    Patients’ preferences for selection of endpoints in cardiovascular clinical trials

    Background: To reduce the duration and overall costs of cardiovascular trials, use of the combined endpoints in trial design has become commonplace. Though this methodology may serve the needs of investigators and trial sponsors, the preferences of patients or potential trial subjects in the trial design process has not been studied. Objective: To determine the preferences of patients in the design of cardiovascular trials. Design: Participants were surveyed in a pilot study regarding preferences among various single endpoints commonly used in cardiovascular trials, preference for single vs. composite endpoints, and the likelihood of compliance with a heart medication if patients similar to them participated in the trial design process. Participants: One hundred adult English-speaking patients, 38% male, from a primary care ambulatory practice located in an urban setting. Key results: Among single endpoints, participants rated heart attack as significantly more important than death from other causes (4.53 vs. 3.69, p=0.004) on a scale of 1–6. Death from heart disease was rated as significantly more important than chest pain (4.73 vs. 2.47, p<0.001), angioplasty/PCI/CABG (4.73 vs. 2.43, p<0.001), and stroke (4.73 vs. 2.43, p<0.001). Participants also expressed a slight preference for combined endpoints over single endpoint (43% vs. 57%), incorporation of the opinions of the study patient population into the design of trials (48% vs. 41% for researchers), and a greater likelihood of medication compliance if patient preferences were considered during trial design (67% indicated a significant to major effect). Conclusions: Patients are able to make judgments and express preferences regarding trial design. They prefer that the opinions of the study population rather than the general population be incorporated into the design of the study. This novel approach to study design would not only incorporate patient preferences into medical decision making, but it also has the potential to improve compliance with cardiovascular medications

    Biosynthesis of rabbit haptoglobin: Chemical evidence for a single chain precursor

    AbstractThe primary translation product of the mRNA for rabbit haptoglobin was obtained from a rabbit reticulocyte lysate cell-free system by immunoprecipitation with an antiserum that was directed to the β chain of haptoglobin. Analysis of the translation product by gel electrophoresis and by protein sequencing analysis identified a single polypeptide of Mr 41 000. Sequence analysis established a signal region of 18 residues that was immediately followed by the α chain sequence. These results give strong evidence that haptoglobin is initially synthesized as a single chain composed of a signal peptide followed by α and β chain regions, respectively

    Ionic and electronic properties of the topological insulator Bi2_2Te2_2Se investigated using β\beta-detected nuclear magnetic relaxation and resonance of 8^8Li

    We report measurements on the high temperature ionic and low temperature electronic properties of the 3D topological insulator Bi2_2Te2_2Se using ion-implanted 8^8Li β\beta-detected nuclear magnetic relaxation and resonance. With implantation energies in the range 5-28 keV, the probes penetrate beyond the expected range of the topological surface state, but are still within 250 nm of the surface. At temperatures above ~150 K, spin-lattice relaxation measurements reveal isolated 8^8Li+^{+} diffusion with an activation energy EA=0.185(8)E_{A} = 0.185(8) eV and attempt frequency τ01=8(3)×1011\tau_{0}^{-1} = 8(3) \times 10^{11} s1^{-1} for atomic site-to-site hopping. At lower temperature, we find a linear Korringa-like relaxation mechanism with a field dependent slope and intercept, which is accompanied by an anomalous field dependence to the resonance shift. We suggest that these may be related to a strong contribution from orbital currents or the magnetic freezeout of charge carriers in this heavily compensated semiconductor, but that conventional theories are unable to account for the extent of the field dependence.     Estimating the feasibility of transition paths in extended finite state machines

    There has been significant interest in automating testing on the basis of an extended finite state machine (EFSM) model of the required behaviour of the implementation under test (IUT). Many test criteria require that certain parts of the EFSM are executed. For example, we may want to execute every transition of the EFSM. In order to find a test suite (set of input sequences) that achieves this we might first derive a set of paths through the EFSM that satisfy the criterion using, for example, algorithms from graph theory. We then attempt to produce input sequences that trigger these paths. Unfortunately, however, the EFSM might have infeasible paths and the problem of determining whether a path is feasible is generally undecidable. This paper describes an approach in which a fitness function is used to estimate how easy it is to find an input sequence to trigger a given path through an EFSM. Such a fitness function could be used in a search-based approach in which we search for a path with good fitness that achieves a test objective, such as executing a particular transition, and then search for an input sequence that triggers the path. If this second search fails then we search for another path with good fitness and repeat the process. We give a computationally inexpensive approach (fitness function) that estimates the feasibility of a path. In order to evaluate this fitness function we compared the fitness of a path with the ease with which an input sequence can be produced using search to trigger the path and we used random sampling in order to estimate this. The empirical evidence suggests that a reasonably good correlation (0.72 and 0.62) exists between the fitness of a path, produced using the proposed fitness function, and an estimate of the ease with which we can randomly generate an input sequence to trigger the path

    Integralni pristup sustavima energetske elektronike

    Today\u27s power electronics systems are typically manufactured using non-standard parts, resulting in labor-intensive manufacturing processes, increased cost and poor reliability. As a possible way to overcome these problems, this paper discusses an integrated approach to design and manufacture power electronics systems to improve performance, reliability and cost effectiveness. Addressed in the paper are the technologies being developed for integration of both power supplies and motor drives. These technologies include the planar metalization to eliminate bonding wires, the integration of power passives, the integration of current sensors, the development of power devices to facilitate integration as well as to improve performance, and the integration of necessary CAD tools to address the multidisciplinary aspects of integrated systems. The development of Integrated Power Electronics Modules (IPEMs) is demonstrated for two applications: (1) 1 kW asymmetrical half-bridge DC-DC converter and (2) 1–3 kW motor drive for heating, ventilation and air conditioning (HVAC). Electrical and thermal design tradeoffs of IPEMs and related enabling technologies are described in the paper.Današnji sustavi energetske elektronike se obično proizvode iz nestandardnih dijelova. Posljedica toga je laboratorijska proizvodnja elektroničkih učinskih pretvarača, povećani troškovi i smanjena pouzdanost. Jedan od mogućih načina prevladavanja ovih poteškoća jest integralni pristup projektiranju i proizvodnji sustava energetske elektronike. Posebice se razmatraju tehnologije razvijene za integraciju učinskih krugova i motora. Ove tehnologije uključuju postupke planarne metalizacije za izbjegavanje žičanih vodova, integraciju pasivnih dijelova učinskih krugova, integraciju strujnih senzora, te razvoj takvih poluvodičkih komponenata koje olakšavaju integraciju i poboljšavaju karakteristike uređaja. Pri projektiranju, zbog multidisciplinarnih aspekata integriranih sustava, treba primijeniti nužne CAD alate. Razvoj integriranih modula elektroničkih učinskih pretvarača (engl. integrated power electronics modules, IPEM) ilustriran je na dvije primjene: (1) istosmjerni pretvarač snage 1 kW u asimetričnom polumosnom spoju i (2) elektromotorni pogon snage 1 . . . 3 kW za grijanje, ventilaciju i klimatizaciju (engl. heating, ventilation and air conditioning, HVAC).     The SED Machine: a robotic spectrograph for fast transient classification

    Current time domain facilities are finding several hundreds of transient astronomical events a year. The discovery rate is expected to increase in the future as soon as new surveys such as the Zwicky Transient Facility (ZTF) and the Large Synoptic Sky Survey (LSST) come on line. At the present time, the rate at which transients are classified is approximately one order or magnitude lower than the discovery rate, leading to an increasing "follow-up drought". Existing telescopes with moderate aperture can help address this deficit when equipped with spectrographs optimized for spectral classification. Here, we provide an overview of the design, operations and first results of the Spectral Energy Distribution Machine (SEDM), operating on the Palomar 60-inch telescope (P60). The instrument is optimized for classification and high observing efficiency. It combines a low-resolution (R\sim100) integral field unit (IFU) spectrograph with "Rainbow Camera" (RC), a multi-band field acquisition camera which also serves as multi-band (ugri) photometer. The SEDM was commissioned during the operation of the intermediate Palomar Transient Factory (iPTF) and has already proved lived up to its promise. The success of the SEDM demonstrates the value of spectrographs optimized to spectral classification. Introduction of similar spectrographs on existing telescopes will help alleviate the follow-up drought and thereby accelerate the rate of discoveries.Comment: 21 pages, 20 figure
