Nebulised heparin: a new approach to the treatment of acute lung injury?

The administration of heparin by nebulisation has been proposed for the 'local' treatment of pulmonary coagulation disturbances in acute lung injury (ALI). Alveolar and lung micro-vascular fibrin accumulation and breakdown inhibition indeed play a central role in the development and clinical course of this disease. Preclinical studies provide some evidence of the beneficial effects of heparin inhalation in several animal models of ALI. Clinical investigations are sparse, and trials such as the one presented by Dixon and colleagues in a recent issue of Critical Care are welcome as they provide insight into the possible clinical use of nebulised heparin in this situation. This phase 1 trial involved 16 patients with early ALI, and showed the feasibility of the approach. In addition, non-significant changes in respiratory functions and systemic anticoagulant effects were documented with the four doses tested. The study of Dixon and colleagues adds to data that helps pave the way towards a possible clinical use of heparin by nebulisation in ALI. It remains to be clarified in which clinical situations, at what time points and with which dosages the best chances exist for a beneficial effect on the prognosis of these patients

Soziale Unterstützung, soziale Belastungen und Gesundheit bei leicht hilfebedürftigen Betagten

Zusammenfassung: Der Beitrag befasst sich mit der Bedeutung verschiedener sozialer Teilnetze für soziale Unterstützung und Belastung und untersucht den Einfluss von sozialer Unterstützung und Belastung auf den Gesundheitszustand von Betagten. 171 (t1, 1992) bzw. 135 (t2, 1993) leicht pflegeoder betreuungsbedürftige Betagte im Alter von mindestens 75 Jahren aus drei Zürcher Stadtkreisen wurden mit einem strukturierten Interview zweimal mündlich befragt. Die Ergebnisse zeigen, dass die familialen und ausserfamilialen Sozialbeziehungen für die befragten Frauen und Männer eine zentrale soziale Ressource für die Mobilisierung von praktischer und emotionaler Unterstützung bilden. Während sich Männer durch das familiale Sozialnetz (v.a. die Partnerschaft) am stärksten unterstützt fühlen, spielen für Frauen sowohl familiale wie ausserfamiliale Kontakte eine wichtige Rolle. Der ausgeprägten Unterstützungsfunktion sozialer Beziehungen stehen nur relativ schwache Belastungswirkungen gegenüber. Soziale Unterstützung wirkt sich hauptsächlich indirekt positiv auf die Gesundheit aus. Im Zeitverlauf zwischen t1 und t2 verändert sich das soziale Beziehungsnetz, die soziale Unterstützung sowie der Gesundheitszustand nur wenig, wohingegen soziale Belastungen eine vergleichsweise niedrigere zeitliche Konstanz aufweise

Der Finanzierungsmix von Sportvereinen ist sehr unterschiedlich ausgestaltet, wobei insbesondere Mitgliederbeiträgen, Sponsoring und Beiträgen der öffentlichen Hand eine tragende Rolle zukommt. Private Spenden hingegen machen nur einen marginalen Anteil aus, obwohl darin noch grosses Potenzial schlummert. Using the example of Swiss housing co-operatives, the study shows how the ndividual member value perception is influenced by the co-operative governance and what other organizational as well as individual factors are relevant in this manner. The strongest effect over all is the age of the co-op: the older a co-op is the better is the economic value for the members but on the same time the lower are the co-operative member values

Simulating evolutionary responses of an introgressed insect resistance trait for ecological effect assessment of transgene flow: a model for supporting informed decisionmaking in environmental risk assessment

Predicting outcomes of transgene flow from arable crops requires a system perspective that considers ecological and evolutionary processes within a landscape context. In Europe, the arable weed Raphanus raphanistrum is a potential hybridization partner of oilseed rape, and the two species are ecologically linked through the common herbivores Meligethes spp. Observations in Switzerland show that high densities of Meligethes beetles maintained by oilseed rape crops can lead to considerable damage on R. raphanistrum. We asked how increased insect resistance in R. raphanistrum – as might be acquired through introgression from transgenic oilseed rape – would affect seed production under natural herbivore pressure. In simulation experiments, plants protected against Meligethes beetles produced about twice as many seeds as unprotected plants. All stages in the development of reproductive structures from buds to pods were negatively affected by the herbivore, with the transition from buds to flowers being the most vulnerable. We conclude that resistance to Meligethes beetles could confer a considerable selective advantage upon R. raphanistrum in regions where oilseed rape is widely grown
