    Résolution parallèle de SAT: mieux collaborer pour aller plus loin

    National audienceLa gestion de la base de clauses apprises est connue pour être une tâche ardue au sein des solveurs SAT. Dans le cadre des solveurs parallèles de type portfolio, la collaboration réalisée entre les différents processus au moyen de l'échange de clauses rend cette gestion plus complexe encore. Différentes techniques ont été proposées ces dernières années, mais les résultats pratiques restent en faveur de solveurs ayant une collaboration limitée voire nulle. Ceci est principalement dû au fait que chacun des processus doit gérer le grand nombre de clauses générées par les autres. Dans cet article, nous proposons une nouvelle technique efficace pour l'échange de clauses au sein d'un solveur parallèle. Contrairement aux méthodes actuelles, notre approche repose sur des politiques pour l'exportation et pour l'importation de clauses, tout en utilisant des techniques récentes, qui ont prouvées leur efficacité dans le cadre séquentiel. Un grand nombre d'expérimentations tend à montrer l'intérêt des idées proposées

    Ro 28-2653 is a new synthetic inhibitor of matrix metalloproteinases. The ability of these enzymes to degrade various components of the extracellular matrix seems to play a major role in tumors progression and is potentially effective against bronchial remodeling in asthma and BPCO. Ro 28-2653 is very poorly soluble in water. This low solubility estimated at about 0.56 lg/ml in water at 25 C gives rise to difficulties in pharmaceutical formulation of oral, injectable or nebulizable solutions. The purpose of our study is to prepare and to characterize inclusion complexes between Ro 28-2653 and cyclodextrins and to investigate the biopharmaceutical repercussion of the inclusion of the active substance.The complex formation was investigated by phase solubility studies. 1H-NMR spectroscopy and molecular modeling studies were carried out to elucidate the structure of the inclusion complex between Ro 28-2653 and cyclodextrin. Oral, intravenous and nebulizable solutions of Ro 28-2653 were developed with cyclodextrin. The in vivo studies were performed on healthy sheep for the pharmacokinetic evaluation of the oral and intravenous formulations while the nebulization of the complex solution was studied by using an asthma model in mouse

    Engaging family supporters of adult patients with diabetes to improve clinical and patient-centered outcomes: study protocol for a randomized controlled trial

    Abstract Background Most adults with diabetes who are at high risk for complications have family or friends who are involved in their medical and self-care (“family supporters”). These family supporters are an important resource who could be leveraged to improve patients’ engagement in their care and patient health outcomes. However, healthcare teams lack structured and feasible approaches to effectively engage family supporters in patient self-management support. This trial tests a strategy to strengthen the capacity of family supporters to help adults with high-risk diabetes engage in healthcare, successfully enact care plans, and lower risk of diabetes complications. Methods/design We will conduct a randomized trial evaluating the CO-IMPACT (Caring Others Increasing EnageMent in Patient Aligned Care Teams) intervention. Two hunded forty adults with diabetes who are at high risk for diabetes complications due to poor glycemic control or high blood pressure will be randomized, along with a family supporter (living either with the patient or remotely), to CO-IMPACT or enhanced usual primary care for 12 months. CO-IMPACT provides patient-supporter dyads: it provides one coaching session addressing supporter techniques for helping patients with behavior change motivation, action planning, and proactive communication with healthcare providers; biweekly automated phone calls to prompt dyad action on new patient health concerns; phone calls to prompt preparation for patients’ primary care visits; and primary care visit summaries sent to both patient and supporter. Primary outcomes are changes in patient activation, as measured by the Patient Activation Measure-13, and change in 5-year cardiac event risk, as measured by the United Kingdom Prospective Diabetes Study cardiac risk score for people with diabetes. Secondary outcomes include patients’ diabetes self-management behaviors, diabetes distress, and glycemic and blood pressure control. Measures among supporters will include use of effective support techniques, burden, and distress about patient’s diabetes care. Discussion If effective in improving patient activation and diabetes management, CO-IMPACT will provide healthcare teams with evidence-based tools and techniques to engage patients’ available family or friends in supporting patient self-management, even if they live remotely. The core skills addressed by CO-IMPACT can be used by patients and their supporters over time to respond to changing patient health needs and priorities.     Medication cost problems among chronically ill adults in the US: did the financial crisis make a bad situation even worse?

    A national internet survey was conducted between March and April 2009 among 27,302 US participants in the Harris Interactive Chronic Illness Panel. Respondents reported behaviors related to cost-related medication non-adherence (CRN) and the impacts of medication costs on other aspects of their daily lives. Among respondents aged 40–64 and looking for work, 66% reported CRN in 2008, and 41% did not fill a prescription due to cost pressures. More than half of respondents aged 40–64 and nearly two-thirds of those in this group who were looking for work or disabled reported other impacts of medication costs, such as cutting back on basic needs or increasing credit card debt. More than one-third of respondents aged 65+ who were working or looking for work reported CRN. Regardless of age or employment status, roughly half of respondents reporting medication cost hardship said that these problems had become more frequent in 2008 than before the economic recession. These data show that many chronically ill patients, particularly those looking for work or disabled, reported greater medication cost problems since the economic crisis began. Given links between CRN and worse health, the financial downturn may have had significant health consequences for adults with chronic illness

    Functionally Fractal Urban Networks: Geospatial Co-location and Homogeneity of Infrastructure

    Just as natural river networks are known to be globally self-similar, recent research has shown that human-built urban networks, such as road networks, are also functionally self-similar, and have fractal topology with power-law node-degree distributions (p(k) = a k). Here we show, for the first time, that other urban infrastructure networks (sanitary and storm-water sewers), which sustain flows of critical services for urban citizens, also show scale-free functional topologies. For roads and drainage networks, we compared functional topological metrics, derived from high-resolution data (70,000 nodes) for a large US city providing services to about 900,000 citizens over an area of about 1,000 km2. For the whole city and for different sized subnets, we also examined these networks in terms of geospatial co-location (roads and sewers). Our analyses reveal functional topological homogeneity among all the subnets within the city, in spite of differences in several urban attributes. The functional topologies of all subnets of both infrastructure types resemble power-law distributions, with tails becoming increasingly power-law as the subnet area increases. Our findings hold implications for assessing the vulnerability of these critical infrastructure networks to cascading shocks based on spatial interdependency, and for improved design and maintenance of urban infrastructure networks
