individual interviews with semi-structured scripts. We researched databases of SUS (National Health Care Organization) as well as official documents on the subject. The data was categorized and compared with the reference bibliography. The results revealed the lack of an adequate number of mental health beds in the municipal hospital network. The existing beds are integrated with RAPS, with varying participation of the Psychosocial Attention Centers (CAPS) in the conduct of individual therapeutic projects. They present an average stay of less than 20 days and happen in restrictive spaces, with little stimulation. They have rigid rules for patients, strongly subject to the biomedical model. The city of Rio de Janeiro has been promoting the inversion of the hospital model to the community care model. Old structures of care and contemporary structures coexist in a system that presents itself in transition
