    Fiberendoscopic monitorized dilatation of oesophageal strictures in children

    Oesophageal dilatations were performed 55 times in 10 children aged 2 months to 8 years presenting with acquired strictures of several origins. Classical bougienage with rubber material and newer technique with the Eder-Puestow's metallic olives system were used successively.     Over a period of 20 yr, the authors have carried out 1420 high endoscopies as an emergency for hematemesis and/or melena. Out of these 1420 endoscopies 122 (8.6%) were carried out in cases of definitive cirrhosis and 64 (4.5%) in chronic alcoholics. Among the patients with cirrhosis of the liver, esogastric endoscopy showed the presence of esophageal varices in 86 cases (70.5%). In 31 of them (25.4%) rupture of the varices was demonstrated. This study included 3 periods, depending on the type of endoscope used: from 1952 to 1966, gastroflex OPL; from 1966 to 1971, gastrofibroscope Olympus GIFA and GFB, esophagoscope Olympus EF; and between 1971-1972, introduction of the JF duodenoscope and the GIFD gastro bulboscope. The percentage of undetermined hemorrhagic lesions passed from 40% in 1952-1966 to 28% from 1966-1971, and 4.5% between 1971-1972. The lesions responsible for the hemorrhage in cirrhotic patients were rupture of varices in 25.4% of cases, erosive gastritis in 23%, gastric ulcer in 10%, Mallory Weiss syndrome in 4.9% and duodenal ulcer in 4%. In alcoholics the most frequent lesion was gastric ulcer in 36% of cases, followed by erosive gastritis in 26%; Mallory Weiss syndrome was noted in 20% of cases, and duodenal ulcer in 9%. A certain number of patients had several hemorrhagic lesions on endoscopy; the association of varices and erosive gastritis was the most common.     This article reports the results of our first 100 examinations of the upper gastrointestinal tract and the colon in children and infants, using standard fiberscopes and also pediatric models. The procedure could be performed under sedation in 80% of the cases and general anesthesia was required mainly with standard fiberscopes. Compared to roentgenographic findings, fiberendoscopy proved to be more reliable in superficial mucosal lesions and gastrointestinal bleeding. Colonoscopy, though difficult, is valuable in diagnosing rectal bleeding lesions and as a follow-up technique in inflammatory bowel diseases. © 1977, American Medical Association.     Colonic endoscopy in children

    From 1971 to 1974, 76 colonoscopies were performed in children aged 3 months to 16 years, using conventional fibrescopes as well as 3 different pediatric models. The main indications were: rectal bleeding, inflammatory bowel disease and parasitic infestations. Fiber endoscopy proved to be more valuable for the diagnosis of rectal bleeding than X ray examination. In inflammatory bowel disease, colonoscopy was performed for control of borderline abnormal X ray pictures or as a followup of known cases of colonic Crohn disease or ulcerative colitis.     L'œsophago-gastro-duodenoscopie d'urgence dans les hémorragies aiguës du tractus digestif supérieur

    L'endoscopie d'urgence dans les hémorragies digestives a permis de reconnaïtre les lésions qui ne sont pas chirurgicales, comme le syndrome de Mallory Wyeth, la gastrite érosive hémorragique, les œsophagites hémorragiques. En ce qui concerne les ulcères gastroduodénaux, si la localisation d'ulcère est très importante à notre avis comme indication à donner avant l'intervention, on a le sentiment que l'endoscopie permettra, dans l'avenir, de reconnaître les ulcères impliquant une thérapeutique chirurgicale et ceux qui, au contraire, pourront bénéficier du traitement médical. La précision du diagnostic endoscopique dans les hémorragies digestives hautes, déjà excellente avec les fibroscopes à vue axiale, est passée à 94% dans notre série depuis l'emploid du duodénoscope à vue latérale. Cette évolution a été favorisée par un nombre accru d'endoscopies précoces. En conséquence, nous préconisons de pratiquer l'endoscopie dans les vingt-quatre heures de l'hémorragie digestive, la compensation volémique puvant être obtenue dans ces délais. Par ailleurs, nous conseillons l'utilisation en premier lieu d'un fibroscope à vision axiale, en deuxième lieu, et systématiquement, d'un duodénoscope à vision latérale. En cas de suspicion de lésion de la poche à air, nous utilisons un troisième appareil qui est le gastroscope à vision latérale. L'endoscopie d'urgence, actuellement reconnue partout comme l'investigation diagnostique la plus rentable dans les hémorragies digestives hautes, est l'aboutissement des idées défendues depuis longtemps par Desneux, ce qui explique le nombre croissant des cas investigués au cours de ces dernières années. © 1972, Springer-Verlag.     Update on ovarian hyperstimulation syndrome

    The ovarian hyperstimulation syndrome (OHSS) is the most important complication of the pharmacological ovulation induction. Exclusively postovulatory, it is clinically characterized by a massive ovarian enlargement associated with an acute third-space fluid shift responsible for the development of ascites, and sometimes pleural and/or pericardial effusion. While mild OHSS has no consequence, severe forms can be life-threatening because of associated hemodynamic troubles. The main risk factors are the polycystic ovarian syndrome and an explosive ovarian response to the stimulation characterized by high serum oestradiol levels and an increased number of follicles. Ultrasound and endocrine monitoring make prevention measures possible, mainly by either abandoning the stimulation cycle or, during in vitro fertilization, cryopreserving the embryos for subsequent replacement in another cycle. Treatment consists in correcting circulatory and electrolyte imbalance. Paracentesis is more and more systematically proposed in the severe forms.SCOPUS: re.jinfo:eu-repo/semantics/publishe
