IgG4-Related Disease and the Spectrum of Mimics in Rheumatology
Immunoglobulin G4 (IgG4)-related disease (IgG4-RD) is an immune-mediated condition that can affect almost any organ. It is a chronic, systemic, inflammatory condition of unknown etiology. Pseudotumor formation is the most common and characteristic clinical symptom. The variable organ dysfunction reflects the clinical presentation. Because there are not specific antibodies for this disease, histopathological assessment provide the pivotal role in the diagnosis. IgG4-RD is characterized by a lymphoplasmacytic infiltrate composed of IgG4+ plasma cells, storiform fibrosis, obliterative phlebitis and mild to moderate eosinophilia. In this chapter we present the newest knowledge of the IgG4-RD pathogenesis and then concentrate on clinical symptoms which can mimic many other conditions in rheumatology, e.g., this common as Sjӧgren syndrome or rare as vasculitis or idiopathic retroperitoneal fibrosis
Cryoglobulinemia in the course of a primary Sjögren’s syndrome
A 58-year-old woman with primary Sjögren’s syndrome (pSS), peripheral neuropathy and positive serum cryoglobulin was admitted for evaluation of purpura on her skin. Cryoglobulinemic vasculitis was diagnosed based on clinical and laboratory findings . There was no response to cyclophosphamide pulse therapy, steroid therapy and cyclosporine. Prevalence and clinical characteristics of rheumatoid arthritis in Poland : a nationwide study
Introduction: There are no reliable data regarding the prevalence of rheumatoid arthritis (RA) in Poland.
Material and methods: The first stage was a face-to-face survey on a nationwide representative sample of 3000 people, which identified respondents with
a physician-confirmed diagnosis of RA. The second stage was a survey of RA patients, which characterized the disease course and treatment. It was evaluated by
analysis of a representative group of 1957 RA patients in routine clinical practice.
Results: The overall RA prevalence in Poland was 0.9% (95% CI: 0.6–1.2%),
1.06% for women, 0.74% for men. Seventy-eight percent were female, mean
age was 56 and mean disease duration 7 years. Younger patients (< 50)
remained professionally active in 90% of cases. Thirty percent of patients
were diagnosed within 3 months of the first RA symptoms, while for 17% it
took more than 1 year. Fifty-six percent of newly diagnosed patients were
characterized by high disease activity (DAS-28 > 5.1). Presently, low disease
activity (DAS-28 < 3.2) was found in 38.5% of patients. In Poland, 94% of patients have been treated with non-steroid anti-inflammatory drugs, almost
80% with glucocorticoids. Meanwhile, methotrexate, as an anchor drug in
Poland, has been used by 80% of patients, biological agents by 2.94% of
patients.
Conclusions: This is the first cross-sectional population-based epidemiological study regarding prevalence of RA in the adult Polish population. The
results demonstrate a high prevalence, falling within the upper boundary
estimates for Europe. Despite ongoing treatment, the majority still have
moderate to high disease activity, and the use of biological therapies is low

Witamina K to grupa związków chemicznych o zbliżonej budowie. W organizmie człowieka witamina K jest niezbędna w procesie potranslacyjnej karboksylacji niektórych reszt kwasu glutaminowego wchodzących w skład białek. Proces ten dotyczy kilkunastu białek i jest niezbędny dla ich prawidłowej czynności. Dotyczy to nie tylko białek uczestniczących w krzepnięciu krwi, ale przede wszystkim białek regulujących proces odkładania hydroksyapatytu w kościach, a także w prewencji ektotopowej kalcyfikacji innych tkanek. Badania doświadczalne, epidemiologiczne i interwencyjne wykazały, że niedobór witaminy K może sprzyjać rozwojowi osteoporozy i jej następstw, a także może sprzyjać zwiększonej częstości incydentów sercowo-naczyniowych oraz innych chorób (w tym dotyczących układu nerwowego). Wykazano też korzystny synergizm działania witaminy K i witaminy D w metabolizmie kości. Dotychczasowe obserwacje sugerują, że suplementacja witaminy K2 może stanowić składową profilaktyki i leczenia osteoporozy, ale nie stało się to dotychczas przedmiotem zapisu żadnych rekomendacji europejskich lub amerykańskich, a także polskich

Position of the Expert Panel of the National Consultant for Rheumatology concerning diagnosis and treatment of rheumatoid arthritis
Celem publikacji przygotowanej przez zespół powołany przez konsultanta krajowego ds. reumatologii jest usystematyzowanie wiedzy na temat diagnostyki i terapii reumatoidalnego zapalenia stawów. Oprócz rozpoznania reumatoidalnego zapalenia stawów
na podstawie kryteriów ACR, zaproponowano kryteria diagnostyczne wczesnego reumatoidalnego i wczesnego niesklasyfikowanego
zapalenia stawów. Określono w nich przydatność oznaczania przeciwciał antycytrulinowych, ultrasonografii stawów oraz rezonansu
magnetycznego. Podstawowym celem we wczesnym postępowaniu jest ustalenie obecności i aktywności zapalenia na podstawie
wywiadu, badania fizykalnego, badań laboratoryjnych, ewentualnie USG i rezonansu magnetycznego. Postępowanie różnicujące
powinno być prowadzone w miarę postępu choroby, do czasu
ustalenia ostatecznego rozpoznania. W każdym przypadku zapale nia stawów (poza infekcyjnym) należy wdrożyć leczenie glikokortykosteroidami w dawce je opanowującej. W zależności od ustalonego rozpoznania stosowanie glikokortykosteroidów powinno być
połączone ze stosowaniem leku podstawowego, nie później jednak
niż od 4. mies. Preferowanym lekiem jest metotreksat w dawce tygodniowej 15–25 mg. W przypadku jego nietolerancji sugerowane
jest zastosowanie leflunomidu. Brak skuteczności monoterapii
którymkolwiek z nich jest wskazaniem do leczenia skojarzonego
kilkoma lekami modyfikującymi przebieg choroby. Leki antycytokinowe należy wdrożyć w przypadku nieskuteczności tego postępowania, powinno ono być połączone z podawaniem metotreksatu
w pełnych dawkach, wyjątkowo innego leku podstawowego. Leczenie rytuksymabem i abataceptem stosuje się u chorych, u których powyższe postępowanie jest nieskuteczne. Niesteroidowe leki przeciwzapalne stanowią leczenie uzupełniające tylko
w okresach zaostrzeń. Przez wszystkie lata powinna być stosowana fizjoterapia. Celem leczenia jest uzyskanie i podtrzymywanie
małej aktywności choroby ocenianej wg kryteriów DAS 28.The aim of the publication prepared by the panel formed by the
State Consultant for Rheumatology is to summarize knowledge on
diagnosis and treatment of rheumatoid arthritis. Except diagnosing
rheumatoid arthritis on the basis of ACR criteria, diagnostic criteria
for early rheumatoid and early unclassified arthritis have been
proposed. These criteria establish the usefulness of anticitruline
antibody determinations, ultrasonography of joints and magnetic
resonance. The most important procedure in early detection is to
establish the presence and activity of inflammatory process, based
on history, physical examination, laboratory tests and, possibly,
ultrasonography and magnetic resonance. Differentiation should
progress following the disease progress, until final diagnosis is
established. In each case of arthritis (except infective arthritis)
treatment with glucocorticoids should be employed, at the dose allowing suppression of symptoms. Depending on established
diagnosis, this treatment should be accompanied by first-line
medication, not later than from the 4th month. Preferred medication
is methotrexate at a weekly dose of 15-25 mg. In the case of
intolerance, it is suggested to use leflunomide. Lack of efficiency of
monotherapy with either of the above mentioned medications is an
indication for a combined treatment with several drugs modifying
the course of the disease. Anticytokine drugs should be used in the
case of inefficiency of such a procedure; this should be connected
with full doses of methotrexate or, in exceptional cases, with
another first-line drug. Treatment with rituximab and abatacept is
reserved for patients in whom the above therapy is ineffective. Nonsteroid anti-inflammatory drugs offer an adjuvant therapy option
only during exacerbations. During all these years, physiotherapy
should be continued. Treatment should aim at achieving and
maintaining low activity of the disease, evaluated according to DAS
28 criteria
Cytogenetic and molecular analyses of de novo translocation dic(9;13)(p11.2;p12) in an infertile male
BACKGROUND: Whole arm t(9;13)(p11;p12) translocations are rare and have been described only a few times; all of the previously reported cases were familial. RESULTS: We present here an infertile male carrier with a whole-arm reciprocal translocation dic(9;13)(p11.2;p12) revealed by GTG-, C-, and NOR-banding karyotypes with no mature sperm cells in his ejaculate. FISH and genome-wide 400 K CGH microarray (Agilent) analyses demonstrated a balanced chromosome complement and further characterised the abnormality as a dicentric chromosome (9;13): dic(9;13)(pter→p11.2::p12→qter),neo(9)(pter→p12→neo→p11.2). An analysis of the patient’s ejaculated cells identified immature germ cells at different phases of spermatogenesis but no mature spermatozoa. Most (82.5%) of the germ cells were recognised as spermatocytes at stage I, and the cell nuclei were most frequently found in pachytene I (41.8%). We have also undertaken FISH analysis and documented an increased rate of aneuploidy of chromosomes 15, 18, X and Y in the peripheral blood leukocytes of our patient. To study the aneuploidy risk in leukocytes, we have additionally included 9 patients with non-obstructive azoospermia with normal karyotypes. CONCLUSIONS: We propose that the azoospermia observed in the patient with the dic(9;13)(p11.2;p12) translocation was most likely a consequence of a very high proportion (90%) of association between XY bivalents and quadrivalent formations in prophase I
Zastosowanie sekukinumabu w leczeniu chorych na łuszczycowe zapalenie stawów — stanowisko ekspertów Polskiego Towarzystwa Reumatologicznego i Polskiego Towarzystwa Dermatologicznego
Sekukinumab jest lekiem biologicznym, ludzkim monoklonalnym przeciwciałem skierowanym przeciwko interleukinie 17A stosowanym w leczeniu chorych na łuszczycowe zapalenie stawów. Lek może być stosowany po nieskuteczności metotreksatu lub leków biologicznych a u chorych na postać osiową choroby jako pierwszy lek, po nieskuteczności niesteroidowych leków przeciwzapalnych. Ze względu na mechanizm działania odmienny od dotychczas stosowanych leków, znaczącą skuteczność i duże bezpieczeństwo stosowania wprowadzenie sekukinumabu do leczenia chorych na łuszczycowe zapalenie stawów istotnie poszerzy możliwości dostępnej farmakoterapii tej choroby.
