    Leigh syndrome is a mitochondrial encephalomyopathy that occurs due to "cytochrome c oxidase deficiency". Few psychiatric disorders have been defined that are associated with Leigh syndrome. The objective of this work is to study relations between mitochondrial dysfunction and psychiatric disorders. It was a 20 year old male patient, who received Modopar, for severe extra pyramidal symptoms caused by Leigh syndrome. He developed, four months ago, acute psychotic symptoms such as audio-visual hallucinations, persecution and mystic delirium. The cerebral MRI has shown signal abnormalities in central grey nucleus. The EEG recording and blood test were normal. The hypothesis of drug induced psychiatric disorders (Modopar) was possible. The evolution under atypical antipsychotic was only partial. In this case, the cerebrospinal fluid and lactate levels mean that mitochondria were not an overall explanation for these psychiatric disorders but may at least play a partial role.     Inverted follicular keratosis: A retrospective study over 10 years

    Inverted follicular keratosis (IFK) is a rare benign tumour of the follicular infundibulum characterized by exo-endophytic growing. Generally, the diagnosis of IFK is histopathologically made because clinical differentiation from other lesions is difficult. We present a retrospective series with thirteen patients with histologically confirmed IFK to evaluate the epidemiological, clinical and histopathologic characteristics of IFK. The mean age of the patients at the time of the excision was 53 years with extremes ranging from 19 to 82 years. The sex ratio M/F was 3.3. The lesions affected the face in nine patients mainly the moustache, followed by the scalp in three cases and the arm in one case, and 92% of the localizations are sun-exposed. The diagnosis was never clinically evoked. The lesion had a pink colour in ten cases and was pigmented in two cases and hypopigmented in one case. More than half of the lesions (53%) had a keratotic centre. Histopathological examination showed endophytic intradermal proliferation of basaloid cells with a variable degree of squamoid differentiation. Horn cysts were present in all cases. According to our series, the IFK occurs predominantly in young men, in the face and more specifically in the moustache. Dermoscopy may suggest the diagnosis of the IFK. In fact, a histopathological examination is the gold standard for the diagnosis of the IFK and helps differentiate these benign tumours from possible malignant neoplasms

    De novo anaplastic Kaposi sarcoma in a HIV-negative man: A case report and review of the literature

    Anaplastic Kaposi sarcoma is a rare variant of Kaposi’s and is typically associated with an agressive clinical course. We report a case of a 67-year-old HIV negative man, presented with multiple, pink nodules on the left ankle and a keratotic lesion of the right heel. Initial histopathological exam concluded to an undifferentiated sarcoma. A second biopsy was performed and concluded to an anaplastic Kaposi sarcoma. Immunohistochemical study was positive for HHV8. Treatment consisted on a tumor excision of all lesions. Our case and the review of the literature highlight the benefit of the non conservative surgical treatment for this aggressive form of Kaposi sarcoma

    The relationship between alopecia areata and alexithymia, anxiety and depression: A case-control study

    Background: Alopecia areata (AA) is a skin disease characterized by the sudden appearance of areas of hair loss on the scalp and other hair-bearing areas, but its aesthetic repercussions can lead to profound changes in patient′s psychological status and relationships. Aim: The goal was to investigate a possible relationship between AA and alexithymia as well as two other emotional dimensions, anxiety and depression. Materials and Methods: Fifty patients with AA seen in the Department of Dermatology of Hedi Chaker University Hospital, Sfax were included in this study. Anxiety and depression were evaluated by Hospital Anxiety and Depression scale questionnaire, alexithymia was assessed by Toronto Alexithymia scale 20, and severity of AA was measured by Severity of Alopecia Tool. Results: Patient′s mean age was 32.92 years. 52% of patients were females. Depression and anxiety were detected respectively in 38% and 62% of patients. There was statistically significant difference between patients and control group in terms of depression (P = 0.047) and anxiety (P = 0.005). Forty-two percent of patients scored positive for alexithymia. No significant difference was found between patient and control groups (P = 0.683) in terms of alexithymia. Anxiety was responsible for 14.7% of variation in alexithymia (P = 0.047). Conclusions: Our study shows a high prevalence of anxiety and depressive symptoms in AA patients. Dermatologists should be aware of the psychological impact of AA, especially as current treatments have limited effectiveness

    Quality of Life in Alopecia Areata: A Sample of Tunisian Patients

    Background. Alopecia areata (AA) has a significant impact on the quality of life and social interaction of those suffering from it. Our aim was to assess the impact of AA on the quality of life. Methods. Fifty patients diagnosed with AA seen in the Department of Dermatology of Hedi Chaker University Hospital, between March 2010 and July 2010, were included. Quality of life was measured by SF 36; severity of AA was measured by SALT. Results. Eighty percent had patchy alopecia with less than 50% involvement, 12% had patchy alopecia with 50–99% involvement, and 8% had alopecia totalis. Compared with the general population, AA patients presented a significantly altered quality of life, found in the global score and in five subscores of the SF-36: mental health, role emotional, social functioning, vitality, and general health. Gender, age, marital status, and severity of alopecia areata had a significant influence on patients’ quality of life. Conclusions. This study indicates that patients with AA experience a poor quality of life, which impacts their overall health. We suggest screening for psychiatric distress. Studies of interventions such as counseling, psychoeducation, and psychotherapeutic interventions to reduce the impact of the disease may be warranted

    Grossesse et hypophysite: une nouvelle observation

    Introduction: L’hypophysite est une lésion hypophysaire rare qui pose des difficultés diagnostiques et thérapeutiques. Ses manifestations cliniques et radiologiques sont difficiles à distinguer des tumeurs hypophysaires.Observation: Nous rapportons, l’observation d’une femme âgée de 40 ans qui s’est présenté pour des céphalées, troubles visuels et vomissements au cours de son troisième trimestre de grossesse. L’imagerie par résonnance magnétique a montré une masse sellaire et supra sellaire de 21 mm et qui adhérait au chiasma optique. L’examen anatomopathologique réalisé après une chirurgie d’exérèse trans-sphénoïdale a montré un tissu fibreux associé à des lymphocytes et à des plasmocytes. Une nette amélioration clinique était obtenue après le traitement chirurgical, une corticothérapie et un traitement hormonal thyroïdien substitutif.Conclusion: L’hypophysite est une maladie rare et doit être considérée dans le diagnostic différentiel des désordres hypophysaires durant la grossesse.Mots clés: pseudotumeur inflammatoire, hypophysite, grossesse, IgG4English Title: Pregnancy and hypohpysitis: a case reportEnglish AbstractIntroducion: Hypophysitis is an uncommon sellar condition in which diagnosis and management pose a significant problem, as its clinical manifestation and radiological features are difficult to distinguish from that of pituitary tumors.Case report: We report, in this study, a case of 40 year-old woman presenting with headaches, vision disturbances and vomiting, during her third trimester. Magnetic resonance imaging (MRI) showed a 21 mm mass at the sellar and supra sellar regions, this mass adhered to the optic chiasm. Pathologic examination, after transsphenoïdal surgery revealed fibrous stroma with dense infiltrates of small lymphocytes and plasma cells. Resolution of the symptoms occured after surgery and replacement treatment by steroïds and physiological thyroxine.Conclusion: Hypophysitis is a rare diasease that should be considered in the differential diagnosis of pituitary disorders during pregnancy.Keywords: inflammatory pseudotumor ; hypophysitis ; pregnancy ; IgG
