    JOINT STIFFNESS IN OLD AND YOUNG RUNNERS DURING SHOD AND BAREFOOT RUNNING

    The purpose of this study was to investigate the relationship between ankle and knee joint stiffness in barefoot and shod running. Fifteen old and 15 young individuals served as participants in this study. Joint kinematics and kinetics were collected while the participants ran overground in two running conditions: barefoot and shod. A quasi-joint stiffness was calculated using the quotient of the change in moment divided by the change in angle for the ankle and knee joints. A two-way ANOVA was conducted to asses ankle and knee joint stiffnesses. There were no interactions and no main effects for the knee joint stiffness. For the ankle joint, there was only a significant difference between conditions. These results indicate that in barefoot running, it is necessary to have a more compliant ankle to maintain the heel from touching the ground

    The purposes of this study were to identify differences of the three-dimensional joint kinetics between linear and curve sprinting and to quantify the asymmetrical loading of both legs during curve sprinting. Six male sprinters performed three linear and curve sprints. The energies of the ankle, knee and hip joint were determined during the ground contact phase with the aid of an adjusted multibody human model. The ankle joint was the largest energy absorber and generator in the sagittal plane while the hip joint was the largest energy absorber and generator in the frontal and transversal plane. Asymmetric functions of the inside and outside leg were determined during curve sprinting. The hip joint of the inside leg might be highly loaded in sprinting on a bend track

    Effects of triceps surae muscle strength and tendon stiffness on the reactive dynamic stability and adaptability of older female adults during perturbed walking

    This study aimed to examine whether the triceps surae (TS) muscle-tendon unit (MTU) mechanical properties affect gait stability and its reactive adaptation potential to repeated perturbation exposure in older adults. Thirty-four older adults each experienced eight separate unexpected perturbations during treadmill walking, while a motion capture system was used to determine the margin of stability (MoS) and base of support (BoS). Ankle plantar flexor muscle strength and Achilles tendon (AT) stiffness were analyzed using ultrasonography and dynamometry. A median split and separation boundaries classified the subjects into two groups with GroupStrong (n = 10) showing higher ankle plantar flexor muscle strength (2.26 +/- 0.17 vs. 1.47 +/- 0.20 N center dot m/kg, means +/- SD: P < 0.001) and AT stiffness (544 +/- 75 vs. 429 +/- 86 N/mm; P = 0.004) than GroupWeak in = 12). The first perturbation caused a negative Delta MoS (MoS in relation to unperturbed baseline walking) at touchdown of perturbed step (Pert(R)), indicating an unstable position. GroupStrong required four recovery steps to return to Delta MoS zero level, whereas GroupWeak was unable to return to baseline within the analyzed steps. However, after repeated perturbations, both groups increased Delta MoS at touchdown of Pert(R) with a similar magnitude. Significant correlations between Delta BoS and Delta MoS at touchdown of the first recovery step and TS MTU capacities (0.41 < r < 0.57: 0.006 < P < 0.048) were found. We conclude that older adults with TS muscle weakness have a diminished ability to control gait stability during unexpected perturbations, increasing their fall risk, but that degeneration in muscle strength and tendon stiffness may not inhibit the ability of the locomotor system to adapt the reactive motor response to repeated perturbations. NEW & NOTEWORTHY Triceps surae muscle weakness and a more compliant Achilles tendon partly limit older adults' ability to effectively enlarge the base of support and recover dynamic stability after an unexpected perturbation during walking, increasing their fail risk. However, the degeneration in muscle strength and tendon stiffness may not inhibit the ability of the locomotor system to adapt the reactive motor response to repeated perturbations

    Deutschsprachige Versionen des Tinnitus Functional Index

    Hintergrund Es existieren zwei deutschsprachige, validierte Versionen des Tinnitus Functional Index (TFI), eine für die Schweiz und eine für Deutschland. Der TFI gilt als möglicher neuer Standard-Fragebogen für die Evaluation eines Tinnitus-Schweregrads und einer Tinnitus-Therapie. Ziel der Arbeit In Anbetracht der stattfindenden Standardisierung bei der Tinnitus-Evaluation war es unser Ziel, die beiden TFI-Versionen miteinander zu vergleichen und im deutschsprachigem Raum nur eine TFI-Version zu empfehlen. Material und Methoden Die beiden deutschsprachigen TFI-Versionen wurden in einer multizentrischen randomisierten Online-Fragebogenstudie im Cross-over-Design miteinander verglichen. Ergebnisse Die Gesamtscores der beiden TFI-Versionen unterschieden sich in der gesamten Population nicht. Bei weiterer Aufschlüsselung in Bezug auf die Population und Reihenfolge der abgegeben TFI-Versionen zeigten sich allerdings teilweise signifikante Unterschiede mit jedoch nur moderaten Effektstärken. Dies deutet darauf hin, dass sich die beiden Versionen leicht unterscheiden, aber trotzdem miteinander vergleichbar sind. Bei der Faktoranalyse konnten bei der TFI-Version für Deutschland in der gesamten Population wie auch für die schweizerische Population 6 Faktoren extrahiert werden. Hingegen konnten bei der deutschen Population in beiden TFI-Versionen und bei der schweizerischen Population in der schweizerischen TFI-Version nur 5 Faktoren extrahiert werden. Schlussfolgerung Die beiden deutschsprachigen Versionen des TFI sind gut miteinander vergleichbar. Jedoch spricht die Faktoranalyse eher für die Verwendung der TFI-Version für Deutschland im gesamten deutschsprachigen Raum. ------- Background Two validated German-language versions of the Tinnitus Functional Index (TFI) exist, one for Switzerland and one for Germany. The TFI is considered to be a possible new standard questionnaire for evaluation of tinnitus severity and tinnitus treatment. Objective Considering the standardization taking place in tinnitus evaluation, our aim was to compare the two German-language TFI versions and to recommend only one TFI version in the German-speaking area. Materials and methods The two German-language TFI versions were compared in a multicenter and randomized online questionnaire study with a crossover design. Results The total score of the two TFI versions did not differ in the total population. However, when further divided in terms of population and order of presentation of the TFI versions, there were significant differences in some cases, albeit with only moderate effect sizes. This suggests that the two versions are slightly different but still comparable. In factor analysis for the TFI version for Germany, in the entire population as well as in the Swiss population, six factors could be extracted. In contrast, for the German and Swiss TFI versions, only five factors could be extracted in the German population, and for the Swiss TFI version, only five factors in the Swiss population. Conclusion The two German-language versions of the TFI are well comparable with each other. However, the factor analysis rather argues for use of the TFI version for Germany in the entire German-speaking region

    Serotype Distribution and Invasive Potential of Group B Streptococcus Isolates Causing Disease in Infants and Colonizing Maternal-Newborn Dyads

    Serotype-specific polysaccharide based group B streptococcus (GBS) vaccines are being developed. An understanding of the serotype epidemiology associated with maternal colonization and invasive disease in infants is necessary to determine the potential coverage of serotype-specific GBS vaccines.Colonizing GBS isolates were identified by vaginal swabbing of mothers during active labor and from skin of their newborns post-delivery. Invasive GBS isolates from infants were identified through laboratory-based surveillance. GBS serotyping was done by latex agglutination. Serologically non-typeable isolates were typed by a serotype-specific PCR method. The invasive potential of GBS serotypes associated with sepsis within seven days of birth was evaluated in association to maternal colonizing serotypes.GBS was identified in 289 (52.4%) newborns born to 551 women with GBS-vaginal colonization and from 113 (5.6%) newborns born to 2,010 mothers in whom GBS was not cultured from vaginal swabs. The serotype distribution among vaginal-colonizing isolates was as follows: III (37.3%), Ia (30.1%), and II (11.3%), V (10.2%), Ib (6.7%) and IV (3.7%). There were no significant differences in serotype distribution between vaginal and newborn colonizing isolates (P = 0.77). Serotype distribution of invasive GBS isolates were significantly different to that of colonizing isolates (P<0.0001). Serotype III was the most common invasive serotype in newborns less than 7 days (57.7%) and in infants 7 to 90 days of age (84.3%; P<0.001). Relative to serotype III, other serotypes showed reduced invasive potential: Ia (0.49; 95%CI 0.31-0.77), II (0.30; 95%CI 0.13-0.67) and V (0.38; 95%CI 0.17-0.83).In South Africa, an anti-GBS vaccine including serotypes Ia, Ib and III has the potential of preventing 74.1%, 85.4% and 98.2% of GBS associated with maternal vaginal-colonization, invasive disease in neonates less than 7 days and invasive disease in infants between 7-90 days of age, respectively

    Dysregulated innate and adaptive immune responses discriminate disease severity in COVID-19

    The clinical spectrum of COVID-19 varies and the differences in host response characterizing this variation have not been fully elucidated. COVID-19 disease severity correlates with an excessive pro-inflammatory immune response and profound lymphopenia. Inflammatory responses according to disease severity were explored by plasma cytokine measurements and proteomics analysis in 147 COVID-19 patients. Furthermore, peripheral blood mononuclear cell cytokine production assays and whole blood flow cytometry were performed. Results confirm a hyperinflammatory innate immune state, while highlighting hepatocyte growth factor and stem cell factor as potential biomarkers for disease severity. Clustering analysis reveals no specific inflammatory endotypes in COVID-19 patients. Functional assays reveal abrogated adaptive cytokine production (interferon-gamma, interleukin-17 and interleukin-22) and prominent T cell exhaustion in critically ill patients, whereas innate immune responses were intact or hyperresponsive. Collectively, this extensive analysis provides a comprehensive insight into the pathobiology of severe to critical COVID-19 and highlight potential biomarkers of disease severity
