    Fertility preservation in ovarian tumours

    A considerable number of patients with a cancer diagnosis are of childbearing age and have not satisfied their desire for a family. Despite ovarian cancer (OC) usually occurring in older patients, 3%–14% are diagnosed at a fertile age with the overall 5-year survival rate being 91.2% in women ≤44 years of age when it is found at 1A–B stage. In this scenario, testing the safety and the efficacy of fertility sparing strategies in OC patients is very important overall in terms of quality of life. Unfortunately, the lack of randomised trials to validate conservative approaches does not guarantee the safety of fertility preservation strategies. However, evidence-based data from descriptive series suggest that in selected cases, the preservation of the uterus and at least one part of the ovary does not lead to a high risk of relapse. This conservative surgery helps to maintain organ function, giving patients of childbearing age the possibility to preserve their fertility. We hereby analysed the main evidence from the international literature on this topic in order to highlight the selected criteria for conservative management of OC patients, including healthy BRCA mutations carriers

    Investigational Paradigms in Downscoring and Upscoring DCIS: Surgical Management Review

    Counseling patients with DCIS in a rational manner can be extremely difficult when the range of treatment criteria results in diverse and confusing clinical recommendations. Surgeons need tools that quantify measurable prognostic factors to be used in conjunction with clinical experience for the complex decision-making process. Combination of statistically significant tumor recurrence predictors and lesion parameters obtained after initial excision suggests that patients with DCIS can be stratified into specific subsets allowing a scientifically based discussion. The goal is to choose the treatment regimen that will significantly benefit each patient group without subjecting the patients to unnecessary risks. Exploring the effectiveness of complete excision may offer a starting place in a new way of reasoning and conceiving surgical modalities in terms of “downscoring” or “upscoring” patient risk, perhaps changing clinical approach. Reexcison may lower the specific subsets' score and improve local recurrence-free survival also by revealing a larger tumor size, a higher nuclear grade, or an involved margin and so suggesting the best management. It seems, that the key could be identifying significant relapse predictive factors, according to validated risk investigation models, whose value is modifiable by the surgical approach which avails of different diagnostic and therapeutic potentials to be optimal. Certainly DCIS clinical question cannot have a single curative mode due to heterogeneity of pathological lesions and histologic classification

    The device for converting direct current into alternate current comprises a multilevel converter associated with at least a source of direct current and a modulation unit having piloting means for piloting the converter for the conversion of the direct current into an alternate output current, in which the modulation unit comprises comparison means for comparing the output current value with a preset positive threshold value and a preset negative threshold value, the piloting means being suitable for piloting the converter with a pulse modulation of the unipolar type in the event of the output current value being above the positive threshold value or below the negative threshold value and with a pulse modulation of the complementary type in the event of the output current value being below the positive threshold value and above the negative threshold value. The method for converting direct current into alternate current comprises a piloting phase of a multilevel converter for the conversion of a direct voltage into an alternate output voltage, a comparison phase of the output current value with a preset positive threshold value and a preset negative threshold value, the piloting phase being suitable for piloting the converter with a pulse modulation of the unipolar type in the event of the output current value being above the positive threshold value or below the negative threshold value and with a pulse modulation of the complementary type in the event of the output current value being below the positive threshold value and above the negative threshold value

    Gestational weight gain in overweight and obese women enrolled in a healthy lifestyle and eating habits program

    OBJECTIVES: To determine whether changes in lifestyle in women with BMI > 25 could decrease gestational weight gain and unfavorable pregnancy outcomes. METHODS: Women with BMI > 25 were randomized at 1st trimester to no intervention or a Therapeutic Lifestyle Changes (TLC) Program including diet (overweight: 1700 kcal/day, obese: 1800 kcal/day) and mild physical activity (30 min/day, 3 times/week). At baseline and at the 36th week women filled-in a Food Frequency Questionnaire. OUTCOMES: gestational weight gain, gestational diabetes mellitus, gestational hypertension, preterm delivery. Data stratified by BMI categories. RESULTS: Socio-demographic features were similar between groups (TLC: 33 cases, CONTROLS: 28 cases). At term, gestational weight gain in obese women randomized to TLC group was lower (6.7 ± 4.3 kg) versus controls (10.1 ± 5.6 kg, p = 0.047). Gestational diabetes mellitus, gestational hypertension and preterm delivery were also significantly lower. TLC was an independent factor in preventing gestational weight gain, gestational diabetes mellitus, gestational hypertension. Significant changes in eating habits occurred in the TLC group, which increased the number of snacks, the intake of fruits-vegetables and decreased the consumption of sugar.     Methyl orange photo‐degradation by tio2 in a pilot unit under different chemical, physical, and hydraulic conditions

    The photo‐catalytic degradation of a textile azo‐dye as Methyl Orange was studied in an innovative unit constituted by a channel over which a layer of titanium dioxide (TiO2) catalyst in anatase form was deposited and activated by UVB irradiation. The degradation kinetics were followed after variation of the chemical, physical, and hydraulic/hydrodynamic parameters of the system. For this purpose, the influence of the TiO2 dosage (g/cm3), dye concentration (mg/L), pH of the solution, flow‐rate (L/s), hydraulic load (cm), and irradiation power (W) were evaluated on the degradation rates. It was observed that the maximum dosage of TiO2 was 0.79 g/cm3 while for higher dosage a reduction of homogeneity of the cement conglomerate occurred. The Langmuir– Hinshelwood (LH) kinetic model was followed up to a dye concentration around 1 mg/L. It was observed that with the increase of the flow rate, an increase of the degradation kinetics was obtained, while the further increase of the flow‐rate associated with the modification of the hydraulic load determined a decrease of the kinetic rates. The results also evidenced an increase of the kinetic rates with the increase of the UVB intensity. A final comparison with other dyes such as Methyl Red and Methylene Blue was carried out in consideration of the pH of the solution, which sensibly affected the removal efficiencies

    REM34 and REM35 control female and male gametophyte development in Arabidopsis thaliana

    The REproductive Meristem (REM) gene family encodes for transcription factors belonging to the B3 DNA binding domain superfamily. In Arabidopsis thaliana the REM gene family is composed of 45 members, preferentially expressed during flower, ovule and seed development. Only a few members of this family have been functionally characterized: VERNALIZATION1 (VRN1) and most recently TARGET OF FLC AND SVP1 (TFS1) regulate flowering time and VERDANDI (VDD), together with VALKYRIE (VAL) control the death of the receptive synergid cell in the female gametophyte. We investigated the role of REM34, REM35 and REM36, three closely related and linked genes similarly expressed in both female and male gametophytes. Simultaneous silencing by RNA interference (RNAi) caused about 50% of the ovules to remain unfertilized. Careful evaluation of both ovule and pollen development showed that this partial sterility of the transgenic RNAi lines was due to a post meiotic block in both female and male gametophytes. Furthermore, protein interaction assays revealed that REM34 and REM35 interact, which suggests that they work together during the first stages of gametogenesis

    Impact of woody encroachment on soil organic carbon and nitrogen in abandoned agricultural lands along a rainfall gradient in Italy

    Land use changes represent one of the most important components of global environmental change andhave a strong influence on carbon cycling. As a consequence of changes in economy during the last century, areas of marginal agriculture have been abandoned leading to secondary successions. The encroachment of woody plants into grasslands, pastures and croplands is generally thought to increase the carbon stored in these ecosystems even though there are evidences for a decrease in soil carbon stocks after land use change. In this paper, we investigate the effects of woody plant invasion on soil carbon and nitrogen stocks along a precipitation gradient (200–2,500 mm) using original data from paired experiment in Italian Alps and Sicily and data from literature (Guo and Gifford Glob Change Biol 8(4):345–360, 2002). We found a clear negative relationship (-0.05% C mm-1) between changes in soil organic carbon and precipitation explaining 70% of the variation in soil Cstocks after recolonization: dry sites gain carbon (up to? 67%) while wet sites lose carbon (up to -45%). In our data set, there seem to be two threshold values for soil carbon accumulation: the first one is 900 mm of mean annual rainfall, which separates the negative from the positive ratio values; the second one is 750 mm, which divides the positive values in two groups of sites. Most interestingly, this threshold of 750 mm corresponds exactly to a bioclimatic threshold: sites with\750 mm mean annual rainfall is classified as thermo-mediterranean sites, while the ones [750 mm are classified as mesomediterranean sites. This suggests that apart from rainfall also temperature values have an important influence on soil carbon accumulation after abandonment. Moreover, our results confirmed that the correlation between rainfall and trend in soil organic carbon may be related to nitrogen dynamics: carbon losses may occur only if there is a substantial decrease in soil nitrogen stock which occurs in wetter sites probably because of the higher leaching

    Positive selection in Europeans and East-Asians at the ABCA12 gene

    Natural selection acts on genetic variants by increasing the frequency of alleles responsible for a cellular function that is favorable in a certain environment. In a previous genome-wide scan for positive selection in contemporary humans, we identified a signal of positive selection in European and Asians at the genetic variant rs10180970. The variant is located in the second intron of the ABCA12 gene, which is implicated in the lipid barrier formation and down-regulated by UVB radiation. We studied the signal of selection in the genomic region surrounding rs10180970 in a larger dataset that includes DNA sequences from ancient samples. We also investigated the functional consequences of gene expression of the alleles of rs10180970 and another genetic variant in its proximity in healthy volunteers exposed to similar UV radiation. We confirmed the selection signal and refine its location that extends over 35 kb and includes the first intron, the first two exons and the transcription starting site of ABCA12. We found no obvious effect of rs10180970 alleles on ABCA12 gene expression. We reconstructed the trajectory of the T allele over the last 80,000 years to discover that it was specific to H. sapiens and present in non-Africans 45,000 years ago

    Uptake-release by MSCs of a cationic platinum(II) complex active in vitro on human malignant cancer cell lines

    In this study, the in vitro stability of cisplatin (CisPt) and cationic platinum(II)-complex (caPt(II)-complex) and their in vitro activity (antiproliferative and anti-angiogenic properties) were investigated against three aggressive human tumor cell lines. caPt(II)-complex shown a high stability until 9 days of treatment and displayed a significant and higher activity than CisPt against both NCI-H28 mesothelioma (19.37 \ub1 9.57 \u3bcM versus 34.66 \ub1 7.65 \u3bcM for CisPt) and U87 MG glioblastoma (19.85 \ub1 0.97 \u3bcM versus 54.14 \ub1 3.19 for CisPt). Mesenchymal Stromal Cells (AT-MSCs) showed a significant different sensitivity (IC50=71.9 \ub1 15.1 \u3bcM for caPt(II)-complex and 8.7 \ub1 4.5 \u3bcM for CisPt) to the antiproliferative activity of caPt(II)-complex and CisPt. The ability of MSCs to uptake both the drugs in a similar amount of 2.49 pM /cell, suggested a possible development of new therapies based on cell mediated drug delivery
