Difficult dual stage transcutaneous lead extraction complicated by fracture of both leads
Extraction of two long-implanted (11 year-old) active leads due to local infection was complicated by the fracture of both leads with a metal sheath. The presence of a temporary pacing lead, inserted due to pacemaker dependence, made extraction even more difficult. The fixation of both proximal extracted lead endings in the occluded subclavian vein was much stronger than the connection of the tips of the leads with the RAA and RVA endocardium. The disintegration of both leads caused by drawing upon the distal ending added additional technical complications to the final procedure. The presented case underscores the necessity of having a full set of tools to hand even during what appears to be a routine lead extraction procedure. The importance of proper lead implantation on its extraction (in what may be the distant future) is discussed
Doświadczenia ośrodka lubelskiego w przezskórnym usuwaniu wrośniętych elektrod wewnątrzsercowych - analiza ostatnich 3 lat
W niniejszym artykule przedstawiono 3-letnie doświadczenia zespołu lekarzy z kilku ośrodków
w przezskórnym usuwaniu wrośniętych elektrod wewnątrzsercowych u pacjentów z terenu
całego kraju, wykonywanych w Lublinie. We wstępie omówiono wcześniejsze, ponad 25-letnie
doświadczenia. Szczegółowa analiza dotyczy usunięć elektrod z ostatnich 3 lat, od kiedy jest
prowadzony szczegółowy rejestr. Z analizy wynika niemal 100-procentowa ostateczna efektywność
zabiegów i akceptowalny odsetek powikłań (< 5%). Procedura przezskórnego usuwania
wrośniętych elektrod wewnątrzsercowych składa się z kilku uzupełniających się technik. Chociaż
podstawowy dostęp podobojczykowy pozwala na kompletne usunięcie większości elektrod,
to zastosowanie technik alternatywnych (dostęp udowy, zestaw koszyków, cewników typu lasso)
jest niezbędne do dokończenia poniżej 10% zabiegów, zaś współpraca z doświadczonym
radiologiem interwencyjnym i dobrze wyposażona pracownia radiologiczna są potrzebne do
wykonania kilku procent zabiegów. Bardzo istotne jest zabezpieczenie zabiegów przezżylnego
usuwania elektrod przez pozostający w gotowości operacyjnej zespół kardiochirurgiczny
Broken leads with proximal endings in the cardiovascular system: Serious consequences and extraction diffi culties
Background: Retrospective analysis of effectiveness, technical problems, and complications oftransvenous extraction of leads with the free endings migrated to the cardiovascular system (CVS).Methods: A 5-year-old database of transvenous lead extraction (TLE) procedures comprising906 patients with 1563 leads being removed was analyzed. TLE procedures of leads migratedin the CVS were compared with TLE procedures of leads with their proximal ends accessiblein the pacemaker/implantable cardioverter-defi brillator (PM/ICD) pocket.Results: In our material, the phenomenon of leads migration occurred in 5% of patients referredfor TLE and affected most frequently unipolar and atrial leads. The presence of migratingleads was associated with local venous occlusion in 64% of patients. Removal of migratingleads required other techniques than extraction of leads with their proximal ends accessible inthe PM/ICD pocket. More than 95% of migrating leads were extracted transvenously, but procedureswere signifi cantly longer. The presence of other leads made extraction of migrated leadseven more complicated. Effectiveness and complication rates for removal of migrated leads andleads accessible in the PM/ICD pocket were similar.Conclusions: We postulate that every lead migrating in the CVS should be considered forTLE. However, this extraction is technically more diffi cult and challenging than extraction ofleads accessible in the PM/ICD pocket
Pacemaker lead extraction and recapture of venous access: Technical problems arising from extensive venous obstruction
We report the case of the extraction of 18 year-old leads in a patient with a DDD pacemaker,
and chronic obstruction of the left subclavian and innominate veins coexisting with extensive
stenoses in the upper caval vein. After removal of pacing leads, angiographic guidewires were
introduced via the Byrd dilatators and new pacing leads introduced with the use of long
sheaths originally dedicated for transvenous left ventricular leads implantation. With this
case, we discuss the problems arising during reimplantation of pacing leads in patients with

Ciąża w bliźnie po cięciu cesarskim: Embolizacja tętnic macicznych połączona z histerektomią w 13 tygodniu ciąży – opis przypadku i przegląd piśmiennictwa
A cesarean scar pregnancy is a pregnancy located within the uterine muscle after previous cesarean sections. Recent years have shown a significant increase in the rate of CS and an improvement in the ultrasound diagnosis, and therefore a trend towards an increase in the rate of CSP cases has been reported in many countries. We report on a case of CSP diagnosed using ultrasound at 5/6 weeks’ gestation and confirmed by magnetic resonance imaging. The patient underwent surgical management at 13 weeks, combined with the chemioembolization of the
uterine arteries. The current review aims to update the knowledge of the available treatment modalities.Ciąża w bliźnie po cięciu cesarskim jest ciążą zlokalizowaną w warstwie mięśniówki przedniej ściany macicy, w miejscu blizny po cięciu cesarskim. W ostatnich latach zaobserwowano znaczący wzrost odsetka cięć cesarskich oraz udoskonalenie diagnostyki ultrasonograficznej. W wielu krajach doprowadziło to do znacznego zwiększenia liczby rozpoznawanych przypadków ciąży w bliźnie po cięciu cesarskim. Opisywany przypadek przedstawia rozpoznanie ciąży w bliźnie po cięciu cesarskim w 5/6 tygodniu ciąży w badaniu ultrasonograficznym oraz jej potwierdzenie w rezonansie magnetycznym. W 13 tygodniu ciąży wykonano embolizację tętnic macicznych z następową histerektomią bez przydatków. Przedstawiono przegląd piśmiennictwa dotyczący aktualnych rodzajów postępowania klinicznego
Diagnosis and treatment of renal artery stenosis — review of the literature
Zwężenie tętnic nerkowych (RAS) występuje u 6–8% osób po 65. roku życia. Skutkami zwężenia są zwiększenie częstości występowania incydentów sercowo-naczyniowych oraz wyższa śmiertelność pacjentów. Ciągły rozwój metod diagnostycznych ułatwia wykrycie tej patologii oraz umożliwia jej dokładny opis. Sposób leczenia RAS oraz jego skutków wciąż nie został jednoznacznie ustalony. Opublikowane dotychczas wyniki badań dowiodły wyższość leczenia farmakologicznego nad postępowaniem zabiegowym, jednak wskazano na licznenie dopatrzenia w sposobie ich przeprowadzenia.Skutkowało to rozpoczęciem kolejnych badań, których wyniki jeszcze nie są znane. Celem tej pracy jest przedstawienie aktualnych sposobów diagnostyki i leczenia RAS oraz opis przeprowadzonych dotychczas badań, w których porównywano różne strategie leczenia.Renal artery stenosis (RAS) is encountered in about 6–8% of people older than 65 years. This pathology leads to the higher rate of cardiovascular events as well as higher mortality. Constant progress inimaging techniques allows greater accuracy and sensitivity during diagnosis of RAS. Nevertheless, the way of treating patients with RAS is not clear. Medical treatment is better than renal artery revascularization according to currently published trials. However, some serious mistakes were reported in their methodology. There are a few trials going on but results are still not known. The goal of this review is to present up-to-date methods of diagnosis and treatment of RAS as well as to present published trials comparing alternative methods of treatment
