Resolución por Skype de una tarea de visualización cooperativa por una pareja de estudiantes de talento
En este trabajo analizamos la transmisión de información entre dos estudiantes de alta capacidad matemática mientras resuelven, a través de videoconferencia, una tarea cooperativa de visualización. Las categorías que más afloraron en el análisis del discurso fueron informar, demandar información, sugerir, parafrasear y confirmar. Los indicadores que más manifestaron fueron validación, ampliación y paráfrasis. Los estudiantes manifestaron características del talento matemático como un alto compromiso con la tarea, a pesar de su dificultad, y buena capacidad para organizar los datos y para desarrollar estrategias eficientes de resolución
New regulations regarding Postgraduate Medical Training in Spain: perception of the tutor's role in the Murcia Region
<p>Abstract</p> <p>Background</p> <p>Recently introduced regulatory changes have expanded the Tutor role to include their primary responsibility for Postgraduate Medical Training (PMT). However, accreditation and recognition of that role has been devolved to the autonomic regions. The opinions of the RT may be relevant to future decisions;</p> <p>Methods</p> <p>A comprehensive questionnaire, including demographic characteristics, academic and research achievement and personal views about their role, was sent to 201 RTs in the Murcia Region of Spain. The responses are described using median and interquartile ranges (IQR);</p> <p>Results</p> <p>There were 147 replies (response rate 73%), 69% male, mean age 45 ± 7 yrs. RTs perception of the residents' initial knowledge and commitment throughout the program was 5 (IQR 4-6) and 7 (IQR 5-8), respectively. As regards their impact on the PMT program, RTs considered that their own contribution was similar to that of senior residents. RTs perception of how their role was recognised was 5 (IQR 3-6). Only 16% did not encounter difficulties in accessing specific RT training programs. Regarding the RTs view of their various duties, supervision of patient care was accorded the greatest importance (64%) while the satisfactory completion of the PMT program and supervision of day-to-day activities were also considered important (61% and 59% respectively). The main RT requirements were: a greater professional recognition (97%), protected time (95%), specific RT training programs (95%) and financial recognition (86%);</p> <p>Conclusions</p> <p>This comprehensive study, reflecting the feelings of our RTs, provides a useful insight into the reality of their work and the findings ought to be taken into consideration in the imminent definitive regulatory document on PMT.</p
Immunological response against SARS-CoV-2 following full-dose administration of Comirnaty® COVID-19 vaccine in nursing home residents

Methods: A prospective cohort study in a representative sample from nursing homes in Valencia (n = 881; males: 271, females 610; median age, 86 years) recruited residents using a random one-stage cluster sampling approach. A lateral flow immunochromatography device (LFIC) (OnSite COVID-19 IgG/IgM Rapid Test; CTK BIOTECH, Poway, CA, USA) was used as the front-line test for detecting SARS-CoV-2-Spike (S)-specific antibodies in whole blood obtained using a fingerstick. Residents returning negative LFIC results underwent venipuncture and testing for presence of SARS-CoV-2-S-reactive antibodies and T cells using the Roche Elecsys® Anti-SARS-CoV-2 S (Roche Diagnostics, Pleasanton, CA, USA), the LIAISON® SARS-CoV-2 TrimericS IgG assay (Diasorin S.p.A, Saluggia, Italy) and by flow cytometry, respectively.
Results: The SARS-CoV-2-S antibody detection rate in nursing home residents was 99.6% (283/284) and 98.3% (587/597) for SARS-CoV-2 recovered and naïve residents, respectively, within a median of 99 days (range 17-125 days) after full vaccination. Three out of five residents lacking SARS-CoV-2-S antibodies had detectable S-reactive CD8+ and/or CD4+ T cells. In addition, 50/50 and 40/50 participants with detectable SARS-CoV-2 antibodies also had SARS-CoV-2-S-reactive interferon-γ-producing CD4+ and CD8+ T cells, respectively.
Discussion: The Comirnaty® COVID-19 vaccine is highly immunogenic in nursing home residents.

Robust estimation of bacterial cell count from optical density
Optical density (OD) is widely used to estimate the density of cells in liquid culture, but cannot be compared between instruments without a standardized calibration protocol and is challenging to relate to actual cell count. We address this with an interlaboratory study comparing three simple, low-cost, and highly accessible OD calibration protocols across 244 laboratories, applied to eight strains of constitutive GFP-expressing E. coli. Based on our results, we recommend calibrating OD to estimated cell count using serial dilution of silica microspheres, which produces highly precise calibration (95.5% of residuals <1.2-fold), is easily assessed for quality control, also assesses instrument effective linear range, and can be combined with fluorescence calibration to obtain units of Molecules of Equivalent Fluorescein (MEFL) per cell, allowing direct comparison and data fusion with flow cytometry measurements: in our study, fluorescence per cell measurements showed only a 1.07-fold mean difference between plate reader and flow cytometry data
Alternativas a la distribución de medicamentos y su retribución
La distribución de medicamentos en España se articula mayoritariamente alrededor de las 20.000 oficinas de farmacia (OOFF) independientes radicadas en el territorio. Esta situación contrasta con la de otros países en que otros proveedores sanitarios desempeñan un papel primordial en la dispensación de fármacos, o con la de aquellos cuyas farmacias forman parte de conglomerados industriales o cadenas empresariales. Se presenta una descripción de la cadena de distribución del medicamento española -mayoristas y OOFF-, destacando 5 aspectos relevantes para la consideración de alternativas: titularidad de la farmacia y actuación profesional, criterios de radicación, monopolio de dispensación, concierto automático con el Sistema Nacional de la Salud (SNS) y sistema de retribución. Se consideran algunas alternativas existentes en países de nuestro entorno: distribución a distancia, venta de especialidades farmacéuticas publicitarias en establecimientos distintos de las OOFF, constitución de cadenas farmacéuticas, dispensación por proveedores, repercusiones de la prescripción electrónica y las posibilidades de la aún emergente atención farmacéutica. Asimismo se revisan las características de los sistemas de retribución de las farmacias. Se proponen la modificación de las limitaciones a la propiedad de las OOFF, el establecimiento de convenios de libre aceptación entre éstas y el SNS, y la autorización de canales alternativos o complementarios para la distribución de algunos productos. En cuanto a la retribución, se propugna un sistema mixto que contemple: a) una tarifa fija por dispensación; b) la devolución más o menos total del coste del producto; c) una retribución por servicios definidos explícitamente por el financiador, y d) para determinadas situaciones, la posibilidad de un pago fijo selectivo según los servicios concertados o una garantía de ingresos mínimos
Hepatic allograft arterialization by means of the gastroduodenal bifurcation (branch patch) as a prognostic factor
Abstract INTRODUCTION: Because of the current shortage of cadaveric organs, it is important to determine preoperatively those variables that are readily available, inexpensive, and noninvasive that can predict a higher incidence of hepatic artery thrombosis (HAT). MATERIAL AND METHODS: From April 1986 to October 2001, 717 patients underwent 804 liver transplants. All the arterial reconstructions were performed with fine (7-0) monofilament sutures in an interrupted fashion. Two methods were used: group I, end-to-end arterial anastomosis, and group II, the gastroduodenal branch patch. RESULTS: After a mean follow-up of 72 (range 3-174) months, HAT was observed in 19 patients (overall incidence 2.4%). End-to-end anastomosis (group I) was performed in 39.50% (316) of cases, and HAT developed in 14 (4.4%) cases. Branch-patch anastomoses (group II) were carried out in 60.5% (488) of the patients; the presence of HAT was detected in five cases (1.03%) (P = 0.03, P < 0.05). A total of 21 variables were selected in the univariate analysis; however, after the multivariate analysis, all but two of the factors lost statistical significance, and these corresponded to the type of arterial reconstruction (gastroduodenal branch patch vs. end-to-end) and the ABO compatibility. CONCLUSIONS: Liver transplantation with compatible grafts using branch-patch anastomosis for the arterialization (both manipulative by the transplant team) reduces HAT-derived loss of grafts, with the consequent increase in graft availability and reduced mortality rate on the waiting list
