Analiza w czasie rzeczywistym QRS bazująca na transformacie falkowej
The pacing for permanent pacing in bradyarrhytmias can influence ventricular activation and can contribute to marked asynchrony in extremity. In this work, we search the optimal position for right ventricular pacing according the QRS duration. We evaluate the system for detection of QRS boundaries based on wavelet transform and adaptive threshold. Import 16/04/2013Trvalá kardisotimulace v rámci léčby bradyarytmií, negativně ovlivnit aktivaci obou komor srdečních a může vést k významné komorové dyssynchronii. Tato práce se zabývá analýzou EKG signálu a návrhem stimulační pozice na základě šířky stimulovaného QRS komplexu, s úmyslem předcházet rozvoji komorové dyssynchronie.
V rámci řešení práce byl vytvořen měřící systém pro detekci hranic QRS komplexu založený na vlnkové transformaci a adaptabilním prahování. Spolehlivost algoritmu byla ověřena na dvou standardních databázích z mezinárodního projektu pro standardizaci v elektrokardiografii. Systém byl úspěšně otestován během několika implantací trvalého kardiostimulátoru na elektrofyziologických sálech v Nemocnici Podlesí a.s. v Třinci.
Navržený a realizovaný systém je vhodným nástrojem k dosažení lepších stimulačních výsledků, a může tak zabránit v částečné míře progresi srdečních onemocnění, které léčba trvalou kardiostimulací občas způsobuje.The pacing for permanent pacing in bradyarrhythmias can influence ventricular activation and can contribute to marked asynchrony in extremity. There was evaluated the system for detection of QRS boundaries based on the wavelet transform and an adaptive thresholding. The reliability of an algorithm was verified by two standard CSE databases. The system was tested during pacemaker implantation in Department of Cardiology, Heart Center, Hospital Podlesí Třinec.

The main focus was design an optimal detection method for intracardial signals based on essential intracardial measuring methods and basic principles of gathering and processing data from invasive catheters. Intracardial ECG analysis is further important step in heart behavior understanding, especially it’s electric manners. Detailed signal description generated in heart,together with heart function knowledge can provide us with an useful information about heart’s condition or it’s diseases. Designed detection methodis able to mark significant points in intracardial records, compute it‘s elemental parameters and important time intervals. Cardiac Radiosurgery for Malignant Ventricular Tachycardia
Abstract Ventricular tachycardia is a frequent cause of mortality after myocardial infarction. Current treatment includes the implantation of cardioverter defibrillators and adjunctive therapies such as catheter ablation or cardiac surgery. In patients where standard treatment fails, preclinical data showed that radiosurgery ablation of the ectopic substrate might be a viable option. Authors present a case report of cardiac radiosurgery in a patient with malignant ventricular tachycardia. Stereotactic radiosurgery system CyberKnife was used; the applied dose was 25 Gy in one fraction. Within the follow-up period of 120 days, no signs of toxicity were noted no episode of malignant arrhythmia has been detected. This case report demonstrates that stereotactic radiosurgery of recurrent ventricular tachycardia after inefficient catheter ablation might be a viable option for patients unsuitable for cardiosurgical intervention. Further research on this topic is highly warranted
