    Expectativas da equipe da unidade de terapia intensiva pediátrica quanto à atuação do farmacêutico clínico

    Objetivo: O objetivo do estudo foi conhecer as expectativas da equipe da unidade de terapia intensiva pediátrica quanto às atividades a serem realizadas pelo farmacêutico clínico a fim de planejar seu trabalho junto com a equipe multidisciplinar. Desenho do estudo: Estudo transversal, envolvendo a equipe multidisciplinar da unidade de terapia intensiva pediátrica de um hospital universitário terciário. Método: Um questionário autopreenchível contendo perguntas sobre as expectativas da equipe da unidade de terapia intensiva pediátrica em relação ao papel do farmacêutico clínico na equipe multidisciplinar foi aplicado aos membros da equipe, utilizando a escala de resposta do tipo Likert. Resultados: Os auxiliares/ técnicos de enfermagem e fisioterapeutas mostraram maior expectativa em relação a questões relacionadas com a administração de medicamentos; para enfermeiros, a expectativa sobre o desempenho do farmacêutico clínico focou-se em questões relacionadas à prescrição de medicações, como a avaliação da antibioticoterapia, enquanto os médicos consideraram mais importantes informações sobre a terapia medicamentosa, como a notificação de alergias relacionadas às drogas, informações sobre o aprazamento de medicamentos e avaliação da administração de drogas por sonda gástrica ou enteral. Conclusões: As expectativas da equipe variaram de acordo com as necessidades e a rotina de cada categoria profissional. Os resultados deste estudo têm contribuído para a implantação do serviço de farmácia clínica em uma unidade de terapia intensiva pediátrica brasileiraObjective: We aimed to know the expectations of a pediatric intensive care unit staff for the activities to be undertaken by the clinical pharmacist, in order to plan her work along with the multidisciplinary team. Study design: A cross-sectional study, involving the multidisciplinary team of the pediatric intensive care unit of a tertiary-care university hospital. Method: A self-completion questionnaire containing questions regarding expectations of the pediatric intensive care unit staff for the role of the clinical pharmacist in the multidisciplinary team was applied to the team members, using a Likert-type scale response. Results: The nursing assistants/technicians and physiotherapists showed higher expectation for issues related to administration of medications; for nurses, the expectation for the clinical pharmacist performance focused on issues related to prescription drugs, such as evaluation of antibiotic therapy, while physicians considered most important information on drug therapy, as notification of allergies related to drugs, information on period of validity of medicines and evaluation of drug administration by gastric or enteral tubes. Conclusions: Team expectations varied according to the needs and routine of each professional category.     O objetivo deste estudo foi propor uma lista de indicadores como forma de avaliação de desempenho para o Centro de Engenharia Clínica e Bioequipamentos do HCFMRP-USP. Indicadores são importantes ferramentas de controle e de qualidade e fornecem bases para melhorar o desempenho do estabelecimento de saúde. Para a coleta de dados, foi utilizada a literatura, referente aos indicadores para a área de Engenharia Clínica. Equipe multidisciplinar composta pelos autores do artigo foi formada para discutir e escolher os indicadores que seriam propostos como medidas de avaliação de desempenho. Foi desenvolvido um estudo na bibliografia disponível sobre o tema e também entrevistas com os gestores-chaves que estão responsáveis pela área. Adotou-se o BSC - Balanced Scorecard, em suas perspectivas financeira, de clientes, de processos internos e de aprendizado e crescimento como mapa estruturante dos indicadores. Foram propostos dezesseis indicadores nas quatro perspectivas do BSC.The aim of this study was to propose a list of indicators as a mean of performance evaluation for the Clinical Engineering Center of the Hospital of Pharmaceutical Sciences and Medicine of Ribeirão Preto (HCFMRP – USP). Indicators are important tools of quality control and supply bases to enhance the performance of the health facility. In order to collect data, it was used the literature regarding the indicators for the area of Clinical Engineering. Multidisciplinary team was formed to discuss and choose the indicators that were proposed as measures of performance evaluation. A study in the literature available about the topic and also interviews with the key managers who are responsible for the area were developed.     Lack of effects of typical and atypical antipsychotics in DARPP-32 and NCS-1 levels in PC12 cells overexpressing NCS-1

    <p>Abstract</p> <p>Background</p> <p>Schizophrenia is the major psychiatry disorder, which the exact cause remains unknown. However, it is well known that dopamine-mediated neurotransmission imbalance is associated with this pathology and the main target of antipsychotics is the dopamine receptor D<sub>2</sub>. Recently, it was described alteration in levels of two dopamine signaling related proteins in schizophrenic prefrontal cortex (PFC): Neuronal Calcium Sensor-1 (NCS-1) and DARPP-32. NCS-1, which is upregulated in PFC of schizophrenics, inhibits D<sub>2 </sub>internalization. DARPP-32, which is decreased in PFC of schizophrenics, is a key downstream effector in transducing dopamine signaling. We previously demonstrated that antipsychotics do not change levels of both proteins in rat's brain. However, since NCS-1 and DARPP-32 levels are not altered in wild type rats, we treated wild type PC12 cells (PC12 WT) and PC12 cells stably overexpressing NCS-1 (PC12 Clone) with antipsychotics to investigate if NCS-1 upregulation modulates DARPP-32 expression in response to antipsychotics treatment.</p> <p>Results</p> <p>We chronically treated both PC12 WT and PC12 Clone cells with typical (Haloperidol) or atypical (Clozapine and Risperidone) antipsychotics for 14 days. Using western blot technique we observed that there is no change in NCS-1 and DARPP-32 protein levels in both PC12 WT and PC12 Clone cells after typical and atypical antipsychotic treatments.</p> <p>Conclusions</p> <p>Because we observed no alteration in NCS-1 and DARPP-32 levels in both PC12 WT and Clone cells treated with typical or atypical antipsychotics, we suggest that the alteration in levels of both proteins in schizophrenic's PFC is related to psychopathology but not with antipsychotic treatment.</p
