It has been an enduring favourite topic of many scholars, even more so with the 100th anniversary just passed. Every aspect of the military campaign has been scrutinised, the attitudes of the men analysed, and the decisions of the leadership criticised. It would seem at face-value, there is little left to add to the discussion surrounding Gallipoli. It has long been acknowledged that this battle saw New Zealand, alongside Australia, step on to the word stage as a separate entity to the British Empire – and thus Anzac became synonymous with Gallipoli. Anzac has come to stand for much more than just the Australian and New Zealand Army Corps; it is a spirit and tradition. This was readily recognised when, after the Armistice of Mudros, the Canterbury Mounted Rifles, NZEF and 7th Light Horse Regiment, AIF were chosen to return to the Dardanelles as a part of the wider occupation of Turkey. During this return to Gallipoli, the Canterbury Mounted Rifles and 7th Light Horse took part in a reconnaissance of the Peninsula, and the Canterbury Mounted Rifles worked to identify and rebury the dead from the 1915 battle. The reverence with which this was done, and the fascination these Anzacs displayed when they patrolled the old battleground - shown in the photographs captured during their stay - indicates just how significant Gallipoli has always been as a physical space and a memory. Immediately after the Canterbury Mounted Rifles and 7th Light Horse left, the Australian Historical Mission led by Charles Bean arrived. The images captured by this team help provide a broader context for those captured by the Canterbury Mounted Rifles. The words of journalist Ernest Peacock, embedded with the 7th Light Horse, similarly provide framework for the photographs, as well as giving some information as to how this return to Gallipoli was seen from the home fronts of New Zealand and Australia. The Anzacs’ first pilgrimage to Gallipoli marked the beginning of a long-standing tradition that marks the birth of our nations

    Homologous recombination (HR)-based repair during DNA replication can apparently utilize several partially overlapping repair pathways in response to any given lesion. A key player in HR repair is the Sgs1-Top3-Rmi1 (STR) complex, which is critical for resolving X-shaped recombination intermediates formed following bypass of methyl methanesulfonate (MMS)-induced damage. STR mutants are also sensitive to the ribonucleotide reductase inhibitor, hydroxyurea (HU), but unlike MMS treatment, HU treatment is not accompanied by X-structure accumulation, and it is thus unclear how STR functions in this context. Here we provide evidence that HU-induced fork stalling enlists Top3 prior to recombination intermediate formation. The resistance of sgs1Δ mutants to HU is enhanced by the absence of the putative SUMO (Small Ubiquitin MOdifier)-targeted ubiquitin ligase, Uls1, and we demonstrate that Top3 is required for this enhanced resistance and for coordinated breaks and subsequent d-loop formation at forks stalled at the ribosomal DNA (rDNA) replication fork block (RFB). We also find that HU resistance depends on the catalytic activity of the E3 SUMO ligase, Mms21, and includes a rapid Rad51-dependent restart mechanism that is different from the slow Rad51-independent HR fork restart mechanism operative in sgs1Δ ULS1+ mutants. These data support a model in which repair of HU-induced damage in sgs1Δ mutants involves an error-prone break-induced replication pathway but, in the absence of Uls1, shifts to one that is higher-fidelity and involves the formation of Rad51-dependent d-loops

    Barriers to health promotion with overweight or obese children, young people and their families: a literature review

    Childhood obesity is a worldwide public health issue requiring sustained health promotion efforts by multiple healthcare professionals. Children's nurses are the main providers of education, support and interventions for those who are overweight or obese. Despite sustained health promotion efforts, addressing overweight and obesity in children and young people is an ongoing challenge. This article details a literature review that aimed to identify the barriers that children's nurses experience when undertaking health promotion to address overweight or obesity with children, young people and their families. Three themes were identified: lack of time, training and resources; parental motivation and response; and the 'new normal'. Education and work-based training on structured interventions for addressing overweight or obesity are vital. Skills training in broaching challenging conversations with children, young people and their families sensitively and effectively is also important

    Improving Community Advisory Board Engagement In Precision Medicine Research To Reduce Health Disparities

    Community Advisory Boards (CABs) are used in efforts to reduce health disparities; however, there is little documentation in the literature regarding their use in precision medicine research. In this case study, an academic-CAB partnership developed a questionnaire and patient educational materials for two precision smoking cessation interventions that involved use of genetic information. The community-engaged research (CEnR) literature provided a framework for enhancing benefits to CAB members involved in developing research documents for use with a low-income, ethnically diverse population of smokers. The academic partners integrated three CEnR strategies: 1) in-meeting statements acknowledging their desire to learn from community partners, 2) in-meeting written feedback to and from community partners, and 3) a survey to obtain CAB member feedback post-meetings. Strategies 1 and 2 yielded modifications to pertinent study materials, as well as suggestions for improving meeting operations that were then adopted, as appropriate, by the academic partners. The survey indicated that CAB members valued the meeting procedure changes which appeared to have contributed to improvements in attendance and satisfaction with the meetings. Further operationalization of relevant partnership constructs and development of tools for measuring these aspects of community-academic partnerships is warranted to support community engagement in precision medicine research studies

    MRI and gravimetric studies of hydrology in drip irrigated heaps and its effect on the propagation of bioleaching microorganisms

    Heap bioleaching performance is dependent on the contacting of the leach solution with the ore bed, hence on the system hydrodynamics. In this study two experimental setups were used to examine hydrodynamics associated with irrigation from a single drip emitter, one of the most common methods of heap irrigation. A specialist magnetic resonance imaging (MRI) method which is insensitive to the metal content of the ore was used to examine the liquid flow into an ore bed in the immediate vicinity of an irrigation point. The distribution of liquid in, microbial colonisation of and mineral recovery from a bioleach of a large scale 132 kg “ore slice” were subsequently monitored using sample ports positioned along the breadth and height of the reactor. In both systems the lateral movement of the liquid increased with bed depth, though preferential flow was evident. The majority of the liquid flow was in the region directly below the irrigation point and almost no liquid exchange occurred in the areas of lowest liquid content at the upper corners of the bed in which fluid exchange was driven by capillary action. The MRI studies revealed that the liquid distribution was unchanging following an initial settling of the ore bed and that, at steady state, the majority (~60%) of the liquid flowed directly into established large channels. The limited lateral movement of the liquid had a significant impact on the local leaching efficiencies and microbial colonisation of the ore with cell concentrations in the regions of lowest liquid content lying below the detection limit. Hence poor lateral liquid distribution with drip irrigation, and the associated impact on colonisation was identified as a significant disadvantage of this irrigation approach. Further, the need to optimise fluid exchange throughout the ore bed was identified as key for optimisation of leaching performance

    A targeted psychological treatment for sleep problems in young people at ultra-high risk of psychosis in England (SleepWell): a parallel group, single-blind, randomised controlled feasibility trial

    Background Sleep disturbance is common and problematic for young people at ultra-high risk of psychosis. Sleep disruption is a contributory causal factor in the occurrence of mental health problems, including psychotic experiences, anxiety, and depression. The implication is that treating sleep problems might have additional benefits on mental health outcomes in individuals at high risk. The present study had two aims: first, to establish the feasibility and acceptability of a randomised controlled trial to treat sleep problems with the aim of reducing psychotic experiences in young people at ultra-high risk of psychosis; and second, to provide proof of concept of the clinical efficacy of the treatment. Methods We did a parallel group, single-blind, randomised controlled feasibility trial in two National Health Service trusts in England. Eligible participants were aged 14–25 years, a patient of mental health services, assessed as being at ultra-high risk of psychosis on the Comprehensive Assessment of At-Risk Mental States, and having current sleep problems (score of ≥15 on the self-report Insomnia Severity Index [ISI]). Participants were randomly assigned (1:1) to either a targeted psychological therapy for sleep problems (SleepWell) plus usual care or usual care alone via an automated online system, with non-deterministic minimisation that balanced participants for ISI score and referring service. The SleepWell therapy was delivered on an individual basis in approximately eight 1-h sessions over 12 weeks. Assessments were done at 0, 3, and 9 months, with trial assessors masked to treatment allocation. The key feasibility outcomes were the numbers of patients identified, recruited, and retained, treatment uptake, and data completion. Treatment acceptability was measured with the Abbreviated Acceptability Rating Profile (AARP). In preliminary clinical assessments, the primary clinical outcome was insomnia at 3 and 9 months assessed with the ISI, reported by randomised group (intention-to-treat analysis). Safety was assessed in all randomly assigned participants. The trial was prospectively registered on ISRCTN, 85601537, and is completed. Findings From Nov 18, 2020, to Jan 26, 2022, 67 young people were screened, of whom 40 (60%) at ultra-high risk of psychosis were recruited. Mean age was 16·9 years (SD 2·5; range 14–23), and most participants identified as female (n=19 [48%]) or male (n=19 [48%]) and as White (n=32 [80%]). 21 participants were randomly assigned to SleepWell therapy plus usual care and 19 to usual care alone. All participants provided data on at least one follow-up visit. 39 (98%) of 40 participants completed the primary outcome assessment at 3 and 9 months. 20 (95%) of 21 participants assigned to SleepWell therapy received the prespecified minimum treatment dose of at least four sessions. The median treatment acceptability score on the AARP was 48 (IQR 46 to 48; n=17; maximum possible score 48). At the post-intervention follow-up (3 months), compared with the usual care alone group, the SleepWell therapy group had a reduction in insomnia severity (ISI adjusted mean difference –8·12 [95% CI –11·60 to –4·63]; Cohen's d=–2·67 [95% CI –3·81 to –1·52]), which was sustained at 9 months (ISI adjusted mean difference –5·83 [–9·31 to –2·35]; Cohen's d=–1·91 [–3·06 to –0·77]). Among the 40 participants, eight adverse events were reported in six participants (two [11%] participants in the usual care group and four [19%] participants in the SleepWell therapy group). One serious adverse event involving hospital admission for a physical health problem was reported in the SleepWell therapy group, and one patient in the usual care alone group transitioned to psychosis. None of these events were classed as being related to trial treatment or procedures. Interpretation A randomised controlled trial of a targeted psychological sleep therapy for young people at ultra-high risk of psychosis is feasible. Patients can be retained in the trial and assessments done by masked assessors. Uptake of the sleep therapy was high, and we found preliminary evidence of sustained reductions in sleep problems.     Factors associated with depressive symptoms in young long-term breast cancer survivors

    Purpose Long-term breast cancer survivors frequently report distress (i.e., depressive symptoms) that impacts their quality of life. Previous studies have found that negative social interactions (“social constraints”) from partners contribute to long-term, unresolved cycling of intrusive thoughts and cognitive avoidance, resulting in psychological distress. However, these relationships have not been tested in long-term breast cancer survivors. Furthermore, the effect of partners’ depressive symptoms on the survivors’ depressive symptoms has not been tested within the context of these relationships. Therefore, the purpose of this study was to test relationships between breast cancer survivors’ depressive symptoms and (1) social constraints, cognitive avoidance, and intrusive thoughts, and (2) partners’ depressive symptoms. Methods Data were from a cross-sectional descriptive study of breast cancer survivors (N = 222) 3–8 years post-diagnosis and their partners, who completed surveys assessing demographic characteristics, social constraints, intrusive thoughts, cognitive avoidance, and depressive symptoms. Structural equation modeling confirmatory path analyses were conducted to determine significant relationships between survivors’ depressive symptoms and all other variables. Results Our model fits the data well. Breast cancer survivors’ depressive symptoms were predicted by social constraints and intrusive thoughts. The relationship between survivors’ depressive symptoms and partners’ depressive symptoms was close but not significant. Conclusions As hypothesized, depressive symptoms were predicted by social constraints and intrusive thoughts. Further research is needed to understand the possible relationship between survivors’ long-term depressive symptoms and cognitive avoidance and partners’ depressive symptoms. Our findings highlight the negative impact of social constraints from partners on psychological outcomes in long-term breast cancer survivors

    Long-Term Fear of Recurrence in Young Breast Cancer Survivors and Partners

    Background Fear of a breast cancer recurrence is the most prevalent and disruptive source of distress for long-term survivors and their partners. However, few studies have focused on predictors of fear of recurrence. The aim of this study is to test the efficacy of the Social Cognitive Processing Theory (SCPT) in predicting fear of recurrence in long-term breast cancer survivors diagnosed at age 45 years or younger and their partners. Methods In a large cross-sectional study, breast cancer survivors (n = 222) 3–8 years from diagnosis and their partners completed a survey assessing demographic characteristics, fear of recurrence, social constraints, and cognitive processing (intrusive thoughts and cognitive avoidance). Mediation analyses were conducted for survivors and partners separately to determine if cognitive processing would mediate the relationship between social constraints and fear of recurrence. Results Cognitive processing mediated the relationship between social constraints and fear of recurrence both for survivors [F(3,213) = 47.541, R2 = 0.401, p < 0.001] and partners [F(3,215) = 27.917, R2 = 0.280, p < 0.001). Demographic variables were not significant predictors of fear of recurrence. Conclusions As predicted, cognitive processing mediated the relationship between social constraints and fear of recurrence.
