The usefulness of CBF brain SPECT in forensic medicine: the civil law code cases. A description of four cases
BACKGROUND: The aim of this report was to assess the usefulness
of cerebral blood flow (CBF) scanning utilising the
SPECT technique in forensic medicine cases in the area of civil
law cases.
MATERIAL AND METHODS: CBF SPECT scanning was performed
in four patients utilising 99mTc-ECD and a triple-head gammacamera.
In the analysis both the asymmetry index and cerebellar
normalisation were applied. Reference values were obtained
by studying 30 healthy volunteers.
RESULTS: In those cases CBF SPECT scanning played an important
role in forensic argument. It influenced the sentence
and the amount of financial compensation.
CONCLUSIONS: CBF SPECT scanning may provide valuable
information in forensic medicine argument in civil law cases,
but only when taken together with psychometric tests and other
neuroimaging methods (CT, MRI). The value of CBF SPECT
scanning alone may be limited in judicial proceedings
Congenital abdominal dumbbell fashion neuroblastoma with invasion of spinal canal detected by ultrasonography : case report
Background: A case of congenital abdominal dumbbell fashion neuroblastoma with invasion of the spinal canal detected by ultrasonography (US) is presented. Case report: A 3-week-old male neonate was admitted to the hospital with a palpable mass in the left lumbar region. Ultrasound examination was performed on the same day. It disclosed a pathologic mass filling the left side of the retroperitoneal space - displacing laterally and inferiorly the left kidney. The second part of the tumor was located above the Gerot's fascia in the muscles and infiltrated the spinal canal via lumbar intervertebral foramens. Computer tomography scanning confirmed the presence of solid masses in these locations. Urinary excretion of vanillin-mandelic acid (VMA) was within normal range, ferritin level was elevated (447 μg/ml). Bone scintigraphy showed metastases to the left clavicle. There were no changes in bone marrow. Diagnosis of an undifferentiated malignant neuroblastoma was established in histopathological examination. Wole guzowate olbrzymie — ambulatoryjne leczenie radiojodem
Introduction: 131-I treatment of nodular, especially nontoxic, goitre is still reserved mainly for elderly patients, whose numerous concomitant diseases disqualify them from surgery. Therapy often involves isolation and is available only in selected centres, which may be located far from some patients’ places of residence, which is inconvenient for elderly people. The aim of the study was to assess the effectiveness of outpatient fractionated 131-I treatment of patients with large nodular goitres, as well as to evaluate complications and the factors affecting treatment results.
Material and methods: The study included 35 patients with a large nodular goitre. Thyroid volume and treatment results were evaluated using US and CT neck examination.
Results: Mean thyroid volume prior to treatment was 104.36 mL (range 36.23–301.09 mL). An average administered 131-I activity was 1806 MBq (range 800–4000). The average reduction of goitre volume was 43.18% (range –17.23–89.66%). Final treatment results correlated with the thyroid size reduction obtained three months after treatment (r = 0.74; p = 0.001). Symptoms of transient hyperthyroidism were observed in 8.57% of patients, in 5.4% Graves disease was induced (including severe Graves’ orbitopathy in 2.7%), and in 2.86% TRAb increase without development of hyperthyroidism was observed. The treatment results were not influenced by initial thyroid volume (r = –0.01; p = 0.95). An increase in thyroid volume during the treatment was reported in 20% of patients, with a mean increase of 22.3% (range 0.63–55.03%). Post-treatment hypothyroidism was diagnosed in 42.9% of patients. One patient was diagnosed with salivary gland damage.
Conclusions: Fractionated 131-I treatment of large nodular goitres is an effective method, the results of which are comparable to those obtained from the administration of one-time high doses of radioiodine. (Endokrynol Pol 2015; 66 (4): 301–307)
Wstęp: Leczenie 131-I wola guzowatego olbrzymiego, szczególnie obojętnego, nadal zarezerwowane jest głównie dla pacjentów w starszym wieku, z licznymi chorobami towarzyszącymi dyskwalifikującymi ich z leczenia zabiegowego. Terapia, szczególnie w przypadku dużych rozmiarów wola wiąże się z izolacją po podaniu 131-I, a leczenie jest możliwe tylko w wybranych ośrodkach często oddalonych od miejsca zamieszkania, co jest utrudnieniem dla osób starszych. Celem pracy była ocena skuteczności leczenia wola olbrzymiego 131-I w warunkach ambulatoryjnych dawkami frakcjonowanymi z analizą czynników wpływających na efekt terapii oraz oceną jej powikłań.
Materiały i metody: Analizą objęto 35 osób z wolem guzowatym olbrzymim. Ocenę objętości tarczycy i efektów leczenia wykonano z użyciem badań USG oraz CT szyi.
Wyniki: Średnia objętość wola przed leczeniem wynosiła 104,36 mL (zakres 36,23–301,09), a średnia podana aktywność 131-I to 1806 MBq (zakres 800–4000). Uzyskano redukcję objętości wola średnio o 43,18% (zakres –17,23–89,66%). Ostateczny efekt leczenia korelował z poziomem redukcji uzyskanym po 3 miesiącach leczenia (r = 0,74; p = 0,0010). Efekt leczenia nie zależał od wyjściowej wielkości wola (r = –0,01; p = 0,95). Objawy przejściowej nadczynności tarczycy obserwowano u 8,57% leczonych, u 5,4% wyindukowano chorobę Gravesa-Basedowa (u 2,7% ciężką orbitopatię) u 2,86% zaobserwowano wzrost miana przeciwciał przeciw receptorowi TSH bez objawów nadczynności tarczycy. Przejściowo powiększenie tarczycy w trakcie leczenia obserwowano u 20% leczonych, a objętość gruczołu wzrosła średnio o 22,3% (zakres 0,63–55,03). Niedoczynność tarczycy po zakończeniu leczenia stwierdzono u 42,9% pacjentów. W jednym przypadku obserwowano, nieopisywane wcześniej po stosowaniu małych dawek 131-I, popromienne uszkodzenie ślinianek.
Wnioski: Leczenie wola guzowatego olbrzymiego frakcjonowanymi dawkami 131-I jest leczeniem skutecznym, o efektach porównywalnych do stosowania dużych jednorazowych dawek radiojodu. (Endokrynol Pol 2015; 66 (4): 301–307)
Oncogenic osteomalacia — detection of the tumour site upon physical examination
Ostemalacja onkogenna jest rzadkim zespołem paranowotworowym wynikającym z obecności guza produkującego fosfatoniny. Pacjenci bez właściwej diagnozy i leczenia cierpią na silne bóle kości, nawracające patologiczne złamania i osłabienie mięśni przez miesiące, a nawet lata. Guz jest zwykle niewielki i może występować w różnych miejscach. Najczęstszym typem nowotworu powodującego onkogeniczną osteomalację jest wariant mezenchymalny w tkankach miękkich. Nasze badanie ma na celu przedstawianie przypadku klinicznego 50-letniej pacjentki z onkogeniczną osteomalacją u której w badaniu fizykalnym znaleźliśmy guz w tkance podskórnej okolicy prawej łopatki, co skróciło czas wdrożenia odpowiedniego leczenia do jednego tygodnia. Na tym przykładzie chcielibyśmy podkreślić rolę badania fizykalnego jako ważnego etapu w procesie diagnostycznym tej rzadkiej choroby i zalecić usuwanie łatwo dostępnych guzów bez przeprowadzania zaawansowanych badań obrazowych jako szybkie, skuteczne i korzystne ekonomicznie postępowanie.Not required for Clinical Vignette
Guz Krukenberga – problem interdyscyplinarny – opis przypadku
Metastatic lesions within the ovary constitute a serious diagnostic problem in daily practice. We present an interesting case of Krukenberg tumor in a woman 13 years after partial gastrectomy due to stomach cancer. Our case confirms that every woman with history of cancer should remain under gynecological control. It is important due to a high risk of metastatic changes localized in the ovaries, regardless of the time elapsed since the diagnosis of the primary tumor.Ogniska przerzutowe w obrębie jajnika stanowią trudny interdyscyplinarny problem diagnostyczny. Część zmian wykrywana jest przypadkowo, a brak pełnych danych klinicznych może utrudnić rozpoznanie zarówno radiologowi analizującemu badania obrazowe jak również patomorfologowi oceniającemu materiał histopatologiczny. W pracy przedstawiono kazuistyczny przypadek pacjentki z guzem Krukenberga, rozpoznanym trzynaście lat po częściowej gastrektomii z powodu raka gruczołowego żołądka. Potwierdza on konieczność okresowej kontroli ginekologicznej pacjentek z przeszłością nowotworową ze względu na wysokie ryzyko wystąpienia zmian przerzutowych w obrębie jajników, niezależnie od okresu jaki upłynął od rozpoznania nowotworu pierwotnego

The study presented the reference values for thyroid isthmus thickness in prepubertal healthy children. Ultrasound evaluation of the isthmus seems to be very useful in the diagnostic process and monitoring of thyroid diseases, especially the diffuse ones. The authors are not aware of the existence of any other publications on reference values of isthmus depth in children population. In our study, we evaluated isthmus thickness in a healthy population of prepubertal children from a seaside region. A total of 402 healthy children (214 boys and 188 girls) aged 7-12 years, underwent ultrasound examinations of the thyroid. We propose the following maximum values for isthmus depth: from 2.6 mm for boys at the age of 7-9 years to 3.3 mm for those aged 10-12 years and from 2.7 mm to 3.5 mm for girls, respectively

Intestinal malrotation is usually diagnosed in the first year of life; however; the symptoms can intensify only at an older age, causing diagnostic problems. The patients may suffer from different complaints and have a diagnosis completely unrelated to existing intestinal malrotation. We present a case of an 11-year-old female with malnutrition and acute renal failure incidents who had the intestinal malrotation diagnosed with ultrasound examination. The final diagnosis was confirmed with upper and lower gastrointestinal series and later during surgery
