    Study of fetal doppler velocimetry versus non stress test as predictors of adverse perinatal outcome in high risk pregnancies

    Background: Pregnancy is a unique, physiologically normal event in a women’s life. Objective of this study was to compare the efficacy of the doppler velocimetry versus non stress test in relation to perinatal outcome in high risk pregnancies.Methods: This is a prospective study conducted in the department of obstetrics and gynaecology, Narayana Medical College and Hospital. 100 women with high risk pregnancy were recruited. All were examined systematically, and Doppler velocimetry and non-stress test were done.Results: All cases were divided into four groups based on NST and doppler velocimetry of umbilical artery and middle cerebral artery. 10% of women had abnormal doppler. Middle cerebral artery doppler abnormality was noted in 3% and CPR abnormality in 3% of women in the study group. 15% had abnormal NST. In Group A, out of 88 patients 9 had fetal compromise. In Group B, out of 5 patients all had fetal compromise. In Group C, out of 4 patients none had fetal compromise. In Group D, all 3 patients had fetal compromise. In Group D, all 3 had neonatal deaths. Average birth weights in Group A was 2.7 kg, in Group B was 2 kg, in Group C was 2.5 kg, in Group D was 1.4 kg. Two (2.2%) newborn in Group A, 4 (80%) newborns in Group B, 3 (100%) in Group had Apgar < 7 at 5 minutes. 4 (4.5%) babies in Group A, 5 (100%) babies in Group B, 3 (100%) babies in Group D were admitted in NICU. Umbilical artery doppler was found to have sensitivity 46.6%, specificity - 94%, PPV - 93%, NPV - 54%. Middle cerebral artery doppler was found to have sensitivity 73.3%, specificity - 90%, PPV - 91.6%, NPV- 69.3%.Conclusions: In present study, highest percentage of perinatal complications and perinatal deaths were seen in groups with abnormal tests of NST and velocimetry. Group D had the worst perinatal outcome

    Acceptance of postplacental intrauterine contraceptive device: recent increase in trend

    Background: IUCD is a temporary method of contraception in use for many years. Our main objective is to find the acceptance of (postplacental intrauterine contraceptive device) PPIUCD in recent years.Methods: Retrospective Analysis was carried out to collect information regarding IUCD insertion in all women delivered at semi urban based medical college in Tamil Nadu from April 2014 to March 2016.Results: Total women delivered in our hospital were 20949 over a period of two years. Two year comparative study revealed significant increase in acceptance of PPIUCD insertion among primipara from 39.1% TO 68.9%. Acceptance of PPIUCD in primipara was 51.6% in first year (April 2014 to March 2015) to 87.8% in second year (April 2015 to March 2016).Most of the women were in the age group of 20 to 29 years (81.5%).Conclusions: The present study being retrospective analysis showed sudden increase in coverage rate of PPIUCD from 2014-2015 to 2015-2016. Though the patients were motivated in the same way throughout the period, training and motivation given to the staff nurses and duty doctors in the labour ward for PPIUCD insertion and daily review instead of weekly had brought the significant increase in the coverage. This clearly shows the role of service provider in the success of a national program though acceptor is always ready in most of the times

    Case report on perioperative management of combined factor V and VIII deficiency in a patient with prolapse uterus

    Combined factor V and factor VIII deficiency is a rare autosomal recessive disorder with incidence of one in 10,00,000. We reported a case of seventy-one-year-old patient with third degree uterovaginal prolapse, diagnosed as combined factor V and VIII deficiency and the successful perioperative management during vaginal hysterectomy. With our experience in this, we conclude that for the patient's with factor V and VIII deficiency that the correction should be given for at least 2 weeks post operatively

    Hikami-Larkin-Nagaoka (HLN) Fitting of Magneto Transport of Bi2Se3 Single Crystal in Different Magnetic Field Ranges

    We report the detailed study of structural micro-structuraland high magnetic field magneto transport propertiesof Bi2Se3single crystal. Bi2Se3 single crystal is grown through conventional solid-state reaction route via the self-flux method. Rietveld analysis on Powder X-ray Diffraction showed that the studied Bi2Se3 crystal is crystallized in single-phase without any impurity. The surface morphology analyzed through Scanning Electron Microscopy study which shows that as-grown single crystal exhibit layered type structure and the quantitative weight of the atomic constituents (Bi and Se) are found to be closeto the stoichiometric amount in energy-dispersive X-ray spectroscopy analysis. Low temperature (2.5K) magneto-resistance (MR) exhibited a v-type cusp around origin at lower magnetic field, which is the sign of weak anti-localization effect. Further, Bi2Se3 single crystal magneto conductivity data is fitted by well-known HLN equation in different magnetic field range of 2Tesla, 4Tesla and 6Tesla and the resultant found that the conduction mechanism of Bi2Se3 is dominated by WAL state.Comment: 4 Pages Text + Fig

    Controversies in the management of early endometrial carcinoma: an update

    Endometrial carcinoma (EC) is the commonest genital tract malignancy in developing countries and is usually confined to the uterus at the time of diagnosis with excellent prognosis and high cure rates. But the management is associated with lot of controversies like in staging, best surgical approach, extent of lymphadenectomy, adjuvant therapy, fertility sparing surgery in young women etc. A thorough surgical staging is important to determine uterine and extrauterine spread and also understanding of the pathophysiology and management strategies to identify women who are at high risk and tailoring the adjuvant treatment if necessary without increasing the morbidity. This evidence based narrative review conducted by searching Medline (1994- 2015) and other online articles from Pubmed, Google scholar. Articles were selected based on their currency and relevance to the discussion they summarize the current literature to provide an approach to best practice management of early endometrial carcinoma

    Semperella megaloxea sp. nov. (Family: Pheronematidae): A new hexactinellid sponge from Andaman waters, India

    The hexactinellid sponge described in this paper, Semperella megaloxea sp. nov. was collected from the eastern side of North Andaman waters off Diglipur at 13o 06’ N lat. and 93o 11’ E long. during one of the FORV Sagar Sampada expeditions. Critical examination of the specimen indicated that it belonged to the Class Hexactinellida, Sub-class Amphidiscophora and Order Amphidiscosida. Although diactin spicules are present, the presence of large number of pentactin spicules indicates that it belongs to the Family Pheronematidae. The columnar body with atrial surface divided into several areas separated by dermal areas indicates that the specimen belongs to the genus Semperella. The morphology of the whole sponge specimen, the presence of very long diactin spicules of oscular tuft and the pentactins with very long rays do not match well with any of the already described species of the genus Semperella Gray
