    Comparación de Características de Resistencia y Durabilidad de un Geopolímero obtenido a partir de ceniza Volante, Fibra de Vidrio Esmerilado y Polvo de Vidrio

    Strength and durability characteristics of geopolymers produced using three precursors, consisting of fly ash, Ground Glass Fiber (GGF), and glass-powder were studied. Combinations of sodium hydroxide and sodium silicate were used as the activator solutions, and the effect of different sodium and silica content of the activators on the workability and compressive strength of geopolymers was investigated. The parameters used in this study were the mass ratio of Na2O-to-binder (for sodium content), and SiO2-to-Na2O of the activator (for silica content). Geopolymer mixtures that achieved the highest compressive strength from each precursor were assessed for their resistance to alkali-silica reaction and compared against the performance of portland cement mixtures. Test results revealed that GGF and fly ash-based geopolymers performed better than glass-powder-based geopolymer mixtures. The resistance of GGF-based and fly ash-based geopolymers to alkali-silica reaction was superior to that of portland cement mixtures, while glass-powder-based geopolymer showed inferior performance.Se estudiaron las características de resistencia y durabilidad de geopolímeros producidos utilizando tres precursores, formados por cenizas volantes, Fibra de Vidrio Esmerilado (FVE) y vidrio en polvo. Se utilizaron combinaciones de soluciones de hidróxido de sodio y silicato de sodio como activadores, y se investigó el efecto del diferente contenido de sodio y sílice de los activadores en la trabajabilidad y resistencia a la compresión de los geopolímeros. Los parámetros utilizados en este estudio fueron la relación de masa de Na2O-a-aglutinante (para el contenido de sodio), y SiO2-a-Na2 O del activador (para el contenido de sílice). Las mezclas de geopolímeros obtenidas a partir de cada precursor que alcanzaron la más alta resistencia a la compresión fueron evaluadas por su resistencia a la reacción álcali-sílice y comparadas con el rendimiento de las mezclas de cemento portland. Los resultados de las pruebas revelaron que la FVE y los geopolímeros a base de ceniza volante se comportaron mejor que las mezclas de geopolímeros a base de vidrio en polvo. La resistencia de los geopolímeros a base de ceniza volante y FVE a la reacción álcali-sílice fue superior que la de las mezclas de cemento portland, mientras que los geopolímeros a base de vidrio en polvo mostraron un rendimiento inferior

    Family medicine in Iran: facing the health system challenges

    OBJECTIVE: This study aimed to understand the perspectives of policy makers and decision makers of Iran's health system about the implementation of family medicine in Iran urban areas. MATERIALS/PATIENTS & METHODS: This study is a qualitative study with framework analysis. Purposive semi-structured interviews were conducted with Policy and decision makers in the five main organizations of Iran health care system. The codes were extracted using inductive and deductive methods. RESULTS: According to 27 semi-structured interviews were conducted with Policy and decision makers, three main themes and 8 subthemes extracted, including: The development of referral system, better access to health care and the management of chronic diseases. CONCLUSION: Family medicine is a viable means for a series of crucial reforms in the face of the current challenges of health system. Implementation of family medicine can strengthen the PHC model in Iran urban areas. Attempting to create a general consensus among various stakeholders is essential for effective implementation of the project.     The experience of implementing the board of trustees� policy in teaching hospitals in Iran: An example of health system decentralization

    Background: In 2004, the health system in Iran initiated an organizational reform aiming to increase the autonomy of teaching hospitals and make them more decentralized. The policy led to the formation of a board of trustees in each hospital and significant modifications in hospitals� financing. Since the reform aimed to improve its predecessor policy (implementation of hospital autonomy began in 1995), it expected to increase user satisfaction, as well as enhance effectiveness and efficiency of healthcare services in targeted hospitals. However, such expectations were never realized. In this research, we explored the perceptions and views of expert stakeholders as to why the board of trustees� policy did not achieve its perceived objectives. Methods: We conducted 47 semi-structured face-to-face interviews and two focus group discussions (involving 8 and 10 participants, respectively) with experts at high, middle, and low levels of Iran�s health system, using purposive and snowball sampling. We also collected a comprehensive set of relevant documents. Interviews were transcribed verbatim and analyzed thematically, following a mixed inductive-deductive approach. Results: Three main themes emerged from the analysis. The implementation approach (including the processes, views about the policy and the links between the policy components), using research evidence about the policy (local and global), and policy context (health system structure, health insurers capacity, hospitals� organization and capacity and actors� interrelationships) affected the policy outcomes. Overall, the implementation of hospital decentralization policies in Iran did not seem to achieve their intended targets as a result of assumed failure to take full consideration of the above factors in policy implementation into account. Conclusion: The implementation of the board of trustees� policy did not achieve its desired goals in teaching hospitals in Iran. Similar decentralization policies in the past and their outcomes were overlooked, while the context was not prepared appropriately and key stakeholders, particularly the government, did not support the decentralization of Iran�s health system. © 2015 by Kerman University of Medical Sciences

    1-Methyl tryptophan, an indoleamine 2,3-dioxygenase inhibitor, attenuates cardiac and hepatic dysfunction in rats with biliary cirrhosis

    Kynurenine Pathway (KP) is the dominant metabolic route of tryptophan which is catalyzed by indoleamine-2,3-dioxygenase (IDO). This pathway is upregulated in liver disease where the level of KP metabolites correlates with the severity of disease. Cirrhosis is associated with cardiac dysfunction, which manifests itself during severe physiological challenges such as liver transplantation. Cardiac dysfunction in cirrhosis is linked to systemic inflammation and impaired cardiac beta-adrenergic signaling pathways. The KP pathway is involved in modulation of cardiac signaling and is upregulated by systemic inflammation. Therefore, this study aimed to evaluate the effect of IDO inhibition on development of cardiac dysfunction in an experimental model of cirrhosis. Cirrhosis was induced by bile duct ligation (BDL). Experimental groups were given either 1-methyl tryptophan (1-MT, 1, 3, 9 mg/kg), or saline. 28 days after BDL, cardiac chronotropic response to epinephrine was assessed ex vivo. HPLC was employed to measure hepatic and cardiac levels of tryptophan, kynurenine and kynurenic acid. Cirrhosis in rats was associated with impaired cardiac chronotropic responsiveness to adrenergic stimulation. 1-MT dose-dependently improved cirrhosis-induced chronotropic dysfunction as well as elevated serum levels of CRP and IL-6 in BDL rats. Hepatic and cardiac kynurenine/tryptophan ratio were elevated in cirrhotic rats and were reduced following 1-MT administration. Chronic administration of 1-MT could also reduce hepatic inflammation, fibrosis and ductular proliferation. 1-MT attenuates cardiac dysfunction in rats with biliary cirrhosis. This protective effect is not limited to the cardiac function as liver histopathologic changes were also improved following chronic 1-MT administration
