Provocative Tests of the Hypothalamic-Anterior Pituitary-Target Organ Axis
Anterior pituitary peptide hormones which circulate at nanogram or picogram levels are readily measurable under basal conditions by radioligand assay. In addition, functional reserve may be further assessed by the use of specific provocative tests 'aimed' at either hypothalamus, anterior pituitary or the target organ. The management of recurrent thyrotoxicosis after thyroidectomy
Thirty-six patients who suffered a recurrence of thyrotoxicosis after subtotal thyroidectomy have been retrospectively analysed. After 11 subsequent second operations, 7 further relapses occurred while 2 patients had permanent recurrent laryngeal nerve paralyses. Only 1 patient out of 18 treated with either long-term neomercazole or betaadrenergic blockade remained in remission. Radio-active iodine is the treatment of choice in patients who relapse after surgery for thyrotoxicosis. A significant percentage of thyrotoxic patients treated by subtotal thyroidectomy have a recurrence of the hyperthyroid state.'" Attention has recently been focused on the management of such patients; in particular the poor results of surgery in such recurrent cases have been emphasized: This report concerns a retrospective analysis of the management of 36 thyrotoxic patients seen at Groote Schuur Hospital who had a recurrence of hyperthyroidism after thyroidectomy
Practolol in Hyperthyroid Cardiac Failure
Practolol (Eraldint), a recently developed beta-adrenergic blocking agent, has been found useful in conjunction with digitalis and diuretics in 6 patients with thyrocardiac disease who had been refractory to therapy. In all cases cardiac failure improved dramatically, suggesting better myocardial efficiency and a return of digitalis responsiveness after practolol. This drug might be a useful therapeutic adjunct in thyrocardiac disease
Bone Densitometry of the Femoral Midshaft the Protein-Deprived Rat
Densitometric assessment of radiographs of the femoral midshafts in protein-deprived and age-matched control rats, has shown a significant loss of total bone density in the protein-deprived group. This reduction is no greater than can be accounted for by the loss of cortical bone surface area, suggesting that while bone mass is reduced as a result of protein deprivation, the mineral composition of the residual bone is likely to be normal.
