    Subclinical Myocardial Necrosis and Cardiovascular Risk in Stable Patients Undergoing Elective Cardiac Evaluation

    Objective— The presence of subclinical myocardial necrosis as a prodrome to longer-term adverse cardiac event risk has been debated. The debate has focused predominantly within patients with acute coronary syndrome, and on issues of troponin assay variability and accuracy of detection, rather than on the clinical significance of the presence of subclinical myocardial necrosis (ie, “troponin leak”) within stable cardiac patients. Herein, we examine the relationship between different degrees of subclinical myocardial necrosis and long-term adverse clinical outcomes within a stable cardiac patient population with essentially normal renal function. Methods and Results— Sequential consenting patients (N=3828; median creatinine clearance, 100 mL/min/1.73m2) undergoing elective diagnostic coronary angiography with cardiac troponin I (cTnI) levels below the diagnostic cut-off for defining myocardial infarction (\u3c0.03 ng/mL) were evaluated. The relationship of subclinical myocardial necrosis with incident major adverse cardiovascular events (defined as any death, myocardial infarction, or stroke) over 3-year follow-up was examined. “Probable” (cTnI 0.001–0.008 ng/mL) and “definite” (cTnI 0.009–0.029 ng/mL) subclinical myocardial necrosis were observed frequently within the cohort (34% and 18%, respectively). A linear relationship was observed between the magnitude of subclinical myocardial necrosis and risk of 3-year incident major adverse cardiovascular events, particularly in those with cTnI 0.009 ng/mL or higher (hazard ratio, 3.00; 95% confidence interval, 2.4–3.8), even after adjustment for traditional risk factors, C-reactive protein, and creatinine clearance. The presence of subclinical myocardial necrosis was associated with elevations in acute phase proteins (C-reactive protein, ceruloplasmin; P\u3c0.01 each) and reduction in systemic antioxidant enzyme activities (arylesterase; P\u3c0.01) but showed no significant associations with multiple specific measures of oxidant stress, and showed borderline associations with myeloperoxidase, a marker of leukocyte activation. Conclusion— In stable cardiology patients, prodromal subclinical myocardial necrosis is associated with substantially higher long-term risk for major adverse cardiovascular events. The underlying mechanisms contributing to this minimal troponin leak phenomenon warrants further investigation

    The manipulation of midsole properties to alter impact characteristics in walking

    The midsole of footwear can provide an opportunity to attenuate the impact at the foot-ground interface. The present study was undertaken to quantify impact in walking in different footwear midsoles, comparing footwear thickness and hardness variations. Methods: Footbed thickness (28-41 mm) and hardness (30-55 Shore A) were varied independently in 7 flip-flops. Thirteen subjects walked in the footwear variations on a level walkway in the gait laboratory as lower limb kinematics, vertical ground reaction force and peak positive axial tibial acceleration were quantified. Peak magnitude and time of the acceleration were quantified and the heel-strike transient was characterised for comparison between conditions with a repeated-measures ANOVA. Thickness and hardness variations were also compared using a drop-test protocol to replicate walking. Results: Lower limb joint angles did not vary at heel-strike, however, a faster vertical heel-velocity was recorded in the softer midsoles (e.g. 55 Shore A = -0.294±0.055, 30 Shore A= -0.328±0.052, p<.001). Varying the hardness of the midsoles also significantly altered tibial acceleration and force variables, however limited significant differences existed between the thickness variations in walking. Increasing the hardness of the heel section of the footwear increased the peak positive axial tibial acceleration values, for example increasing Shore A from 30 to 40 resulted in a 35% increase in this variable. Concurrently, the occurrence of heel-strike transients increased from 5.8% in the 30 Shore A condition to 22.5%, 46.7% and 71.7% of all trials in the 40, 47 and 55 Shore A conditions respectively. The drop-test protocol replicated the differences evident in the walking protocol despite magnitudes being elevated. Conclusion: Modifying midsole properties of flip-flop footwear, particularly hardness, alters the gait kinematics and the shock experienced by the wearer in walking. This may pose benefits in terms of comfort and reduction in loading to the lower limb, however the influence on foot motion at initial contact and footwear longevity should be further quantified

    Relationship between psychological and biological factors and physical activity and exercise behaviour in Filipino students

    The aim of the present study was threefold. Firstly, it investigated whether a general measure or specific measure of motivational orientation was better in describing the relationship between motivation and exercise behaviour. Secondly, it examined the relationship between the four most popular indirect methods of body composition assessment and physical activity and exercise patterns. Thirdly, the interaction between motivation and body composition on physical activity and exercise behaviour was explored in a sample of 275 Filipino male and female students. Males were found to have higher levels of exercise whereas females had higher levels of physical activity. Furthermore, general self-motivation together with body weight and percentage body fat were found to be the best predictor of exercise behaviour whereas the tension/pressure subscale of the ‘Intrinsic Motivation Inventory’ (IMI) was the best predictor of levels of physical activity. However, significant gender differences were observed. That is, for the males only self-motivation and for the females only body weight and BMI predicted exercise behaviour. Also, tension/pressure predicted physical activity levels for the females but not the males. No inverse relationship was found between the four body composition measures and exercise and physical activity behaviour. The results support the notion that the psychobiological approach might be particularly relevant for high intensity exercise situations but also highlights some important gender differences. Finally, the results of this study emphasise the need for more cross-cultural research
