    Meaningful Choices? Understanding and Participation in Direct Democracy in the American States

    What role does political knowledge play in campaigning for and participation in direct democracy? A foundational principle of democracy is citizen participation in decision-making. This foundation assumes that citizens are at least somewhat knowledgeable about government and able to make informed choices. This analysis examines the role that meaningful decisions play in direct democracy, because “for voters to make meaningful decisions, they must understand the options on which they are deciding” (Dalton 1988: 13). This analysis uses three different methodologies to investigate this relationship. First, through qualitative analysis and a mail survey of petitioners, this study explores how petitioners view and approach the public. This study finds that expectations of political knowledge affects how petitioners approach the public and how much time they spend educating the public about their initiative. Second, through statistical (multi-level regression) analysis, this study investigates the impact of the ballot language on participation in individual ballot propositions. This study finds that ballot language is a significant barrier to participation. Third, through experimental analysis, this study connects measures of political knowledge and participation on ballot propositions written by petitioners across the country. This study finds that when confronted with more difficult ballot language voters are less likely to participate. However, when controlling for political knowledge this effect is truncated. The findings of this analysis argue the elite bias of direct democracy in ballot language, accessibility, and motives of petitioners. The study of participation in direct democracy and political knowledge across American states advances the theoretical understanding of democratic participation, and furthers our understanding of the role citizen political knowledge plays in policymaking

    QUESTIONS ABOUT THE EFFECT OF IDENTITY ON POLITICAL ATTITUDES AND BEHAVIOR IN THE UNITED STATES ARE INCREASINGLY IMPORTANT AS ITS POPULATION BECOMES MORE DIVERSE. THE PRIMARY DRIVER OF THAT INCREASED DIVERSITY IS THE EXPANSION OF THE LATINO POPULATION. MOST RESEARCH FOCUSES ON DIFFERENCES BETWEEN VARIOUS ETHNIC GROUPS. THIS RESEARCH PROJECT SEEKS TO UNDERSTAND THE EFFECT OF IDENTITY STRENGTH WITHIN THE LATINO POPULATION ON ATTITUDES ABOUT GOVERNMENT. USING THE 2006 LATINO NATIONAL SURVEY, THE ROLE OF IDENTITY ON EFFICACY IS EXPLORED. THE RESULTS INDICATE THAT EFFICACY AMONG LATINOS IS RELATED TO IDENTITY. WE ALSO FIND RESULTS THAT REINFORCE THE PRESENCE OF CLASSIC INFLUENCES ON EFFICACY SUCH AS SOCIOECONOMIC STATUS AND POLITICAL ENGAGEMENT

    Autoimmunity and the oral cavity

    Purpose of Review: This review aims to make the oral health practitioner and researcher aware of autoimmune diseases that present in the mouth and have implications for oral health. Specifically, the autoimmune pathophysiology, examples of treatments, sequelae, and impact on oral health will be discussed. The limitations of our current knowledge and understanding of these diseases will also be highlighted. Recent Findings: Over the past decade, it has become clear that there is a bidirectional relationship between oral health and autoimmune disease at other body sites; including diabetes and rheumatoid arthritis. Improving the oral health of patients with autoimmune diseases may reduce morbidity associated with such diseases. Summary: Autoimmune diseases impact the oral cavity both directly and indirectly and are increasing in prevalence. As such, there is a growing need for the oral health practitioners to be aware of how autoimmune disease impacts the oral cavity. Further research is necessary to improve our understanding of the bidirectional relationship between oral health and autoimmune disease

    Barriers to recruiting men into chronic disease prevention and management programs in rural areas: Perspectives of program delivery staff

    Chronic disease is becoming increasingly prevalent in Canada. Many of these diseases could be prevented by adoption of healthy lifestyle habits including physical activity and healthy eating. Men, especially those in rural areas, are disproportionately affected by chronic disease. However, men are often underrepresented in community-based chronic disease prevention and management (CDPM) programs, including those that focus on physical activity and/or healthy eating. The purpose of this study was to explore the experiences and perceptions of program delivery staff regarding the challenges in recruitment and participation of men in physical activity and healthy eating programs in rural communities, and suggestions for improvement. Semistructured interviews were conducted by telephone with 10 CDPM program delivery staff from rural communities in Southwest Ontario, Canada. Time and travel constraints, relying on spouses, and lack of male program leaders were cited as barriers that contributed to low participation levels by men in CDPM programs. Hiring qualified male instructors and engaging spouses were offered as strategies to increase men’s participation. The results of this study highlight many of the current issues faced by rural health organizations when offering CDPM programming to men. Health care organizations and program delivery staff can use the recommendations in this report to improve male participation levels

    IL-21 receptor expression in human tendinopathy

    The pathogenetic mechanisms underlying tendinopathy remain unclear, with much debate as to whether inflammation or degradation has the prominent role. Increasing evidence points toward and early inflammatory infiltrate and associated inflammatory cytokine production in human and animal models of tendon disease. The IL-21/IL-21R axis is a proinflammatory cytokine complex that has been associated with chronic inflammatory diseases including rheumatoid arthritis and inflammatory bowel disease. This project aimed to investigate the role and expression of the cytokine/receptor pair IL-21/IL-21R in human tendinopathy. We found significantly elevated expression of IL-21 receptor message and protein in human tendon samples but found no convincing evidence of the presence of IL-21 at message or protein level. The level of expression of IL-21R message/protein in human tenocytes was significantly up regulated by proinflammatory cytokines (TNFα/IL-1ÎČ) in vitro. These findings demonstrate that IL-21R is present in early human tendinopathy mainly expressed by tenocytes and macrophages. Despite a lack of IL-21 expression these data again suggest that early tendinopathy has an inflammatory/cytokine phenotype, which may provide novel translational targets in the treatment of tendinopathy

    MicroRNA29a regulates IL-33-mediated tissue remodelling in tendon disease

    MicroRNA (miRNA) has the potential for cross-regulation and functional integration of discrete biological processes during complex physiological events. Utilizing the common human condition tendinopathy as a model system to explore the cross-regulation of immediate inflammation and matrix synthesis by miRNA we observed that elevated IL-33 expression is a characteristic of early tendinopathy. Using in vitro tenocyte cultures and in vivo models of tendon damage, we demonstrate that such IL-33 expression plays a pivotal role in the transition from type 1 to type 3 collagen (Col3) synthesis and thus early tendon remodelling. Both IL-33 effector function, via its decoy receptor sST2, and Col3 synthesis are regulated by miRNA29a. Downregulation of miRNA29a in human tenocytes is sufficient to induce an increase in Col3 expression. These data provide a molecular mechanism of miRNA-mediated integration of the early pathophysiologic events that facilitate tissue remodelling in human tendon after injury

    Participants’ perceptions of “C.H.A.M.P. families”: A parent-focused intervention targeting paediatric overweight and obesity

    Background: Recently, our team implemented a 13-week group-based intervention for parents of children with obesity (“C.H.A.M.P. Families”). The primary objective of this study was to explore, qualitatively, parents’ perspectives of their experiences in and influence of C.H.A.M.P. Families, as well as their recommendations for future paediatric obesity treatment interventions. Methods: Twelve parents (seven mothers, five fathers/step-fathers) representing seven children (four girls, three boys) with obesity participated in one of two focus groups following the intervention. Focus groups were audio recorded and transcribed verbatim and data were analyzed using inductive thematic analysis. Results: Findings showed that parents perceived their participation in C.H.A.M.P. Families to be a positive experience. Participants highlighted several positive health-related outcomes for children, families, and parents. Parents also underscored the importance and positive impact of the group environment, specific educational content, and additional program components such as free child-minding. Recommendations for future interventions were also provided, including greater child involvement and more practical strategies. Finally, parents identified several barriers including socioenvironmental issues, time constraints, and parenting challenges. Conclusions: Researchers developing family-based childhood obesity interventions should consider the balance of parent and child involvement, as well as emphasize group dynamics strategies and positive family communication

    Targeting danger molecules in tendinopathy: the HMGB1/TLR4 axis

    Objectives: To seek evidence of the danger molecule, high-mobility group protein B1 (HMGB1) expression in human tendinopathy and thereafter, to explore mechanisms where HMGB1 may regulate inflammatory mediators and matrix regulation in human tendinopathy. Methods: Torn supraspinatus tendon (established pathology) and matched intact subscapularis tendon (representing ‘early pathology’) biopsies were collected from patients undergoing arthroscopic shoulder surgery. Control samples of subscapularis tendon were collected from patients undergoing arthroscopic stabilisation surgery. Markers of inflammation and HMGB1 were quantified by reverse transcriptase PCR (RT-PCR) and immunohistochemistry. Human tendon-derived primary cells were derived from hamstring tendon tissue obtained during hamstring tendon anterior cruciate ligament reconstruction and used through passage 3. In vitro effects of recombinant HMGB1 on tenocyte matrix and inflammatory potential were measured using quantitative RT-PCR, ELISA and immunohistochemistry staining. Results: Tendinopathic tissues demonstrated significantly increased levels of the danger molecule HMGB1 compared with control tissues with early tendinopathy tissue showing the greatest expression. The addition of recombinant human HMGB1 to tenocytes led to significant increase in expression of a number of inflammatory mediators, including interleukin 1 beta (IL-1ÎČ), IL-6, IL-33, CCL2 and CXCL12, in vitro. Further analysis demonstrated rhHMGB1 treatment resulted in increased expression of genes involved in matrix remodelling. Significant increases were observed in Col3, Tenascin-C and Decorin. Moreover, blocking HMGB1 signalling via toll-like receptor 4 (TLR4) silencing reversed these key inflammatory and matrix changes. Conclusion: HMGB1 is present in human tendinopathy and can regulate inflammatory cytokines and matrix changes. We propose HMGB1 as a mediator driving the inflammatory/matrix crosstalk and manipulation of the HMGB1/TLR4 axis may offer novel therapeutic approaches targeting inflammatory mechanisms in the management of human tendon disorders
