Anthropometric and physiologic assessment in sleep apnoea patients regarding body fat distribution
Background: Obstructive sleep apnoea (OSA) is characterised by repeated episodes of pauses in breathing during sleep due to obstruction of the upper airway that result in transient hypoxaemia, sleep fragmentation and long-term cardiovascular disease. The most common risk factors for OSA include: obesity, age over 50 and neck circumference of more than 41 cm for females and more than 43 cm in males. Sleep apnoea is more common in men than in women. The aim of the conducted research was to evaluate relations between the anthropometric features connected with adipose tissue distribution and the severity of OSA.
Materials and methods: The study was carried out on 180 patients (144 males and 36 females) diagnosed with OSA syndrome. The standard sleep parameters obtained from night polysomnography as well as skin-fat fold thickness and neck circumference and waist-to-hip ratio were analysed. Statistical analysis was performed using STATISTICA 10.
Results: It was stated that anthropometric parameters connected with the accumulation of adipose tissue in upper body were significantly related to severity of OSA in males (p ≤ 0.05). Body mass index (BMI) was significantly correlated with severity of OSA in females (p ≤ 0.05).
Conclusions: In males, there is a connection between the severity of OSA, BMI and a higher accumulation of adipose tissue in upper part of the body measured by neck circumference and shoulder thickness of skin-fat folds, whereas in females only by BMI
Estimation of age at death: examination of variation in cortical bone histology within the human clavicle
Background: Continuously, numerous human remains of unknown identity are revealed all over the world. One of the elements of the identification process may be a proper assessment of a histological section of bone fragments in order to answer questions related to the age of the subject. The aim of the study was to define an optimum bone fragment to obtain samples for histological examination.
Materials and methods: The study material consisted of fragments of shafts of left clavicles taken from 39 males and 25 females (aged 22–86). The clavicles came from autopsies conducted between 2005 and 2011 at the Department of Forensic Medicine of Poznan and the Bialystok Medical University. The following were taken into account while estimating the age of the bone remains: clavicle length (CL), clavicle width (CW), clavicle thickness (CT), number of osteons in the field of vision (ON), number of osteons with the Haversian canal of more than 70 μm (HC > 70 μm), average diameter of the Haversian canals (avg. ØHC), area occupied by interstitial lamellae (ILA %), area occupied by osteons (OA %), area occupied by fragments-remnants of osteons remain as irregular arcs of lamellar fragments (OFA %), average thickness of outer circumferential lamellae (avg. OCL, μm), the relation of osteons with the Haversian canal of more than 70 μm in diameter to the total number of osteons (HC > 70 μm, %), at p < 0.00001. The age of the bone remains was estimated using univariate linear regression function.
Results: It was determined that the best place for sampling the osseous tissue for the analysis was the shaft of the clavicle. It was stated than the number of osteons with a large diameter increased with age. The relation of osteons with the Haversian canal of more than 70 μm in diameter to the total number of osteons (HC > 70 μm, %). The level of statistical significant was p < 0.00001. All analysed microscopic features of the osseous tissue showed significant statistical changes occurring with age.
Conclusions: The exact method for preparing osseous tissue for a microscopic analysis to determine the age of the remains is the preparation of histological sections, as the structure of the osseous tissue does not change while processing the material and the time of preparations is relatively short (7–8 days). The best predictors of age with the use of the function of univariate linear regression were: the diameter of Haversian canal, the number of osteons with Haversian canal of more than 70 μm in diameter, the relation of osteons with Haversian canal bigger than 70 μm in diameter to the total number of osteons as well as fragments of secondary osteons
Diagnosis and treatment of thyroid cancer in children in the multicenter analysis in Poland for PPGGL
Introduction: Differentiated thyroid carcinoma (DTC) in
children presents different biological behavior in comparison
to adults. Authors presents preliminary results of multicenter
analysis concerning incidence, diagnostics and treatment
of DTC in children.
Material and methods: The study is a retrospective analysis
of 107 pediatric patients from 14 academic centers based
on the data from 2000 to 2005 obtained by questionnaire in
hospitals involved in the treatment of DTC in children.
Results: Papillary thyroid cancer was diagnosed in 83 children,
follicular thyroid cancer in 10 children and medullary
thyroid cancer in 14 children. Incidence of DTC in children
was estimated between 18 and 23 cases per year. The biggest
group of patients consisted of children between 11 and
15 years of age, with girls to boys ratio 3.3 : 1. Clinically DTC
in children presented most often as solitary thyroid nodule.
Cervical lymphadenopathy was observed in 42% of patients.
Intraoperative verification indicated metastatic nodes
in 50% of children. Low stage DTC predominated (T1
in 36% and T2 in 26% of children). One step surgery was performed in 65% of children with DTC, two step surgery
in 25% of patients. I131 therapy was undertaken in 80% of
children. Lung metastases were indicated in post therapeutic
studies in 14% of children with DTC. Prophylactic thyroidectomies
were performed in 79% of children in the group
of patients with MTC and RET gene mutations.
Conclusions: The necessity of introduction of unified therapeutic
standard in children with DTC in Poland is underlined.Wstęp: Zróżnicowane raki tarczycy (DTC, differentiated thyroid
carcinoma) występują u dzieci rzadko. Większość przypadków
wykrywanych jest w wieku 11-17 lat. W odróżnieniu
od dorosłych DTC u dzieci prezentują odmienne zachowanie
biologiczne. Mała liczba przypadków DTC
w poszczególnych ośrodkach oraz względnie łagodny ich
przebieg utrudniają ocenę występowania i leczenia DTC
u dzieci w Polsce, uzależniając ją od wysiłków włożonych
w uzyskanie rzetelnych danych. Autorzy przedstawiają
wstępne wyniki analizy wieloośrodkowej dotyczące występowania,
diagnostyki i leczenia DTC u dzieci.
Materiał i metody: Podjęte badania są retrospektywną analizą
obejmującą lata 2000-2005, opartą na danych z historii
chorób uzyskanych z ankiet rozesłanych do ośrodków dla
dzieci i dorosłych podejmujących leczenie DTC. Do analizy
zgłoszono 107 pacjentów z 14 ośrodków akademickich
w Polsce. Analizie poddano wiek i płeć dzieci z DTC, wielkość
i lokalizację zmian w tarczycy, sposoby rozpoznawania
DTC, rodzaje i zakres wykonywanych zabiegów operacyjnych
oraz leczenie uzupełniające izotopem J131.
Wyniki: Raka brodawkowatego stwierdzono u 83 dzieci,
pęcherzykowego u 10 dzieci, a rdzeniastego u 14 dzieci. Częstość
występowania DTC u dzieci w Polsce wahała się między
18 a 23 przypadkami rocznie. W województwach: mazowieckim
i połączonych wielkopolskim i lubuskim wykazano
w okresie 2000-2005 wyższą (24 i 25) częstość występowania
DTC, w pozostałych województwach wykazywano
od 2 do 10 przypadków DTC. Największą grupę pacjentów
stanowiły dzieci w wieku 11-15 lat, a stosunek dziewcząt do chłopców wynosił 3,3 : 1. Klinicznie DTC prezentowały
się najczęściej jako pojedyncze guzki tarczycy. Limfadenopatię
szyjną w badaniu klinicznym stwierdzono
u 42% pacjentów, a śródoperacyjnie u 50% dzieci. U większości
pacjentów dominowały niższe stopnie zaawansowania
DTC (T1 u 36% i T2 u 26% dzieci). Operacje jednoetapowe
wykonano u 65% dzieci, operacje dwuetapowe u 25%
dzieci, a profilaktyczne tyreoidektomie u 79% dzieci z grupy
pacjentów z rakiem rdzeniastym tarczycy (MTC, medullary
thyroid cancinoma) i mutacją genu Ret. Leczenie izotopowe
J131 podjęto u 80% dzieci. Przerzuty do płuc w scyntygrafii
poterapeutycznej wykazano u 14% dzieci z DTC.
Wnioski: We wnioskach podkreśla się konieczność wdrożenia
na terenie całego kraju ujednoliconego i ocenianego
na podstawie obiektywnych przesłanek sposobu postępowania
z dziećmi z DTC
Pęknięcie serca u pacjentów z zawałem serca z uniesieniem odcinka ST. Wpływ skuteczności leczenia inwazyjnego
Background: Cardiac rupture is a rare but potentially lethal complication of acute myocardial infarction with ST-elevation
(STEMI). Primary percutaneous coronary intervention (pPCI) is a preferable treatment method of acute STEMI. Reperfusion
at vascular and myocardial levels may be the key parameters determining probability of cardiac rupture.
Aim: To analyse the relationship between reperfusion parameters and cardiac rupture occurrence in a group of patients with
STEMI treated with pPCI.
Methods: Twenty three patients with cardiac rupture were selected out of 2800 patients with acute STEMI hospitalised and
treated with pPCI from 2000 to 2007. Free wall or interventricular septum rupture was diagnosed on echocardiography or
autopsy. The control group consisted of 255 patients with STEMI and pPCI treatment, but without cardiac rupture. The TIMI
flow score was used to assess blood flow in an infarct-related artery (IRA). Myocardial perfusion was evaluated with the use of
the myocardial blush grade (MBG) score.
Results: Female gender accounted for 47.8% of patients with cardiac rupture. Mean age was 72.9 ± 4.8 years for women
and 64.3 ± 11.5 years for men. In 12 (52%) patients anterior descending artery was the IRA. Before the pPCI, the average
blood flow in IRA was significantly lower in patients with subsequent cardiac rupture (0.41 ± 0.59) than in the reference
group (0.81 ± 1.15; p < 0.05), and remained lower after pPCI (1.96 ± 0.93 in comparison to 2.93 ± 0.36; p < 0.05).
Adequate blood flow (TIMI 3) was achieved only in 30.4% (7) of patients with cardiac rupture and in 95.3% (243) of the
control group (p < 0.05). Myocardial tissue perfusion, assessed by MBG, was also lower (0.76 ± 1 vs 1.92 ± 1.13; p < 0.05).
In-hospital mortality in patients with cardiac rupture reached 56.5% (13 subjects) compared to 3 (1.2%) patients in the
reference group (p < 0.05). Multivariable analysis confirmed independent effects of lower TIMI and MBG after PCI as well as
female gender on the occurrence of cardiac rupture.
Conclusions: Poorer blood flow in IRA and worse tissue microvascular perfusion after pPCI are important risk factors of
cardiac rupture occurrence in patients with STEMI.
Kardiol Pol 2011; 69, 8: 795–800Wstęp: Pęknięcie serca jest rzadkim powikłaniem zawału serca z uniesieniem odcinka ST (STEMI), jednak stanowi jedną
z głównych przyczyn śmiertelności wewnątrzszpitalnej. Dzięki rozpowszechnieniu pierwotnej przezskórnej interwencji wieńcowej
(pPCI) jako metody z wyboru w leczeniu ostrej fazy zawału, częstość pęknięć serca zmalała. Brakuje jednak dokładnych
opracowań czynników ryzyka wystąpienia pęknięcia serca w populacji chorych leczonych interwencyjnie. Wydaje się,
że kluczowym parametrem mogącym wpływać na częstość występowania tego powikłania jest skuteczność reperfuzji zarówno
na poziomie naczynia odpowiedzialnego za zawał, jak i mikrokrążenia.
Cel: Celem pracy była ocena relacji między wybranymi parametrami skuteczności przezskórnej angioplastyki wieńcowej
a wystąpieniem pęknięcia serca u pacjentów ze STEMI leczonych interwencyjnie.
Metody: W populacji ok. 2800 chorych ze STEMI, hospitalizowanych w latach 2000–2007, zidentyfikowano 23 pacjentów
leczonych pPCI z rozpoznanym następnie pęknięciem ściany wolnej lewej komory lub przegrody międzykomorowej. Diagnozę
ustalono na podstawie autopsji lub w przezklatkowym badaniu echokardiograficznym. Populację odniesienia stanowiło
255 losowo dobranych pacjentów ze STEMI leczonych pPCI, u których nie wystąpiło pęknięcie serca, reprezentatywnych
dla ogólnej populacji osób ze STEMI leczonych pPCI. Przepływ w naczyniu odpowiedzialnym za zawał przed i po pPCI
wyrażono za pomocą skali TIMI flow grade, a perfuzję miokardium oceniano na podstawie myocardial blush grade (MBG).
Analizę statystyczną wykonano za pomocą testów t-studenta oraz x2.
Wyniki: Wśród pacjentów z pęknięciem serca 47,8% populacji stanowiły kobiety. Średni wiek w tej grupie wynosił 72,9 ± 4,8 roku
wśród kobiet i 64,3 ± 11,5 roku wśród mężczyzn. W koronarografii wykazano, że w 12 (52%) przypadkach naczyniem odpowiedzialnym
za zawał była gałąź przednia zstępująca lewej tętnicy wieńcowej. Średni przepływ w tętnicy wieńcowej przed zabiegiem
był istotnie statystycznie niższy w populacji chorych z pęknięciem (odpowiednio 0,41 ± 0,59 w porównaniu z grupą
referencyjną 0,81 ± 1,15; p < 0,05). Po wykonaniu pPCI przepływ był nadal istotnie niższy w grupie badanej (odpowiednio
1,96 ± 0,93 v. 2,93 ± 0,36; p < 0,05). Pełen przepływ w naczyniu odpowiedzialnym za zawał (TIMI 3) osiągnięto jedynie
u 30,4% (7) osób z grupy z pęknięciem i u 95,3% (243) z grupy odniesienia (p < 0,05). Perfuzja mięśnia zaopatrywanego przez
tętnicę odpowiedzialną za zawał oceniana w skali MBG była również istotnie statystycznie niższa u osób, u których doszło później
do pęknięcia miokardium (odpowiednio 0,76 ± 1 v. 1,92 ± 1,13; p < 0,05). Śmiertelność wewnątrzszpitalna w przypadku
zawału powikłanego pęknięciem wynosiła w populacji leczonej pPCI 56,5% (13 osób), natomiast w grupie referencyjnej zmarły
jedynie 3 osoby (1,2%; p < 0,05). Analiza wieloczynnikowa wykazała, że niezależnymi czynnikami wpływającymi na wystąpienie
pęknięcia serca są: gorszy przepływ TIMI w naczyniu, słabsza perfuzja tkankowa MBG po interwencji wieńcowej i płeć żeńska.
Wnioski: Zajęcie gałęzi przedniej zstępującej, niepełny przepływ w naczyniu odpowiedzialnym za zawał i upośledzenie
mikrokrążenia po interwencji wieńcowej są istotnymi angiograficznymi czynnikami ryzyka wystąpienia pęknięcia mięśnia
sercowego u chorych ze STEMI leczonych pierwotną interwencją wieńcową.

Dataset of the next-generation sequencing of variable 16S rRNA from bacteria and ITS2 regions from fungi and plants derived from honeybees kept under anthropogenic landscapes
Forager Apis melliefera honeybees were collected from four localities located in Europe, i.e.: London, UK; Athens, Greece; Marchamalo, Spain and Lublin, Poland. Furthermore, from Asia we have collected A. mellifera as well as A. cerana foragers form Chiang Mai in Thailand We used next generation sequencing (NGS) to analyse the 16S rRNA bacterial gene amplicons based on the V3-V4 region and the ITS2 region from fungi and plants derived from honeybee samples. Amplicon libraries, were prepared using the 16S Metagenomic Sequencing Library Preparation, Preparing 16S Ribosomal RNA Gene Amplicons for the Illumina MiSeq System (Illumina®) protocol. NGS raw data are available at https://www.ncbi.nlm.nih.gov/bioproject/PRJNA686953. Furthermore, isolated DNA was used as the template for screening pathogens: Nosema apis, N. ceranae, N. bombi, tracheal mite (Acarapis woodi), any organism in the parasitic order Trypanosomatida, including Crithidia spp. (i.e., Crithidia mellificae), neogregarines including Mattesia and Apicystis spp. (i.e., Apicistis bombi). The presented data can be used to compare the metagenomic samples from different honeybee population all over the world. A higher load of fungi, and bacteria groups such as: Firmicutes (Lactobacillus); γ-proteobacteria, Neisseriaceae, and other unidentified bacteria was observed for Nosema cearana and neogregarines infected honeybees. Healthy honeybees had a higher load of plant pollens, and bacteria groups such as: Orbales, Gilliamella, Snodgrassella, and Enterobacteriaceae.
