ROLE OF INTERLEUKIN-1 IN OSTEOARTHROSIS AND POSSIBILITIES OF ITS ARREST

Research Institute of Rheumatology, Russian Academy of Medical Sciences, Moscow The study of the pathogenesis of primary osteoarthrosis (OA) allows it to be considered as a chronic inflammatory disease in which proinflammatory cytokines, such as interleukin-1 (IL-1) and tumor necrosis factor-α(TNF-α), the expression of which is observed in the subchondral bone, cartilage, and synovium even in the absence of clinical signs of inflammation, are of great importance. The severity of OA and its site depend on IL-1 gene polymorphism. The mechanism of action of IL-1 in OA is to activate osteoclasts, to enhance the expression of calcium, and to modulate pain mediators, resulting in cartilaginous plate and subchondral bone destructions. The inhibition of IL-1 generation and activity requires the use of diacerein, the mechanism of whose action is to suppress the synthesis of cytokines and to reduce the number of their receptors onto the surface of chondrocytes, which lowers their sensitivity to IL-1. The drug intracellularly blocks nuclear factor NFκB activation, by diminishing the expression of the genes responsible for the production of other proinflammatory cytokines. Clinical trials have provided evidence for the symptomatic and structure-modifying activities of diacerein that has, in addition, an aftereffect, i. e. the effect is retained even after the drug is discontinued

Тяжесть ОА и его локализация зависят от полиморфизма генов ИЛ 1. Механизм действия ИЛ 1 при ОА заключается в активации остеокластов, повышении экскреции кальция, модуляции медиаторов боли, что приводит к деструкции хрящевой пластины и субхондральной кости. Для ингибиции выработки и активности ИЛ 1 используют препарат диацереин, механизм действия которого заключается в ингибировании синтеза цитокинов и уменьшении числа их рецепторов на поверхности хондроцитов, что снижает их чувствительность к действию ИЛ 1. Препарат внутриклеточно блокирует активацию ядерного фактора NFκB, снижая экспрессию генов, ответственных за продукцию других провоспалительных цитокинов. Заболеваемость населения России острой ревматической лихорадкой и хроническими ревматическими болезнями сердца (2011–2012 гг.)

The problems of acute rheumatic fever (ARF) and chronic rheumatic heart disease (CRHD) are discussed. Carditis, polyarthritis, erythema annulare, subcutaneous nodules, and chorea minor are the main clinical manifestations of ARF caused by β-hemolytic streptococcus A. Cardiac failures emerge in 60–65% of patients after the first ARF episode. Repeated ARF attacks promote CRHD. The aim of the study is to evaluate the prevalence of rheumatic heart disease in different age groups in the regions of Russian Federation in 2011–2012. Materials and Methods. Analysis of the Ministry of Healthcare of the Russian Federation reports on population morbidity in 2011–2012 (Form N12).Results. The highest ARF prevalence was observed in 15–17-year-old teenagers, while the lowest, in adults over 18-year-old. The lowest CRHD figures were registered in juniors, while the highest ones, in adults. No ARF cases in 0–14-year-old children were registered in 31 entities of the Russian Federation; single cases, in 17 entities; no ARF cases in 15–17-year-old teenagers was registered in 37 entities, and 1–2 ARF cases were registered in 23 entities. Only in 10 entities, no ARF cases were registered in adults, and single cases were registered in 22 entities. Extremely unfavorable situations were found in the Chechen Republic (140 ARF cases in 0–14-year-old children) and in the Dagestan Republic (140 cases). High morbidity among 15–17-year-old teenagers was registered in tje Kaliningrad region (83 cases) and the Chechen Republic (100). ARF cases in adults were most frequent in St. Petersburg (124 cases), the Chechen Republic (154), and the Moscow region (161).The article lists the ARF and CRHD preventive measures.Обсуждаются проблемы острой ревматической лихорадки (ОРЛ) и хронических ревматических болезней сердца (ХРБС). Основными клиническими проявлениями ОРЛ, обусловленной β-гемолитическим стрептококком группы А, являются кардит, полиартрит, кольцевидная эритема, подкожные узелки, хорея. После первого эпизода ОРЛ у 60–65% больных развиваются пороки сердца. Повторные атаки ОРЛ способствуют возникновению ХРБС.Цель исследования – оценка распространенности ревматических заболеваний сердца в различных возрастных группах в регионах Российской Федерации (РФ) за 2011–2012 гг.Материал и методы. Проанализированы отчеты Минздрава России о заболеваемости населения за 2011–2012 гг. (форма №12). Результаты исследования. Наиболее высокой распространенность ОРЛ оказалась у подростков 15–17 лет, самой низкой – у взрослых старше 18 лет. Самые низкие показатели ХРБС отмечены в младшей возрастной группе, а наиболее высокие – у взрослых. В 31 субъекте РФ не выявлено ни одного случая ОРЛ у детей 0–14 лет, в 17 субъектах наблюдались единичные случаи, в 37 субъектах у подростков 15–17 лет не отмечено случаев ОРЛ и в 23 субъектах зафиксировано по 1–2 случая ОРЛ. У взрослого населения лишь в 10 субъектах не зарегистрировано случаев ОРЛ, и в 22 субъектах они были единичными. Крайне неблагоприятная ситуация отмечается в Чеченской республике, где среди детей 0–14 лет зафиксировано 140 случаев ОРЛ, и в Дагестане – 149 случаев. Среди подростков 15–17 лет высокие показатели заболеваемости выявлены в Калининградской области (83 случая) и Чеченской республике (100). Среди взрослого населения случаи ОРЛ чаще регистрировались в Санкт-Петербурге (124 случая), Чеченской республике (154) и Московской области (161).Указаны меры профилактики ОРЛ и ХРБС

ADULT-ONSET STILL'S DISEASE: DIAGNOSIS AND TREATMENT

The paper describes adult-onset Still's disease (AOSD), a rare multisystemic disease of unknown etiology that is referred to as seronegative rheumatoid arthritis. It presents the major manifestations of AOSD: long-term fever, arthritis or persistent arthralgias, maculopapular eruption, seronegativity for rheumatoid factor, neutrophilic leukocytosis, and disease onset after 16 years of age, as well as additional signs: lymphadenopathy, hepatosplenomegaly, polyserositis, nasopharyngeal infection. It is noted that particular difficulties in the diagnosis of AOSD emerge when it is complicated by the hematophagocytic syndrome (HPS). The distinctive features of AOSD are the development of cutaneous and articular symptoms in practically 80% of patients and their absence in HPS. It is stated that of greater value in the diagnosis of HPS is examination of aspirate of the bone marrow than its biopsy.
