Bloodstream Infections in Neutropenic Patients â Early Detection of Pathogens and Directed Antimicrobial Therapy due to Surveillance Blood Cultures
Das Absinken der Anzahl der neutrophilen Granulozyten (Neutropenie) nach der
Chemotherapie ist ein bekanntes PhÀnomen, welches eine entscheidende Rolle auf
die InfektanfĂ€lligkeit der Patienten ausĂŒbt. Infektionen sind bei
hÀmatologisch-onkologischen Patienten nach Chemotherapie hÀufig auftretende
Komplikationen, die eine schnelle Einleitung der Antibiotikatherapie
erfordern. In der vorliegenden Arbeit wird beschrieben, dass durch die
prophylaktische Blutkulturentnahme bei neutropenischen hÀmatologisch-
onkologischen Patienten, ein Erregernachweis vor klinischer Manifestation
einer Infektionskrankheit möglich und sinnvoll ist. Hierbei wurden regelmĂ€Ăig
Blutkulturen von 100 Patienten untersucht, bei denen dreimal wöchentlich aus
zentralen Venenkathetern Blut entnommen wurde. In den Kulturen von 42
Patienten mit Fieber konnten positive Befunde erhoben werden, wobei in 18
FĂ€llen die Erreger bereits vor Einsetzen von Fieber nachgewiesen werden
konnten, was zu einer frĂŒheren antibiogrammgerechten Behandlung fĂŒhrte. Durch
die Kenntnis des Erregers konnte bei diesen Patienten bei Fieberanstieg eine
gerichtete Antibiotikatherapie durchgefĂŒhrt werden, was in einer verkĂŒrzten
Fieberdauer resultierte. Dies zeigte in der statistischen Auswertung (Mann-
Whitney U Test, SPSS) einen signifikanten Unterschied (p=0.004) in der
Fieberdauer. Ferner wurde anhand der untersuchten Blutkulturen gezeigt, dass
die am hÀufigsten auftretenden Keime koagulase negative Staphylokokken sind,
die durch die Standardtherapie gemÀà den aktuellen Leitlinien der
Fachgesellschaften nicht erfasst werden. Eine routinemĂ€Ăige prophylaktische
Anlage von Blutkulturen wÀhrend der Neutropenie bei Patienten nach intensiven
Chemotherapien erscheint sinnvoll, da es durch Infektionen bei hÀmatologisch-
onkologischen Patienten zu schwer beherrschbaren Komplikationen mit Todesfolge
kommen kann. Ob Blutkulturscreening die Prognose von Patienten verbessern
kann, sollte in einer randomisierten Studie geprĂŒft werden.The loss of neutrophile granulocytes following chemotherapy (âneutropeniaâ)
increases the risk of infectious complications in patients with hematological
malignancies. In this study the probability for detection of bacterial
bloodstream infections prior to clinical manifestation in prophylactic blood
cultures obtained from patients with hematological malignancies and long term
neutropenia was determined. For that purpose blood cultures of 100 patients
(which were drawn from the central venous line) were analyzed three times a
week during neutropenia. In 42 patients with fever blood stream infections
were detected. In 18 out of those the microorganisms were identifies prior to
the onset of fever, which opened the possibility of specific antimicrobial
therapy. This specific antimicrobial therapy decreased the time of
defeverescence significantly (Mann-Whitney U Test, SPSS; p=0.004). The most
abundant microorganisms were identified as coagulase-positive staphylococci,
which would normally not be targeted by standardized treatments of infectious
complications. Therefore, routinely performed analysis of prophylactic blood
cultures could minimize the risk of severe complications in patients with
hematological malignancies during neutropenia
