A Comparison of Osteoblast Cell Proliferation and Osteocalcin Expression in Cuttlefish Bone and Bovine Bone Xenograft
Background: Cuttlefish bone Xenograft, calcium phosphate (CaP)-based biomaterial graft, offers an alternative and has been accepted for osteoconductive and probable osteo-inductive attributes. This study aims to compare the bone healing potential between the bovine-derived (BHA) and cuttlefish bone-derived (CHA).Materials and Methods: The study compared osteoblast cell proliferation of 27 New Zealand rabbits in 2.5 mm bone defect made in the femoral bone. The samples were divided into three groups, which were control, BHA and CHA group. The chemical and physical characteristics of BHA and CHA were determined for the content of hydroxyapatite by FourierTransform Infrared Spectroscopy (FTIR) and X-Ray Diffraction (XRD), then tested by Scanning Electron Microscopy (SEM) to evaluate the porosity. In the end of the second week, histopathologic and immunohistochemistry examinations were performed to evaluate the amount of osteoblast and osteocalcin expression.Results: The FTIR, XRD and SEM analysis showed both BHA and CHA samples were hydroxyapatite according to Joint Committee on Powder Diffraction Standards (JCPDS). The CHA was significantly higher (297.22±19.772) compared to BHA (258.22±30.926) and control (131.67±34.213). Osteocalcin expression in CHA (7.82±2.230) compared to BHA (6.09±3.724) and control (4.07±3.606), was not significant (p>0.05). Conclusion: CHA group has the highest osteoblast cell proliferation and osteocalcin expression, meaning has a good potential as future source of bone graft.Keywords: cuttlefish bone, bovine, bone graft, osteoblast cel
Effect of Platelet-Rich Plasma and Amniotic Membrane in Patients with Rotator Cuff Repair
Rotator cuff disorders are the most common source of shoulder problems, ranging from mild strain to massive tears. Platelet-rich plasma (PRP), an autologous blood with platelets concentration above baseline values represents a source of multiple growth factors that promotes tissue repair. This review examines the potential of using PRP to augment rotator cuff repair. Reporting 4 patients with impingement syndrome and supraspinatus tear who underwent decompression acromioplasty and supraspinatus repair augmented with platelet-rich plasma and amniotic membrane. An evaluation was made 3-24 months postoperative using Shoulder Pain and Disability Index (SPADI). Average preoperative pain score is 64%, disability score 54.58%, and total score 58.19%. Average postoperative pain score is 0%, disability score 0.42%, and total score 0.26% (Minimum Detectable Change at 90% confidence for pain score is 18%, disability score 13%, and total score 11%). This result is consequent with research by Luoay Fallouh, stating that improvement is caused by growth factor effects in platelet-rich plasma which promotes soft tissue healing. It can be concluded that platelet-rich plasma and amniotic membrane have promising effects to enhance soft tissue healing in patients with rotator cuff syndrome. Shoulder function is restored with no limitation on daily activity and pain is no longer present
PERBANDINGAN JUMLAH OSTEOBLAST DAN ALKALINE PHOSPHATASE PADA SMALL BONE DEFECT FEMUR NEW ZEALAND WHITE RABBIT YANG DIBERI CUTTLEFISH ATAU BOVINE BONE XENOGRAFT
Background: Defek tulang akibat trauma, tumor, kelainan kongenital, dan infeksi
masih merupakan problem besar di bidang orthopaedi dan traumatologi. Sering
kali dalam penanganannya diperlukan bone substitute. Tujuan studi ini yaitu
mengevaluasi HA-cuttlefish sebagai bone graft alternatif.
Metode: Studi ini merupakan uji eksperimental yang dilakukan di Tropical
Disease Center Fakultas Kedokteran Universitas Airlangga setelah mendapatkan
ethical clearance dari komite etik RSUD Dr. Soetomo. Pada penelitian ini
dilakukan pembuatan defek sebesar 2,5 mm pada metafisis os femur kelinci New
Zealand dengan 3 kelompok yaitu kontrol, HA-bovine, dan HA-cuttlefish. Pada
akhir minggu kedua dilakukan pengumpulan sample dan pemeriksaan
histopatologi untuk menilai jumlah osteoblast dan ekspresi osteocalcin. Data
dianalisis dengan uji Kolmogorov-Smirnov. Kemudian dilakukan uji komparasi
menggunakan uji Kruskal Wallis dan Mann-Whitney.
Hasil: Hasil uji post hoc osteoblast dengan Mann-Whitney didapatkan perbedaan
bermakna antara kelompok HA-cuttlefish dibandingkan dengan HA-bovine
(p=0.006), HA-cuttlefish dibandingkan dengan kontrol (p=0.000), serta HA-bovine
dibandingkan dengan kontrol (p=0.000). Sedangkan untuk uji post hoc
osteocalcin didapatkan perbedaan yang bermakna antara kelompok HA-cuttlefish
dibandingkan dengan HA-bovine (p=0.040). Namun tidak didapatkan perbedaan
yang bermakna antara HA-cuttlefish dengan kontrol (p=0.546), serta HA-bovine
dengan kontrol (p=0.222).
Kesimpulan: HA-cuttlefish memiliki jumlah osteoblast dan ekspresi osteocalcin
terbanyak jika dibandingkan dengan kelompok HA-bovine dan kontrol. Namun
dari hasil uji post hoc dapat diasumsikan bahwa meskipun jumlah osteoblast lebih
banyak, namun tingkat maturitasnya sama
