    Autism Spectrum Disorder: Parenting Stress, Family Functioning and Health-Related Quality of Life

    The prevalence of Autism Spectrum Disorder (ASD) is 1 in 110 persons in the U.S. Both parents of children with ASD are under stress that may impact their health-related quality of life (HRQL) (physical and mental health). The purpose of the current study was to explore the relationship of parenting stress, support from family functioning and the HRQL (physical and mental health) of both parents. Female (n = 64) and male (n = 64) parents of children with ASD completed Web-based surveys examining parenting stress, family functioning, and physical and mental health. Results of a Wilcoxon signed-ranks test showed that female parent discrepant (D) scores between “what is” and “should be” family functioning were significantly larger than male parents, p = .002. Results of stepwise linear regression for the male-female partners showed that (1) higher female caregiving stress was related to lower female physical health (p \u3c .001), (2) a higher discrepancy score in family functioning predicted lower mental health (p \u3c .001), accounting for 31% of the variance for females and (3) male parent personal and family life stress (p \u3c .001) and family functioning discrepant (D) score (p \u3c .001) predicted poor mental health, with the discrepancy score accounting for 35% of the variance. These findings suggest that there may be differences in mothers\u27 and fathers\u27 perceptions and expectations about family functioning and this difference needs to be explored and applied when working with families of children with ASD.     Intrinsic Value for the Environmental Pragmatist

    Motivated by the disconnect between the seeming impossibility of resolving deep metaphysical disputes, and the pressing need for environmental action, many environmental philosophers have embraced environmental pragmatism. Environmental pragmatists focus on the practical effects of philosophical arguments. With this as their agenda, environmental pragmatists have consistently endorsed anthropocentrism as the value system for discussing environmental issues, in order to achieve efficacious results. This is based on the notion that appeals to human goods are the best means for motivating humans to action. This essay will show some conceptual and empirical problems with this pretense, to argue that true environmental pragmatists ought to supplement anthropocentric values with nonanthropocentric value of the non-human world

    Lack of Father Involvement in Research on Children with Autism Spectrum Disorder: Maternal Parenting Stress and Family Functioning

    Autism Spectrum Disorder (ASD) has an estimated prevalence of greater than 1% of people in the US. Caring for children with ASD is stressful and challenging for parents. The purpose of the study is to understand the ramifications of the findings of a spouse/father\u27s lack of participation for a study focused on stress and family functioning that attempted to recruit both parents of a child with ASD. The Kruskal-Wallis test compared medians of three groups of mothers of children with ASD in order to assess differences in parenting stress and family functioning discrepancy depending on their marital status and spouse survey participation. There were differences across the groups of mothers of children with ASD for the discrepancy in expectations for help, from the participants’ spouse or relatives, with family tasks, meeting the demands of other work responsibilities, child care, challenging behaviors, and school absences. Mothers of children with ASD are at risk for isolation and stress from negotiating family functions with the fathers of the children. Health care providers can assess for stress and family functioning and may anticipate different needs based on marital status and by father\u27s involvement in decision-making. Autism spectrum disorder (ASD) is a chronic neurodevelopmental disorder of communication, behavior, and socialization, with typical onset occurring before three years of age (American Psychiatric Association, 2000). Four times more common in boys than girls, the prevalence of ASD is increasing and is now estimated to be 1/88 people (\u3e1% of the US population) (Autism and Developmental Disabilities Monitoring Network Surveillance Principal Investigators, 2012). Persons with ASDs present with a range in severity of symptoms with the cause thought to be a combination of genetic predisposition and environmental factors (Hallmayer et al., 2011; Rutter, 2005). Although ASD is not curable, there are several treatment options with a range of evidence to support them (Agency for Healthcare Research and Quality [AHRQ], 2011)

    Building social capital with interprofessional student teams in rural settings: A service-learning model

    Objective: To describe outcomes of a model of service learning in interprofessional learning (IPL) aimed at developing a sustainable model of training that also contributed to service strengthening. Design: A total of 57 semi-structured interviews with key informants and document review exploring the impacts of interprofessional student teams engaged in locally relevant IPL activities. Setting: Six rural towns in South East New South Wales. Participants: Local facilitators, staff of local health and other services, health professionals who supervised the 89 students in 37 IPL teams, and academic and administrative staff. Main outcome measures: Perceived benefits as a consequence of interprofessional, service-learning interventions in these rural towns. Results: Reported outcomes included increased local awareness of a particular issue addressed by the team; improved communication between different health professions; continued use of the team\u27s product or a changed procedure in response to the teams\u27 work; and evidence of improved use of a particular local health service. Conclusions: Given the limited workforce available in rural areas to supervise clinical IPL placements, a service-learning IPL model that aims to build social capital may be a useful educational model

    Evaluation of an Intimate Partner Violence Curriculum in a Pediatric Hospital

    OBJECTIVE. Intimate partner violence harms victims as well as families and communities. Many barriers account for limited intimate partner violence screening by nurses. The purpose of this study was to measure how participation in a curriculum about screening parents for intimate partner violence, at a pediatric hospital, affects a nurse\u27s knowledge, attitudes, behaviors, and self-efficacy for intimate partner violence screening. METHODS. In this interventional, longitudinal study, data were collected before participation in an intimate partner violence screening curriculum, after participation, and 3 months later. The measurement tool was adapted from Maiuro\u27s (2000) Self-efficacy for Screening for Intimate Partner Violence Questionnaire. RESULTS. Sixty-eight pediatric nurses completed all aspects of the study. At baseline, 18 (27%) nurses self-reported seeing a parent with an injury, and of those only 7 (39%) followed up with intimate partner violence screening. Factor analysis was performed on the baseline Self-efficacy for Screening for Intimate Partner Violence Questionnaire by using varimax rotation. Five factors were identified: conflict, fear of offending parent, self-confidence, appropriateness, and attitude. Only fear of offending parent was significantly different from times 1 to 3, indicating that nurses were less fearful after the training. Cronbach\u27s α value for the total questionnaire at baseline was .85. Nurses reported significant improvement (baseline to 3-month follow-up) in several self-efficacy items. CONCLUSIONS. Participation in a 30-minute curriculum on intimate partner violence screening was associated with improvements in self-efficacy and significantly lower fear of offending parents 3 months after training. Nurses also showed improvement in the perception of resources available for nurses to manage intimate partner violence. Thirty-minute hospital-based curriculums that include victim testimonial video and practice role-playing to simulate parent interactions are recommended

    Feeding of soy protein isolate to rats during pregnancy and lactation suppresses formation of aberrant crypt foci in their progeny's colons: interaction of diet with fetal alcohol exposure

    Soy protein isolate (SPI) in the diet may inhibit colon tumorigenesis. We examined azoxymethane (AOM)-induced aberrant crypt foci (ACF) in male rats in relation to lifetime, pre-weaning, or post-weaning dietary exposure to SPI and also within the context of fetal alcohol exposure. Pregnant Sprague Dawley rats were fed AIN-93G diets containing casein (20%, the control diet) or SPI (20%) as the sole protein source starting on gestation day 4 (GD 4). Progeny were weaned on postnatal day (PND) 21 to the same diet as their dams and were fed this diet until termination of the experiment at PND 138. Rats received AOM on PND 89 and 96. Lifetime (GD 4 to PND 138) feeding of SPI led to reduced frequency of ACF with 4 or more crypts in the distal colon. Progeny of dams fed SPI only during pregnancy and lactation or progeny fed SPI only after weaning exhibited similarly reduced frequency of large ACF in distal colon. Number of epithelial cells, in the distal colon, undergoing apoptosis was unaffected by diet. SPI reduced weight gain and adiposity, but these were not correlated with fewer numbers of large ACF. Lifetime SPI exposure similarly inhibited development of large ACF in Sprague Dawley rats whose dams were exposed to ethanol during pregnancy. In summary, feeding of SPI to rat dams during pregnancy and lactation suppresses numbers of large ACF in their progeny, implying a long-term or permanent change elicited by the maternal diet. Moreover, results support the use of ACF as an intermediate endpoint for elucidating effects of SPI and its biochemical constituents in colon cancer prevention in rats

    Controls on Last Glacial Maximum ice extent in the Weddell Sea embayment, Antarctica

    The Weddell Sea sector of the Antarctic Ice Sheet is hypothesized to have made a significant contribution to sea-level rise since the Last Glacial Maximum. Using a numerical flowline model we investigate the controls on grounding line motion across the eastern Weddell Sea and compare our results with field data relating to past ice extent. Specifically, we investigate the influence of changes in ice temperature, accumulation, sea level, ice shelf basal melt, and ice shelf buttressing on the dynamics of the Foundation Ice Stream. We find that ice shelf basal melt plays an important role in controlling grounding line advance, while a reduction in ice shelf buttressing is found to be necessary for grounding line retreat. There are two stable positions for the grounding line under glacial conditions: at the northern margin of Berkner Island and at the continental shelf break. Global mean sea-level contributions associated with these two scenarios are ~50 mm and ~130 mm, respectively. Comparing model results with field evidence from the Pensacola Mountains and the Shackleton Range, we find it unlikely that ice was grounded at the continental shelf break for a prolonged period during the last glacial cycle. However, we cannot rule out a brief advance to this position or a scenario in which the grounding line retreated behind present during deglaciation and has since re-advanced. Better constraints on past ice sheet and ice shelf geometry, ocean temperature, and ocean circulation are needed to reconstruct more robustly past behavior of the Foundation Ice Stream

    Complete Healing of a Giant Wart in a Severely Immune-Compromised Patient with HIV Infection Treated with Acupuncture

    Giant warts are infrequent dermatological viral infections caused by Papillomavirus (HPV) in immune-compromised patients. Treatment may often be difficult and unsatisfactory, either by surgery or cytotoxic agents, because of poor immune control of viral activity in such hosts. Here we report on the case of a patient with advanced and persistent immune suppression caused by HIV disease, who developed a monstrous wart covering the entirety of the radial district of his right hand. He was completely healed after a long treatment with traditional Chinese acupuncture, in spite of minimal immune recovery induced by efficacious antiretroviral therapy. To the best of our knowledge, therefore, the present report may be the first direct clinical evidence that acupuncture may be effective in the treatment of cutaneous warts also in HIV-infected patients

    Temperature and UV-B-insensitive performance in tadpoles of the ornate burrowing frog: an ephemeral pond specialist

    Animals may overcome the challenges of temperature instability through behavioural and physiological mechanisms in response to short-and long-term temperature changes. When ectotherms face the challenge of large diel temperature fluctuations, one strategy may be to reduce the thermal sensitivity of key traits in order to maintain performance across the range of temperatures experienced. Additional stressors may limit the ability of animals to respond to these thermally challenging environments through changes to energy partitioning or interactive effects. Ornate burrowing frog (Platyplectrum ornatum) tadpoles develop in shallow ephemeral pools that experience high diel thermal variability (> 20 degrees C) and can be exposed to high levels of UV-B radiation. Here, we investigated how development in fluctuating versus stable temperature conditions in the presence of high or low UV-B radiation influences thermal tolerance and thermal sensitivity of performance traits of P. ornatum tadpoles. Tadpoles developed in either stable (24 C) or fluctuating temperatures (18-32 degrees C) under high or low UV-B conditions. Tadpoles were tested for upper critical thermal limits, thermal dependence of resting metabolic rate and maximum burst swimming performance. We hypothesised that developmental responses to thermal fluctuations would increase thermal tolerance and reduce thermal dependence of physiological traits, and that trade-offs in the allocation of metabolic resources towards repairing UV-B-induced damage may limit the ability to maintain performance over the full range of temperatures experienced. We found that P. ornatum tadpoles were thermally insensitive for both burst swimming performance, across the range of temperatures tested, and resting metabolic rate at high temperatures independent of developmental conditions. Maintenance of performance led to a trade-off for growth under fluctuating temperatures and UV-B exposure. Temperature treatment and UV-B exposure had an interactive effect on upper critical thermal limits possibly due to the upregulation of the cellular stress response. Thermal independence of key traits may allow P. ornatum tadpoles to maintain performance in the thermal variability inherent in their environment

    The dusty environment of Quasars. Far-IR properties of Optical Quasars

    We present the ISO far-IR photometry of a complete sub-sample of optically selected bright quasars belonging to two complete surveys selected through multicolour (U,B,V,R,I) techniques. The ISOPHOT camera on board of the ISO Satellite was used to target these quasars at wavelengths of 7.3, 11.5, 60, 100 and 160 micron. Almost two thirds of the objects were detected at least in one ISOPHOT band. The detection rate is independent of the source redshift, very likely due to the negative K-correction of the far-IR thermal emission. More than a half of the optically selected QSOs show significant emission between 4 and 100 micron in the quasar rest-frame. These fluxes have a very likely thermal origin, although in a few objects an additional contribution from a non-thermal component is plausible in the long wavelength bands. In a colour-colour diagram these objects span a wide range of properties from AGN-dominated to ULIRG-like. The far-IR composite spectrum of the quasar population presents a broad far-IR bump between 10 and 30 micron and a sharp drop at wavelengths greater than 100 micron in the quasar restframe. The amount of energy emitted in the far-IR, is on average a few times larger than that emitted in the blue and the ratio L(FIR)/L(B) increases with the bolometric luminosity. Objects with fainter blue magnitudes have larger ratios between the far-IR (wavelengths > 60 micron) fluxes and the blue band flux, which is attributed to extinction by dust around the central source. No relation between the blue absolute magnitude and the dust colour temperature is seen, suggesting that the dominant source of FIR energy could be linked to a concurrent starburst rather than to gravitational energy produced by the central engine.Comment: Astronomical Journal, in pres
