    The metabolic syndrome and renal function in an African cohort infected with human immunodeficiency virus

    Introduction: The human immunodeficiency virus (HIV) is often accompanied by renal dysfunction. It is expected that metabolic syndrome (MetS) may exacerbate renal impairment.Objective: We therefore determined the prevalence of MetS and the association thereof with renal function in a South African cohort infected with HIV.Methods: We matched 114 HIV-infected (77.3% on antiretroviral therapy [ART] and 22.7% ART-naïve) and 114 HIV-uninfected individuals according to age, sex and locality. We examined cardiovascular, anthropometric and metabolic measurements and determined the MetS. Renal function was assessed using standardised procedures.Results: The prevalence of MetS was lower in the HIV-infected individuals as compared to the uninfected individuals (28% vs. 44%, p = 0.013). The HIV-infected group presented with a lower body mass index (BMI) and waist circumference (WC) (all p < 0.001), as well as blood pressure (BP) ( p ≤ 0.0021). The results were confirmed when comparing the HIV-infected group using ART (N = 85) and the HIV-uninfected group. When comparing the HIV-infected individuals with MetS to the HIV-uninfected individuals with MetS, no differences in BP were seen. With regard to renal function, the HIV-infected individuals with MetS (n = 32) had 43% higher urinary albumin-creatinine ratio (uACR) compared to the HIV-uninfected individuals with MetS, after adjusting for age, sex and WC (p = 0.032). None of the other renal function markers differed after adjustments for WC or BMI.Conclusion: The HIV-infected Africans with MetS had almost twofold higher uACR, despite the low prevalence of MetS, compared to their uninfected counterparts. The combination of HIV and MetS seemed to increase the risk for renal impairment

    Gender health disparities among older adults in South Africa

    In Africa, South Africa included, not so many population-based studies examining gender differences in health outcomes of older people have been conducted. This population-based study examined gender differences in health outcomes among older South Africans. Data from the “Global Ageing and Adults Health (SAGE) wave 1” study (N=3284) involving South Africans aged 50+ years was analysed. Associations between socio-demographics (exposures) and health outcomes were examined through logistic regression analyses. Results indicate that the odds of having arthritis (p&lt;0.001), lung disease (p=0.012), obesity (p&lt;0.001), diabetes (p=0.003), anxiety (p=0.026), and nocturnal sleep problems (p&lt;0.001) were lower among men than women, and the odds for having underweight were higher among men than women (p=0.004). In conclusion, there are significant health differences among older men and women in South Africa. Understanding gender health disparities is critical for designing equitable health promotion strategies

    May Measurement Month 2017: an analysis of blood pressure screening results in South Africa-Sub-Saharan Africa

    Elevated blood pressure (BP) is a growing burden worldwide, leading to over 10 million deaths each year. May Measurement Month (MMM) is a global initiative of the International Society of Hypertension (ISH) aimed at raising awareness of high BP and to act as a temporary solution to the lack of screening programs worldwide. A surveillance study in 2016 in South Africa revealed that 45% of adults have hypertension and only 6–9% of men and women respectively had controlled BP on medication, highlighting the need for regular screening and awareness campaigns. An opportunistic cross-sectional survey of volunteers aged ≥18 years was carried out in May 2017. Blood pressure measurement, the definition of hypertension, and statistical analyses followed the MMM protocol. The sites screened were primarily university campuses and general populations in preference to hospitals and clinics, aiming to raise awareness and allow access to screening in those less likely to be aware of their BP. In total, 3250 individuals (mean age 31.0 ± 13.3 years) were screened. After multiple imputation for missing BP readings, 795 (24.5%) had hypertension. Of individuals not receiving antihypertensive medication, 459 (15.7%) were hypertensive, and 157 (46.9%) of individuals receiving antihypertensive medication had uncontrolled BP. These results suggest that opportunistic screening campaigns can identify significant numbers with undiagnosed and uncontrolled hypertension, even amongst the fairly young.
