    Patiënten met chronische invaliderende aandoeningen krijgen, onder meer door het beslag dat zij leggen op de gezondheidszorg en op de maatschappij, langzamerhand meer aandacht. Er wordt steeds vaker gesproken over de ‘kwaliteit van het leven’. Hier is alle reden toe, daar de politieke belangstelling voor de gezondheidszorg de afgelopen decennia vooral is gegaan naar aandoeningen die voor iedereen zichtbaar de ‘kwantiteit’ van het leven bedreigen: hart- en vaatziekten en kanker. Het is dan ook noodzakelijk en een groot goed dat ook patiënten met aandoeningen waarbij de kwaliteit van het leven in het geding is, aandacht en zorg krijgen.\ud \ud Veel patiëntjaren zijn het gevolg van chronische invaliderende ziekten en een zeer groot gedeelte van de gezondheidszorg is ermee gemoeid. Verbetering van de kwaliteit van leven is voor de patiënten en hun familie van het grootste belang en is ook meetbaar

    Sexual functioning of people with rheumatoid arthritis:a multicenter study

    The objective of this study is to compare men and women with rheumatoid arthritis (RA) to controls regarding sexual motivation, activity, satisfaction, and specific sexual problems, and to determine the correlation of physical aspects of the disease with sexual functioning. Questionnaire for screening sexual dysfunctions (QSD), self-constructed questionnaire on experienced distress with joints during sexual activities, arthritis impact measurements scales 2 (AIMS2), and the modified disease activity score 28 (DAS 28) were the methods used. RA patients were recruited from a registration base in three Dutch hospitals. Controls were age and sex matched healthy volunteers. A completed questionnaire was sent back by 271 patients (response 23%). Forty-seven men and 93 women were clinically examined to obtain the DAS 28. Male patients felt less sexual desire, and female patients masturbated and fantasized less than controls. Differences in satisfaction were not found. Male and female patients did not experience more sexual problems than controls. Among the women, correlations were predominantly found between age and sexual motivation and activities, among the men between physical health and sexual problems. Up to 41% of the men (4-41 depending on the joints), and up to 51% of the women (10-51 depending on the joints) have troubles with several joints during sexual activities. Medications influencing ejaculation in men correlated with distress with orgasm. Conclusions are that patients are less sexually active than controls and a considerable number of both male and female patients have trouble with their joints during sexual activities. However, patients do not differ from controls regarding sexual satisfaction. Physiological changes due to RA are apparently independent from those on psychological level. It is argued that sexual satisfaction also depends on personal and social factors. In men, physical health and disease activity are more related with sexual problems than in women.</p

    Serum levels of matrix metalloproteinase-3 in relation to the development of radiological damage in patients with early rheumatoid arthritis

    Objective. To evaluate the significance of serum matrix metalloproteinase-3 (MMP-3) levels in relation to the development of radiological damage (X-ray damage) in early rheumatoid arthritis (RA).Methods. Serum MMP-3 levels were measured in 46 healthy controls (CTRL), 19 osteoarthritis (OA) and 78 RA patients with joint symptoms forResults. MMP-3 levels in CTRL and OA were low or undetectable with no differences between the groups (P = 0.19). Levels in RA were higher than in CTRL (P &lt;0.01). Initial MMP-3 levels in patients with X-ray damage at T0 (n = 30) were higher than the levels in patients without any X-ray damage during follow-up (n = 19) (P &lt;0.01), but were not different from those in patients who developed X-ray damage during the study (n = 29) (P = 0.11). In the patients without X-ray damage at T0, there was a significant correlation between MMP-3 at TO and the total X-ray damage after 6 months (r = 0.34, P = 0.02) and 12 months (r = 0.32, P = 0.03). This correlation was almost exclusively determined by joint space narrowing in the Sharp score.Conclusion. The serum MMP-3 level seems to be an indicator for the development of radiological damage in patients with early RA and appears to be particularly indicative of cartilage degradation.</p
