    The Earls of Derby and the Early-Modern Performance Culture of North-West England

    This special issue of Shakespeare Bulletin is concerned with the 3rd to 7th Earls of Derby, their patronage roles and other involvements in performance cultures in the early-modern period. This forms one central strand of research into the early-modern theatrical history of the small town of Prescot (now in Liverpool City Region's Borough of Knowsley; formerly in south Lancashire). It is this history, and the connections between Knowsley and Shakespearean theatre which it evidences, that inform the current Shakespeare North Playhouse Project, a major, heritage-based, urban regeneration initiative that has been developing for over a decade and which is now coming to fruition. In taking members of the Lancashire-based Stanley dynasty as focalizers of early-modern performance history, this issue addresses under-researched issues of regionality in theatre history. It reverses an older, London-based outlook: by taking Knowsley and Prescot as a viewpoint and looking outwards from north-west England towards other regions, London, and beyond, it adds to work offering new perspectives on the place(s) of theatre in the early-modern period.     An economic model of long-term use of celecoxib in patients with osteoarthritis

    <p>Abstract</p> <p>Background</p> <p>Previous evaluations of the cost-effectiveness of the cyclooxygenase-2 selective inhibitor celecoxib (Celebrex, Pfizer Inc, USA) have produced conflicting results. The recent controversy over the cardiovascular (CV) risks of rofecoxib and other coxibs has renewed interest in the economic profile of celecoxib, the only coxib now available in the United States. The objective of our study was to evaluate the long-term cost-effectiveness of celecoxib compared with nonselective nonsteroidal anti-inflammatory drugs (nsNSAIDs) in a population of 60-year-old osteoarthritis (OA) patients with average risks of upper gastrointestinal (UGI) complications who require chronic daily NSAID therapy.</p> <p>Methods</p> <p>We used decision analysis based on data from the literature to evaluate cost-effectiveness from a modified societal perspective over patients' lifetimes, with outcomes expressed as incremental costs per quality-adjusted life-year (QALY) gained. Sensitivity tests were performed to evaluate the impacts of advancing age, CV thromboembolic event risk, different analytic horizons and alternate treatment strategies after UGI adverse events.</p> <p>Results</p> <p>Our main findings were: 1) the base model incremental cost-effectiveness ratio (ICER) for celecoxib versus nsNSAIDs was 31,097perQALY;2)theICERperQALYwas31,097 per QALY; 2) the ICER per QALY was 19,309 for a model in which UGI ulcer and ulcer complication event risks increased with advancing age; 3) the ICER per QALY was $17,120 in sensitivity analyses combining serious CV thromboembolic event (myocardial infarction, stroke, CV death) risks with base model assumptions.</p> <p>Conclusion</p> <p>Our model suggests that chronic celecoxib is cost-effective versus nsNSAIDs in a population of 60-year-old OA patients with average risks of UGI events.</p

    The fundamentals of Lyman alpha exoplanet transits

    Lyman α transits have been detected from several nearby exoplanets and are one of our best insights into the atmospheric escape process. However, due to ISM absorption, we typically only observe the transit signature in the blue-wing, making them challenging to interpret. This challenge has been recently highlighted by non-detections from planets thought to be undergoing vigorous escape. Pioneering 3D simulations have shown that escaping hydrogen is shaped into a cometary tail receding from the planet. Motivated by this work, we develop a simple model to interpret Lyman α transits. Using this framework, we show that the Lyman α transit depth is primarily controlled by the properties of the stellar tidal field rather than details of the escape process. Instead, the transit duration provides a direct measurement of the velocity of the planetary outflow. This result arises because the underlying physics is the distance a neutral hydrogen atom can travel before it is photoionized in the outflow. Thus, higher irradiation levels, expected to drive more powerful outflows, produce weaker, shorter Lyman α transits because the outflowing gas is ionized more quickly. Our framework suggests that the generation of energetic neutral atoms may dominate the transit signature early, but the acceleration of planetary material produces long tails. Thus, Lyman α transits do not primarily probe the mass-loss rates. Instead, they inform us about the velocity at which the escape mechanism is ejecting material from the planet, providing a clean test of predictions from atmospheric escape models
