Local spillovers, production technology and the choice to make and/or buy. Empirical evidence from Emilia Romagna
By exploiting a new rich firm-level dataset, this paper investigates the decision to subcontract production activities (outsourcing) with respect to vertically integrate them. In particular, we aim at identifying the main factors underlying the decision to either fully or partially decentralise production activities by mechanical firms located in Emilia Romagna (Italy). In so doing, we first account for firm characteristics, such as size, age and the skill composition of the labour force, then we focus on labour costs per employee, product diversity and the presence of the firm on international markets. Finally, and differently from previous research, we include in the analysis both the qualitative composition of the production process, as given by the stages of production potentially developed by the firm, and the industrial composition of the local market. On this last purpose, we estimate the relationship between the propensity and the intensity of concurrent and total sourcing and the main sources of agglomeration economies identified in the literature: specialisation economies, variety and urbanisation economies. Our estimates show a particularly strong and positive relation between the intensity of 'pure' outsourcing and our measure of variety, workforce skill intensity and the internal composition of production, while a negative relation emerges with respect to firm size, age and labour cost. Results concerning concurrent sourcing, instead, appear weaker, but, differently from the case of full outsourcing, we nd a positive relationship with rm size and product diversity.
Acido urico, spettatore o artefice dell'aumentato rischio cardiovascolare e renale?
Numerosi studi epidemiologici sottolineano una relazione tra livelli di acido urico e condizioni cliniche quali ipertensione, insulinoresistenza e malattia renale e cardiovascolare. Infatti, con la rapida diffusione di tali condizioni, si è assistito al progressivo aumento dei valori medi di acido urico nella popolazione generale (da 3.5 mg/dL negli anni '20 a 6.5 mg/dL negli anni '70).
In questa revisione cercheremo di approfondire se l'acido urico possa essere considerato un promotore attivo del danno aterosclerotico o solo un marcatore indipendente di un aumentato rischio di malattia. Nonostante non vi sia ancora un'indicazione basata sulle evidenze al trattamento farmacologico dell'iperuricemia asintomatica, recenti studi suggeriscono una relazione diretta tra le variazioni di AU indotte dalla terapia e l'incidenza di eventi cardio-renali
Electronic transport, ionic activation energy and trapping phenomena in a polymer-hybrid halide perovskite composite
Abstract The exploitation of methylammonium lead iodide perovskite-polymer composites is a promising strategy for the preparation of photoactive thin layers for solar cells. The preparation of these composites is a simple fabrication method with improved moisture stability when compared to that of pristine perovskite films. To deepen the understanding of the charge transport properties of these films, we investigated charge carrier mobility, traps, and ion migration. For this purpose, we applied a combinatory measurement approach that proves how such composites can still retain an ambipolar charge transport nature and the same mobility values of the related perovskite. Furthermore, thermally stimulated current measurements revealed that the polymer influenced the creation of additional defects during film formation without affecting charge mobility. Finally, impedance spectroscopy measurements suggested the addition of starch may hinder ion migration, which would require larger activation energies to move ions in composite films. These results pave the way for new strategies of polymer-assisted perovskite film development
Antihypertensive Treatment in Diabetic Kidney Disease: The Need for a Patient-Centered Approach
Diabetic kidney disease affects up to forty percent of patients with diabetes during their lifespan. Prevention and treatment of diabetic kidney disease is currently based on optimal glucose and blood pressure control. Renin-angiotensin aldosterone inhibitors are considered the mainstay treatment for hypertension in diabetic patients, especially in the presence of albuminuria. Whether strict blood pressure reduction entails a favorable renal outcome also in non-albuminuric patients is at present unclear. Results of several clinical trials suggest that an overly aggressive blood pressure reduction, especially in the context of profound pharmacologic inhibition of the renin-angiotensin-aldosterone system may result in a paradoxical worsening of renal function. On the basis of this evidence, it is proposed that blood pressure reduction should be tailored in each individual patient according to renal phenotype
Breathing Abnormalities During Sleep and Wakefulness in Rett Syndrome: Clinical Relevance and Paradoxical Relationship With Circulating Pro-oxidant Markers
BackgroundBreathing abnormalities are common in Rett syndrome (RTT), a pervasive neurodevelopmental disorder almost exclusively affecting females. RTT is linked to mutations in the methyl-CpG-binding protein 2 (MeCP2) gene. Our aim was to assess the clinical relevance of apneas during sleep-wakefulness cycle in a population with RTT and the possible impact of apneas on circulating oxidative stress markers. MethodsFemale patients with a clinical diagnosis of typical RTT (n = 66), MECP2 gene mutation, and apneas were enrolled (mean age: 12.5 years). Baseline clinical severity, arterial blood gas analysis, and red blood cell count were assessed. Breathing was monitored during the wakefulness and sleep states (average recording time: 13 +/- 0.5 h) with a portable polygraphic screening device. According to prevalence of breath holdings, the population was categorized into the wakefulness apnea (WA) and sleep apnea (SA) groups, and apnea-hypopnea index (AHI) was calculated. The impact of respiratory events on oxidative stress was assessed by plasma and intra-erythrocyte non-protein-bound iron (P-NPBI and IE-NPBI, respectively), and plasma F-2-isoprostane (F-2-IsoP) assays. ResultsSignificant prevalence of obstructive apneas with values of AHI > 15 was present in 69.7% of the population with RTT. The group with SA showed significantly increased AHI values > 15 (p = 0.0032), total breath holding episodes (p = 0.007), and average SpO(2) (p = 0.0001) as well as lower nadir SpO(2) (p = 0.0004) compared with the patients with WAs. The subgroups of patients with WA and SA showed no significant differences in arterial blood gas analysis variables (p > 0.089). Decreased mean cell hemoglobin (MCH) (p = 0.038) was observed in the group with WAs. P-NPBI levels were significantly higher in the group with WA than in that with SAs (p = 0.0001). Stepwise multiple linear regression models showed WA being related to nadir SpO(2), average SpO(2), and P-NPBI (adjusted R-2 = 0.613, multiple correlation coefficient = 0.795 p < 0.0001), and P-NPBI being related to average SpO(2), blood PaCO2, red blood cell mean corpuscular volume (MCV), age, and topiramate treatment (adjusted R-2 = 0.551, multiple correlation coefficient = 0.765, p < 0.0001). ConclusionOur findings indicate that the impact of apneas in RTT is uneven according to the sleep-wakefulness cycle, and that plasma redox active iron represents a potential novel therapeutic target
Human osteoarthritic chondrocytes exposed to extremely low-frequency electromagnetic fields (ELF) and therapeutic application of musically modulated electromagnetic fields (TAMMEF) systems: a comparative study.
Osteoarthritis (OA) is the most common joint disease, characterized by matrix
degradation and changes in chondrocyte morphology and metabolism. Literature
reported that electromagnetic fields (EMFs) can produce benefits in OA patients,
even if EMFs mechanism of action is debated. Human osteoarthritic chondrocytes
isolated from femoral heads were cultured in vitro in bidimensional (2-D) flasks
and in three-dimensional (3-D) alginate beads to mimic closely cartilage
environment in vivo. Cells were exposed 30 min/day for 2 weeks to extremely
low-frequency electromagnetic field (ELF) with fixed frequency (100 Hz) and to
therapeutic application of musically modulated electromagnetic field (TAMMEF)
with variable frequencies, intensities, and waveforms. Cell viability was
measured at days 7 and 14, while healthy-cell density, heavily vacuolized (hv)
cell density, and cluster density were measured by light microscopy only for 3-D
cultures after treatments. Cell morphology was observed for 2-D and 3-D cultures
by transmission electron microscopy (TEM). Chondrocyte exposure to TAMMEF
enhances cell viability at days 7 and 14 compared to ELF. Light microscopy
analysis showed that TAMMEF enhances healthy-cell density, reduces hv-cell
density and clustering, compared to ELF. Furthermore, TEM analysis showed
different morphology for 2-D (fibroblast-like) and 3-D (rounded shape) cultures,
confirming light microscopy results. In conclusion, EMFs are effective and safe
for OA chondrocytes. TAMMEF can positively interfere with OA chondrocytes
representing an innovative non-pharmacological approach to treat OA

Metallothioneins as dynamic markers for brain disease in lysosomal disorders
Objective: To facilitate development of novel disease-modifying therapies for lysosomal storage disorder (LSDs) characterized by nervous system involvement such as metachromatic leukodystrophy (MLD), molecular markers for monitoring disease progression and therapeutic response are needed. To this end, we sought to identify blood transcripts associated with the progression of MLD. Methods: Genome-wide expression analysis was performed in primary T lymphocytes of 24 patients with MLD compared to 24 age- and sex-matched healthy controls. Genes associated with MLD were identified, confirmed on a quantitative polymerase chain reaction platform, and replicated in an independent patient cohort. mRNA and protein expression of the prioritized gene family of metallothioneins was evaluated in postmortem patient brains and in mouse models representing 6 other LSDs. Metallothionein expression during disease progression and in response to specific treatment was evaluated in 1 of the tested LSD mouse models. Finally, a set of in vitro studies was planned to dissect the biological functions exerted by this class of molecules. Results: Metallothionein genes were significantly overexpressed in T lymphocytes and brain of patients with MLD and generally marked nervous tissue damage in the LSDs here evaluated. Overexpression of metallothioneins correlated with measures of disease progression in mice and patients, whereas their levels decreased in mice upon therapeutic treatment. In vitro studies indicated that metallothionein expression is regulated in response to oxidative stress and inflammation, which are biochemical hallmarks of lysosomal storage diseases. Interpretation Metallothioneins are potential markers of neurologic disease processes and treatment response in LSDs
