ROLE OF INTERLEUKIN-1 IN OSTEOARTHROSIS AND POSSIBILITIES OF ITS ARREST

The study of the pathogenesis of primary osteoarthrosis (OA) allows it to be considered as a chronic inflammatory disease in which proinflammatory
cytokines, such as interleukin-1 (IL-1) and tumor necrosis factor-Ξ±(TNF-Ξ±), the expression of which is observed in the subchondral
bone, cartilage, and synovium even in the absence of clinical signs of inflammation, are of great importance.
The severity of OA and its site depend on IL-1 gene polymorphism.
The mechanism of action of IL-1 in OA is to activate osteoclasts, to enhance the expression of calcium, and to modulate pain mediators, resulting
in cartilaginous plate and subchondral bone destructions.
The inhibition of IL-1 generation and activity requires the use of diacerein, the mechanism of whose action is to suppress the synthesis of cytokines
and to reduce the number of their receptors onto the surface of chondrocytes, which lowers their sensitivity to IL-1. The drug intracellularly blocks
nuclear factor NFΞΊB activation, by diminishing the expression of the genes responsible for the production of other proinflammatory cytokines.
Clinical trials have provided evidence for the symptomatic and structure-modifying activities of diacerein that has, in addition, an aftereffect, i. e. the effect is retained even after the drug is discontinued

The problems of acute rheumatic fever (ARF) and chronic rheumatic heart disease (CRHD) are discussed. Carditis, polyarthritis, erythemaΒ annulare, subcutaneous nodules, and chorea minor are the main clinical manifestations of ARF caused by Ξ²-hemolytic streptococcus A.Β Cardiac failures emerge in 60β65% of patients after the first ARF episode. Repeated ARF attacks promote CRHD. The aim of the study is to evaluate the prevalence of rheumatic heart disease in different age groups in the regions of Russian FederationΒ in 2011β2012. Materials and Methods. Analysis of the Ministry of Healthcare of the Russian Federation reports on population morbidity inΒ 2011β2012 (Form N12).Results. The highest ARF prevalence was observed in 15β17-year-old teenagers, while the lowest, in adults over 18-year-old. TheΒ lowest CRHD figures were registered in juniors, while the highest ones, in adults. No ARF cases in 0β14-year-old children were registered in 31 entities of the Russian Federation; single cases, in 17 entities; no ARF cases in 15β17-year-old teenagers was registered inΒ 37 entities, and 1β2 ARF cases were registered in 23 entities. Only in 10 entities, no ARF cases were registered in adults, and singleΒ cases were registered in 22 entities. Extremely unfavorable situations were found in the Chechen Republic (140 ARF cases in 0β14-year-old children) and in the Dagestan Republic (140 cases). High morbidity among 15β17-year-old teenagers was registeredΒ in tje Kaliningrad region (83 cases) and the Chechen Republic (100). ADULT-ONSET STILL'S DISEASE: DIAGNOSIS AND TREATMENT
The paper describes adult-onset Still's disease (AOSD), a rare multisystemic disease of unknown etiology that is referred to as seronegative rheumatoid arthritis. It presents the major manifestations of AOSD: long-term fever, arthritis or persistent arthralgias, maculopapular eruption, seronegativity for rheumatoid factor, neutrophilic leukocytosis, and disease onset after 16 years of age, as well as additional signs: lymphadenopathy, hepatosplenomegaly, polyserositis, nasopharyngeal infection. It is noted that particular difficulties in the diagnosis of AOSD emerge when it is complicated by the hematophagocytic syndrome (HPS). The distinctive features of AOSD are the development of cutaneous and articular symptoms in practically 80% of patients and their absence in HPS. It is stated that of greater value in the diagnosis of HPS is examination of aspirate of the bone marrow than its biopsy.
