    Discourse Relations and Connectives in Higher Text Structure

    The present article investigates possibilities and limits of local (shallow) analysis of discourse coherence with respect to the phenomena of global coherence and higher composition of texts. We study corpora annotated with local discourse relations in Czech and partly in English to try and find clues in the local annotation indicating a higher discourse structure. First, we classify patterns of subsequent or overlapping pairs of local relations, and hierarchies formed by nested local relations. Special attention is then given to relations crossing paragraph boundaries and their semantic types, and to paragraph-initial discourse connectives. In the third part, we examine situations in which annotators incline to marking a large argument (larger than one sentence) of a discourse relation even with a minimality principle annotation rule in place. Our analyses bring (i) new linguistic insights regarding coherence signals in local and higher contexts, e.g. detection and description of hierarchies of local discourse relations up to 5 levels in Czech and English, description of distribution differences in semantic types in cross-paragraph and other settings, identification of Czech connectives only typical for higher structures, or the detection of prevalence of large left-sided arguments in locally annotated data; (ii) as another type of contribution, some new reflections on methodologies of the approaches under scrutiny

    Important Aspects Influencing Delivery of Serious News in Pediatric Oncology: A Scoping Review

    Delivering serious news presents a major challenge for clinical practice in pediatric oncology due to the complexity of the communication process and a number of aspects that influence how the serious news is delivered and received. This study aims to review and explore the aspects influencing the delivery of serious news in pediatric oncology from the perspective of physicians, parents, siblings and patients themselves. The MEDLINE, Embase, Scopus, Cochrane Library, PsycInfo and Medvik databases were systematically searched for relevant articles published from 1990 to 2017. Following the Preferred Reporting Items for Systematic Review and Meta-analysis extension for scoping reviews (PRISMA-ScR) guidelines, 36 original papers were included. Identified aspects of communication were categorized into six thematic groups: initial setting, physician’s approach, information exchange, parental role, illness related aspects and age of the ill child. The importance of the aspects is perceived differently by parents, patients, siblings and physicians. This scoping review highlights that delivering serious news requires an individualized approach towards the patient and the family. Ten key objectives built upon the results of the literature review offer guidance for daily clinical practice in communication with pediatric patients and their families

    Experimental Combined Immunotherapy of Tumours with Major Histocompatibility Complex Class I Downregulation

    Combined immunotherapy constitutes a novel, advanced strategy in cancer treatment. In this study, we investigated immunotherapy in the mouse TC-1/A9 model of human papillomavirus type 16 (HPV16)-associated tumors characterized by major histocompatibility complex class I (MHC-I) downregulation. We found that the induction of a significant anti-tumor response required a combination of DNA vaccination with the administration of an adjuvant, either the synthetic oligodeoxynucleotide ODN1826, carrying immunostimulatory CpG motifs, or α-galactosylceramide (α-GalCer). The most profound anti-tumor effect was achieved when these adjuvants were applied in a mix with a one-week delay relative to DNA immunization. Combined immunotherapy induced tumor infiltration with various subsets of immune cells contributing to tumor regression, of which cluster of differentiation (CD) 8+ T cells were the predominant subpopulation. In contrast, the numbers of tumor-associated macrophages (TAMs) were not markedly increased after immunotherapy but in vivo and in vitro results showed that they could be repolarized to an anti-tumor M1 phenotype. A blockade of T cell immunoglobulin and mucin-domain containing-3 (Tim-3) immune checkpoint had a negligible effect on anti-tumor immunity and TAMs repolarization.
