Újdonságok a prosztatarák diagnosztikájában és kezelésében
Similarly to earlier years, a vast majority of novel findings were published on prostate cancer, which is the most common urological cancer. Clinical trials with long-term follow-up and promising observational studies were published. A hĂmvesszĹ‘rák egy ritka kĂłrelváltozás. Ellátásában az elsĹ‘dleges tumor Ă©s a nyirokcsomóáttĂ©t kezelĂ©se jelentĹ‘s. Napjainkban egyre nagyobb igĂ©ny van kevĂ©sbĂ© invazĂv, de kuratĂv kezelĂ©sre.
Célkitűzés és módszer:
A szerzĹ‘k áttekintik az irodalmi adatok alapján a hĂmvesszĹ‘rák kezelĂ©sĂ©nek legĂşjabb lehetĹ‘sĂ©geit. Az elsĹ‘dleges daganat ellátásában komoly kihĂvás Ă©s egyben cĂ©l egy kozmetikailag elfogadhatĂł kinĂ©zetű hĂmvesszĹ‘ hátrahagyása. Az inguinalis lymphadenectomia morbiditása miatt bemutatják a kevĂ©sbĂ© invazĂv ellátás lehetĹ‘sĂ©gĂ©t.
Eredmények:
A sebĂ©szi kezelĂ©s fejlĹ‘dĂ©se Ă©s a korábbi szigorĂş irányelvek felĂĽlĂ©rtĂ©kelĂ©se lehetĹ‘vĂ© tette hĂmvesszĹ‘rák esetĂ©ben is a szervmegtartĂł beavatkozások bevezetĂ©sĂ©t. A közĂ©ptávĂş eredmĂ©nyek a hĂmvesszĹ‘ korábbi csonkolásával nyert onkolĂłgiai eredmĂ©nyekkel megegyeznek. A Ĺ‘rszemnyirokcsomĂłk kimutatásában Ă©s eltávolĂtásában nyert legĂşjabb ismereteink segĂtenek csökkenteni a beavatkozás morbiditását Ă©s Ăgy a betegek gyorsabb felĂ©pĂĽlĂ©sĂ©t.
Következtetések:
A hĂmvesszĹ‘megĹ‘rzĹ‘ technika kiterjesztĂ©sĂ©vel biztonságos onkolĂłgiai eredmĂ©ny mellett lehetĹ‘vĂ© válik a csonkolás elkerĂĽlĂ©se. A legĂşjabb kĂ©palkotĂł vizsgálatoknak köszönhetĹ‘en a lágyĂ©ki nyirokcsomĂłk állapotának pontosabb felmĂ©rĂ©se lehetĹ‘vĂ© vált. A nyirokcsomĂłk eltávolĂtásának technikai fejlĹ‘dĂ©sĂ©vel a beavatkozás morbiditása jelentĹ‘sen csökkenthetĹ‘.

Penile cancer is a rare anomaly. Primary tumor and lymphnodes metastasis managements are essential. Nowadays, there is a growing wish for less invasive but curative treatment.
Aims and methods:
Newest opportunities of the management of penile cancer are reviewed according to the recent literature by the authors. It is a serious challenge and also aims to preserve a cosmetically acceptable looking penis. Because of the morbidity of the inguinal lymphadenectomy, the possibilities of less invasive procedures are shown.
Results:
Development of surgical management and re-evaluation of previous serious guidelines made possible to introduce the organ preserving surgery. Midterm oncological results are similar either performing penectomy or organ preserving surgery. Most recent knowledge in the diagnosis and management of sentinel lymph nodes helps to decrease the morbidity of the procedure and assists in faster recovery of patients.
Conclusion:
Extension of penis preserving technique makes possible to avoid amputation with safe oncological result. More accurate evaluation of inguinal lymph-node status is possible with the latest diagnostic examinations. Morbidity of removal of lymph-nodes can be significantly decreased with development of the procedure

A prosztatarák az egyik leggyakoribb, ennĂ©lfogva az egyik legjelentĹ‘sebb daganatos megbetegedĂ©s a fĂ©rfiak körĂ©ben. Ezen összefoglalĂłban az epidemiolĂłgiai Ă©s az etiolĂłgiai adatok mellett rĂ©szletesen kitĂ©rĂĽnk a szűrĂ©sben leginkább használt prosztataspecifikus antigĂ©n legfontosabb ismĂ©rveire Ă©s azokra a további markerekre, amelyek segĂthetnek diagnosztizálni a betegsĂ©get. Bemutatjuk azokat a fajsĂşlyos nemzetközi Ă©s magyar tanulmányokat, amelyek a prosztatarák szűrĂ©sĂ©vel foglalkoznak. = Prostate cancer is one of the most common and therefore one of the most significant cancer among men. A metasztatikus kasztrációrezisztens prosztatarák kezelése = Treatment of metastatic prostate cancer
Az áttétes prosztatarák kezelése az utóbbi években jelentős átalakuláson ment keresztül. Abban nem történt változás, hogy a gyógyszeres kezelés alapja a tesztoszteron hormon megvonása. Ennek a folyamatnak csaknem törvényszerű velejárója a később bekövetkező csökkent eredményesség, a kasztrációrezisztencia. Az ekkor bekövetkező daganatos progresszió kezelésére több új vegyületet vezettek be különböző szekvenciákkal. Ezek áttekintése mellett foglalkozunk még az ebben a stádiumban legtöbbször megfigyelhető csontáttétek korszerű gyógyszeres kezelési lehetőségeivel. = The treatment of metastatic prostate cancer has changed significantly in recent years, but the basis is still the withdrawal of the testosterone. An almost inherent part of this process is the castration resistance. Several new treatment modalities with different sequences have been introduced to treat the cancer progression that occurred at this time. A húgyhólyag urothelialis daganatainak molekuláris alcsoportbeosztása és annak klinikai vonatkozásai = Molecular subtype classification of urothelial bladder cancer and its clinical relevance

A molekuláris biolĂłgia Ă©s a bioinformatika fejlĹ‘dĂ©se által lehetĹ‘sĂ©g nyĂlt a
leggyakrabban elĹ‘fordulĂł daganatok – köztĂĽk az izominvazĂv hĂşgyhĂłlyagrákok –
molekuláris hátterének teljes feltérképezésére. Ennek következtében az elmúlt
évek során számos, a tumorok génexpressziós és fehérjemintázata alapján történő
alcsoportbeosztás látott napvilágot az urothelialis húgyhólyagrákok molekuláris
alapon törtĂ©nĹ‘ elkĂĽlönĂtĂ©sĂ©re. A közelmĂşltban megszĂĽletett egy szĂ©les körű
konszenzuson alapulĂł klasszifikáciĂł, amely hat molekuláris alcsoportot kĂĽlönĂt
el egymástól, és a jövőben jelentősen befolyásolhatja a terápiás döntéshozatalt
is. Ezzel párhuzamosan az izominvazĂv hĂşgyhĂłlyagrák gyĂłgyszeres terápiája is
sokrĂ©tűbbĂ© vált, hiszen a továbbra is az elsĹ‘ vonalban alkalmazott perioperatĂv
ciszplatinalapú kemoterápián túl a közelmúltban elérhetővé váltak új,
checkpointgátlĂł kĂ©szĂtmĂ©nyek a kemoterápiára rezisztens, illetve alkalmatlan
betegek számára. Továbbá ĂgĂ©retes kezelĂ©si lehetĹ‘sĂ©get jelentenek az
FGFR-támadáspontú szerek is. A terápiás fegyvertár bővülésével a molekuláris
alcsoportok elkĂĽlönĂtĂ©se lĂ©nyeges szerepet kaphat a legoptimálisabb kezelĂ©s
kiválasztásában. Jelen munkánkban összefoglalást nyĂşjtunk az izominvazĂv

Abstract:
Current advances in molecular techniques and bioinformatics allowed the analysis
of complex molecular patterns in various cancers including muscle-invasive
bladder cancer. As a consequence, in the last few years numerous gene- and
protein expression-based molecular classifications have been recommended.
Recently a comprehensive consensus classification for muscle-invasive urothelial
bladder cancer has been published, distinguishing 6 subgroups with a potential
impact on clinical decision-making. At the same time, the therapeutic landscape
of muscle-invasive bladder cancer becomes increasingly differentiated as novel
checkpoint inhibitors have been available for cisplatin-ineligible and/or
resistant patients. Furthermore, promising results have been obtained with FGFR
targeting agents. Therefore, molecular subtyping will probably have a crucial
role in individualized therapeutic decision-making in bladder cancer. In the
present work, we summarize the evolution, recent advances and potential
therapeutic relevance of molecular subclassifications in bladder cancer. Orv

A prosztatarák hormonterápiája (elsődleges kezelés) = Hormonal treatment of prostate cancer (primary hormonal therapy)
Az elĹ‘rehaladott Ă©s metasztatikus prosztatarák kezelĂ©sĂ©nek alapja az androgĂ©ndepriváciĂłs terápia, amely során a here androgĂ©n szekrĂ©ciĂłjának kĂĽlönbözĹ‘ mĂłdon törtĂ©nĹ‘ elnyomását vĂ©gezzĂĽk. Leggyakrabban használt formája a kĂ©miai kasztráciĂł LHRH analĂłg/antagonista kĂ©szĂtmĂ©nyekkel. Lokálisan elĹ‘rehaladott prosztatarák esetĂ©n a hormonterápia (ADT) kombináciĂłja javasolt helyi kezelĂ©ssel (radioterápia vagy radikális prostatectomia) a tĂşlĂ©lĂ©s javĂtása Ă©rdekĂ©ben. Radikális műtĂ©t után igazolt nyirokcsomĂł-pozitivitás esetĂ©n a korai adjuváns hormonterápia javĂtja a daganatspecifikus tĂşlĂ©lĂ©st, amelynek kiegĂ©szĂtĂ©se a prosztataágy sugárkezelĂ©sĂ©vel további elĹ‘nyt jelenthet. Metasztatikus prosztatarák esetĂ©n is a kombináciĂłs kezelĂ©sek jelentik a standard elsĹ‘vonalbeli terápiát, elsĹ‘sorban ADT Ă©s ARPI (abirateron, apalutamid, enzalutamid) egyĂĽttes adásával. = The basis of the treatment of advanced and metastatic prostate cancer is androgen deprivation therapy, where the androgen secretion of testis is suppressed in different ways. The most frequently used form is chemical castration with LHRH agonist/ antagonist injections. For locally advanced prostate cancer a combination of hormone therapy (ADT) with local treatment (radiotherapy or radical prostatectomy) is recommended to improve survival. In case of confirmed lymph node positivity after radical surgery early adjuvant hormone therapy improves tumour-specific survival, which can be combined with irradiation of the prostate bed as an additional benefit. Even in metastatic prostate cancer combination treatments are the standard first-line therapies, primarily with the combined administration of ADT and ARPI (abiraterone, apalutamide, enzalutamide)

AbstractIn the past half century the IC technology could produce an unprecedented growth and penetration into all segments of our daily life due to the achieved extreme productivity and reliability by using well established and continuously refined processing platforms. This uninterrupted trend continued also with the advent of the More than Moore-type MEMS technology allowing the combination of the signal processing capability of the mature IC technology with the exploitation of mechanical, thermal, optical properties of the materials used in IC technology in different sensing and actuation purposes. Mass producibility by monolithic integration required here also the development of appropriate unified set of techniques for the various applications, i.e. sort of standardised platforms to explore.In this paper we focus on the development of bulk micromachined membrane platforms, being exploited in such applications. Jóindulatú daganat-e a húgyhólyag invertált papillomája? = Can inverted papilloma in urinary bladder be considered as a benign tumor?
Invertált papilloma elĹ‘fordulása a hĂşgyhĂłlyagban igen ritka. Irodalmi adatok szerint a betegsĂ©g nem malignus, kiĂşjulási hajlama alacsony. A szerzĹ‘k a Semmelweis Egyetem UrolĂłgiai Klinikáján Ă©s UroonkolĂłgiai Centrumában az elmĂşlt 11 Ă©vben felfedezett, invertált papillomás eseteket vizsgálták. Választ kĂvántak kapni arra, hogy az invertált papillomák milyen gyakorisággal Ăşjulnak ki, Ă©s milyen gyakran alakul ki belĹ‘lĂĽk rosszindulatĂş hĂłlyagdaganat.
Betegek Ă©s mĂłdszer:
Harminc beteget prospektĂv mĂłdszerrel követtek, akiknĂ©l a hĂşgyhĂłlyagdaganat transurethralis reszekciĂłjának szövettani eredmĂ©nye invertált papilloma volt. A betegek szoros követĂ©sĂ©t a háromhavonta esedĂ©kes vizeletvizsgálat, hasi ultrahangvizsgálat Ă©s hĂłlyagtĂĽkrözĂ©s alkotta. Egy Ă©v után a fenti vizsgálatokat fĂ©lĂ©vente vĂ©geztĂ©k el.
Eredmények:
Három betegĂĽkben alakult ki a követĂ©si idĹ‘ alatt transitiocellularis carcinoma (17, 60 Ă©s 92 hĂłnap mĂşlva). Egy betegnĂ©l szinkrĂłn kerĂĽlt felismerĂ©sre invertált papilloma Ă©s pTa G1 átmeneti sejtes daganat. Egy betegben 15 hĂłnappal a nem izominvazĂv hĂłlyagrák (pT1 G2) miatt vĂ©gzett műtĂ©t Ă©s lokális kemoterápia után találtak invertált papillomát a kontroll-cisztoszkĂłpia során.
Következtetések:
A szerzĹ‘k vizsgálatai alapján az invertált papilloma jĂłindulatĂş betegsĂ©g, de követĂ©se javasolt, mert elĹ‘fordulhat malignizálĂłdása, vagy kĂsĂ©rheti átmeneti sejtes hĂłlyagrák. EredmĂ©nyeik alapján, bár ebben az irodalom nem foglal teljesen egyformán állást, az invertált papillomával kezelt betegek követĂ©sĂ©t a primer pTa G1 hĂłlyagrákoknak megfelelĹ‘en ajánlják.

Inverted papilloma of the urinary bladder is a rare entity. According to literature data, this disease is not malignant, and has low recurrence rate. Authors studied cases detected at the Urology Department and Urooncological Centrum at Semmelweis University in the last 11 years. They aimed to find out the rate of inverted papilloma recurrences, and transformations into malignant bladder cancer.
Materials and methods:
Thirty patients with histologically proven inverted papilloma were followed after transurethral resection of bladder, which meant urine tests every three months, abdominal ultrasound and cystoscopy. After a year, these examinations were done in every six months.
Results:
Three patients presented transitiocellular carcinoma (17, 60, 92 months later) during this period. In one case, inverted papilloma and transitiocellular tumor (pTa G1) were detected. In one patient, inverted papilloma was found by control cystoscopy after transurethral resection of bladder (pT1 G2) and local chemotherapy 15 months later.
Conclusions:
Based on authors’ experience, inverted papilloma of the urinary bladder is a benign lesion, but malignant changes or concomitant transitiocellular tumor may occur, thus follow-up is needed. Although references are not standardized, authors suggest following patients with inverted papilloma as a primary (pTa G1) bladder cancer
