    Posttraumatic Stress among Young Urban Children Exposed to Family Violence and Other Potentially Traumatic Events

    This study examines the relationship between the number of types of traumatic events experienced by children 3 to 6 years old, parenting stress, and children\u27s posttraumatic stress (PTS). Parents and caregivers provided data for 154 urban children admitted into community-based mental health or developmental services. By parent and caregiver report, children experienced an average of 4.9 different types of potentially traumatic events. Nearly one quarter of the children evidenced clinically significant PTS. Posttraumatic stress was positively and significantly related to family violence and other family-related trauma exposure, nonfamily violence and trauma exposure, and parenting stress. Additionally, parenting stress partially mediated the relationship between family violence and trauma exposure and PTS. This study highlights the need for early violence and trauma exposure screening in help-seeking populations so that appropriate interventions are initiated

    The Alcohol Intervention Training Program (AITP): A response to alcohol misuse in the farming community

    Background Farm men and women in Australia have higher levels of problematic alcohol use than their urban counterparts and experience elevated health risks associated with excessive alcohol consumption. The Sustainable Farm Families (SFF) program has worked successfully with farm men and women to address health, well- being and safety and has identified that further research and training is required to understand and address alcohol misuse behaviours. This project will add an innovative component to the program by training health professionals working with farm men and women to discuss and respond to alcohol-related physical and mental health problems. Methods/Design A mixed method design with multi-level evaluation will be implemented following the development and delivery of a training program (The Alcohol Intervention Training Program {AITP}) for Sustainable Farm Families health professionals. Pre-, post- and follow-up surveys will be used to assess both the impact of the training on the knowledge, confidence and skills of the health professionals to work with alcohol misuse and associated problems, and the impact of the training on the attitudes, behaviour and mental health of farm men and women who participate in the SFF project. Evaluations will take a range of forms including self-rated outcome measures and interviews. Discussion The success of this project will enhance the health and well-being of a critical population, the farm men and women of Australia, by producing an evidence-based strategy to assist them to adopt more positive alcohol-related behaviours that will lead to better physical and mental health

    Proprotein Convertase Subtilisin/Kexin Type 9 Inhibitors and Ezetimibe on Risk of New-Onset Diabetes: A Systematic Review and Meta-Analysis of Large, Double-Blinded Randomized Controlled Trials

    © The Author(s) 2020. Background: Previous meta-analyses have shown that statins may cause incident diabetes. This article reviews randomized controlled trials using proprotein convertase subtilisin/kexin 9 inhibitors (PCSK9i) or ezetimibe on the risk of new-onset diabetes. Methods: Eight trials involving PCSK9i and 3 trials of ezetimibe were selected for review. PubMed, Cochrane Central Register of Controlled Trials, and Clinicaltrials.gov were thoroughly searched for relevant trials. Inclusion criteria included at least 100 patients per treatment arm, follow-up of at least 52 weeks, and at least double-blinded study design. Exclusion criteria included patients with previously diagnosed diabetes, nonrandomized, placebo-controlled, open-label, and crossover trials. The primary outcome was the number of incident diabetes cases. A random effects model was used. Heterogeneity in effect sizes was measured with I2 parameter and the Q statistic was used to test for excessive between-study heterogeneity. Results: A total of 52 214 participants for the PCSK9i and a total of 20 084 for the ezetimibe meta-analyses were included. Participants randomized to PCSK9i did not differ from the control patients in diabetes incidence (risk ratio [RR] = 0.99, P =.87, 95% CI = 0.92-1.07). Participants randomized to ezetimibe did not differ from the control patients in diabetes incidence (RR = 1.05, P =.37, 95% CI = 0.95-1.15). Discussion: The use of PCSK9i and ezetimibe does not appear to impact the risk of incident diabetes mellitus when added to guideline-directed medical therapy

    Monitoring and responding to emerging infectious diseases in a university setting: A case study using COVID-19

    AbstractEmerging infection diseases (EIDs) are an increasing threat to global public health, especially when the disease is newly emerging. Institutions of higher education (IHEs) are particularly vulnerable to EIDs because student populations frequently share high-density residences and strongly mix with local and distant populations. In fall 2020, IHEs responded to a novel EID, COVID-19. Here, we describe Quinnipiac University’s response to SARS-CoV-2 and evaluate its effectiveness through empirical data and model results. Using an agent-based model to approximate disease dynamics in the student body, the University established a policy of dedensification, universal masking, surveillance testing via a targeted sampling design, and app-based symptom monitoring. After an extended period of low incidence, the infection rate grew through October, likely due to growing incidence rates in the surrounding community. A super-spreader event at the end of October caused a spike in cases in November. Student violations of the University’s policies contributed to this event, but lax adherence to state health laws in the community may have also contributed. The model results further suggest that the infection rate was sensitive to the rate of imported infections and was disproportionately impacted by non-residential students, a result supported by the observed data. Collectively, this suggests that campus-community interactions play a major role in campus disease dynamics. Further model results suggest that app-based symptom monitoring may have been an important regulator of the University’s incidence, likely because it quarantined infectious students without necessitating test results. Targeted sampling had no substantial advantages over simple random sampling when the model incorporated contact tracing and app-based symptom monitoring but reduced the upper boundary on 90% prediction intervals for cumulative infections when either was removed. Thus, targeted sampling designs for surveillance testing may mitigate worst-case outcomes when other interventions are less effective. The results’ implications for future EIDs are discussed.</jats:p
